Pharmacological action
Mucolytic agent
Indications
Acetylcysteine is used for the treatment of respiratory diseases accompanied by the formation of viscous sputum that is difficult to separate:
• acute and chronic
bronchitis• * obstructive bronchitis;
• tracheitis;
* laryngotracheitis
• * pneumonia•
* lung abscess
• * bronchiolitis;
* bronchiectatic disease•
* bronchial asthma;
• cystic
fibrosis• * chronic obstructive pulmonary disease (COPD);
• acute and chronic sinusitis;
• inflammation of the middle ear (otitis media).
Contraindications
– Hypersensitivity to acetylcysteine and other components of the drug;
– Deficiency of sucrase/isomaltase, fructose intolerance, glucose-galactose malabsorption;
Phenylketonuria;
– Children’s age up to 14 years (for a dosage of 600 mg), children’s age up to 6 years (for a dosage of 200 mg), children’s age up to 2 years (for a dosage of 100 mg);
– Pregnancy and lactation.
With caution
Bronchial asthma, hepatic and/or renal insufficiency, adrenal diseases, esophageal varicose veins, arterial hypertension, tendency to pulmonary bleeding, hemoptysis, gastric and duodenal ulcers in the anamnesis, histamine intolerance (long-term use of the drug should be avoided, since acetylcysteine affects the metabolism of histamine and may lead to signs of intolerance, such as headache, headache). vasomotor rhinitis, pruritus).

Nervous system disorders:  very rarely – headache.

Respiratory system disorders: Â rarely-shortness of breath, bronchospasm (mainly in patients with hyperreactivity of the bronchi in bronchial asthma).
From the gastrointestinal tract: infrequently-stomatitis, abdominal pain, nausea, vomiting, diarrhea, heartburn, dyspepsia.
From the side of the senses: infrequently-tinnitus.
Other: very rare – fever, isolated reports of bleeding due to hypersensitivity reactions, decreased platelet aggregation.
Interaction
With the simultaneous use of acetylcysteine and antitussive agents, mucus congestion may occur due to suppression of the cough reflex. Therefore, such combinations should be chosen with caution.
Pharmacologically incompatible with antibiotics (penicillins, cephalosporins, erythromycin, tetracycline and amphotericin B) and proteolytic enzymes.
Reduces the absorption of penicillins, cephalosporins, tetracycline (they should be taken no earlier than 2 hours after ingestion of acetylcysteine).
Simultaneous use of acetylcysteine and nitroglycerin can lead to increased vasodilating effect of the latter.
Upon contact with metals and rubber, sulfides with a characteristic odor are formed.
How to take, course of use and dosage
Inside. The pellets should be dissolved in water at room temperature and taken after meals.

In the absence of other prescriptions, it is recommended to adhere to the following dosages: :
Mucolytic therapy.
Adults and adolescents over 14 years of age: it is recommended to take 2 sachets of Acetylcysteine Canon 100 mg or 1 sachet of Acetylcysteine Canon 200 mg 2-3 times a day (400-600 mg per day).
Children aged 6 to 14 years: it is recommended to take 1 sachet 3 times a day or 2 sachets 2 times a day of the drug Acetylcysteine Canon 100 mg (300-400 mg per day). The drug Acetylcysteine Canon 200 mg should be taken 1/2 sachet 3 times a day or 1 sachet 2 times a day (300-400 mg per day).
Children aged 2 to 6 years: it is recommended to take 1 sachet of the drug Acetylcysteine Canon 100 mg 2-3 times a day (200-300 mg per day).
Cystic fibrosis.
Children over the age of 6 years: it is recommended to take 2 sachets of Acetylcysteine Canon 100 mg or 1 sachet of Acetylcysteine Canon 200 mg 3 times a day (600 mg per day).
Children aged 2 to 6 years: it is recommended to take 1 sachet of the drug Acetylcysteine Canon 100 mg 4 times a day (400 mg per day). Patients with cystic fibrosis and a body weight of more than 30 kg, if necessary, can increase the dose to 800 mg of acetylcysteine per day.
Additional fluid intake enhances the mucolytic effect of the drug. For short-term colds, the duration of admission is 5-7 days. For long-term diseases, the duration of therapy is determined by the attending physician. For chronic bronchitis and cystic fibrosis, the drug should be taken for a longer time to achieve a preventive effect in infections.
Overdose
Symptoms: Â In case of accidental or deliberate overdose, such phenomena as diarrhea, vomiting, stomach pain, heartburn and nausea are observed. So far, no serious or life-threatening side effects have been observed.
Treatment: Â Symptomatic.
Special instructions
When treating patients with diabetes mellitus, it should be taken into account that the drug contains sucrose (1 sachet of Acetylcysteine Canon 600 mg corresponds to 0.19 XE,200 mg – 0.23 XE,100 mg – 0.24 XE).
When working with the drug, it is necessary to use glassware, avoid contact with metals, rubber, oxygen, and easily oxidizing substances.
Severe allergic reactions such as Stevens-Johnson syndrome and toxic epidermal necrolysis (Lyell’s syndrome) have been reported very rarely with acetylcysteine. If you experience changes in the skin and mucous membranes, you should immediately consult a doctor, stop taking the drug.
Do not take the drug immediately before bedtime (it is recommended to take the drug before 18.00).
