Pharmacological action
Pharmacological Group:
Anti-allergic agent – H1-histamine receptor blocker.
Pharmacological action: Â
Aerius is a long-acting antihistamine, a blocker of peripheral histamine H1 receptors. Desloratadine is the primary active metabolite of loratadine.
Inhibits the cascade of reactions of allergic inflammation, including the release of proinflammatory cytokines, including interleukins IL-4, IL-6, IL-8, IL-13, release of proinflammatory chemokines, the production of superoxide anion by activated polymorphonuclear neutrophils, adhesion and chemotaxis of eosinophils, release of adhesion molecules such as P-selectin, IgE – mediated release of histamine, prostaglandin D2, and of leukotriene C 4.
Thus, it prevents the development and facilitates the course of allergic reactions, has an antipruritic and antiexudative effect, reduces capillary permeability, prevents the development of tissue edema, smooth muscle spasm.
The drug has no effect on the central nervous system, has practically no sedative effect (does not cause drowsiness) and does not affect the speed of psychomotor reactions. It does not cause prolongation of the QT interval on the ECG.
The action of Aerius begins within 30 minutes after ingestion and continues for 24 hours.
Indications
-Seasonal (pollinosis) and year-round allergic rhinitis (elimination or relief of sneezing, nasal congestion, mucus discharge from the nose, itching in the nose, itching of the palate, itching and redness of the eyes, lacrimation).
– Chronic idiopathic urticaria (reduction or elimination of skin itching, rash).
Use during pregnancy and lactation
The use of the drug during pregnancy is not recommended due to the lack of clinical data on the safety of its use during this period.
Desloratadine is excreted in breast milk, so the use of Aerius during breastfeeding is not recommended.
Contraindications
-hypersensitivity to the components of the drug;Â
– pregnancy;
– lactation (breastfeeding);
– age up to 12 years;
– hereditary diseases (fructose intolerance, glucose/galactose malabsorption or sucrose/isomaltase deficiency in the body) due to the presence of sucrose and sorbitol in the syrup.
With caution: the drug should be used in severe renal insufficiency.
Side effects
When using the drug, increased fatigue (1.2%), dry mouth (0.8%), headache (0.6%) were noted.
From the cardiovascular system: very rarely – tachycardia, palpitations.
From the digestive system: very rarely – increased activity of liver enzymes, increased bilirubin concentration.
Allergic reactions: very rare-anaphylaxis, rash.
Interaction
Aerius does not enhance the effects of ethanol on the central nervous system.
Food intake does not affect the effectiveness of the drug.
How to take, course of use and dosage
Adults and adolescents aged 12 years and older are prescribed 5 mg 1 time a day, regardless of food intake, preferably at the same time of day.
The tablet should be swallowed whole, without chewing, with a small amount of water.
Overdose
Symptoms: Taking a dose 5 times higher than the recommended dose did not cause any symptoms. In clinical trials, daily use of desloratadine in adults and adolescents at a dose of up to 20 mg for 14 days was not accompanied by statistically or clinically significant changes in the cardiovascular system.
In a clinical and pharmacological study, the use of desloratadine at a dose of 45 mg/day (9 times higher than recommended) for 10 days did not cause prolongation of the QT interval and was not accompanied by the appearance of serious side effects.
Treatment: in case of accidental ingestion of a large amount of the drug-gastric lavage, taking activated charcoal; if necessary – symptomatic therapy.
Desloratadine is not eliminated by hemodialysis, and the effectiveness of peritoneal dialysis has not been established.

Antihistaminic, anti-inflammatory, and antiallergic properties of the nonsedating second-generation antihistamine desloratadine: a review of the evidence. World Allergy Organ J. 2011 Feb;4(2): 47-53.2. For example, when fexofenadine is co-administered with erythromycin or ketoconazole, the concentration of fexofenadine in plasma increases, aluminum-or magnesium-containing antacids reduce the bioavailability of fexofenadine (instructions for use of Allegra); concomitant use of cetirizine with alcohol or drugs that depress the central nervous system may further reduce concentration and reaction speed (instructions for use of Zyrtek).3. Concomitantly with alcohol should be used with caution.
Special instructions
The efficacy and safety of Aerius syrup in children under 1 year of age has not been established. In most cases, rhinitis in children under 2 years of age is infectious in nature.
Studies of the effectiveness of Aerius in rhinitis of infectious etiology have not been conducted.
Differential diagnosis between allergic rhinitis and rhinitis of other origin in children under 2 years of age presents certain difficulties.
When making a differential diagnosis, you should pay attention to the presence or absence of foci of infection or structural abnormalities of the upper respiratory tract, conduct a thorough medical history, examination, as well as appropriate laboratory tests and skin tests.
At the recommended dose, Aerius does not affect the ability to drive vehicles or operate mechanisms.
