Pharmacological action
Pharmacodynamics . Almagel A is a balanced combination of aluminum hydroxide and magnesium hydroxide combined with sorbitol. The drug Almagel A has a moderate antacid effect when used in the recommended single and daily dose for 40-60 minutes after meals. Aluminum hydroxide neutralizes the increased secretion of hydrochloric acid and reduces the activity of pepsin in the stomach, forming aluminum chloride. Under the influence of the alkaline environment of the intestine, the latter is converted into alkaline aluminum salts, which are almost not absorbed and slightly change the concentration of aluminum salts in the blood with prolonged use of Almagel A. On the other hand, aluminum hydroxide has the property of changing the concentration of phosphates, binding phosphate ions in the intestine and thus limiting their absorption. Antacids containing aluminum, including Almagel A, also have a known cytoprotective effect on the gastric mucosa associated with the activation of prostaglandin synthesis. This increases the resistance of the mucous membrane, which protects it from inflammatory-necrotic and erosive-hemorrhagic changes caused by irritating and ulcerogenic agents, such as acetylsalicylic acid, NSAIDs, and ethyl alcohol. Magnesium hydroxide also neutralizes hydrochloric acid in the stomach, turning into magnesium chloride, which has a minor laxative effect. Benzocaine has a local analgesic effect in the presence of severe pain syndrome. Sorbitol has a mild carminative and moderate choleretic and laxative effect. Such effects compensate for the tendency of most patients to constipation under the influence of aluminum hydroxide. The drug does not lead to the development of alkalosis and the formation of carbon dioxide (CO2) in the stomach. Pharmacokinetics. Aluminum salts are absorbed to a small extent in the intestines. Magnesium ions are absorbed only by 10% and their concentration in the blood is almost unchanged. The duration of action depends on the rate of gastric emptying. When applied on an empty stomach, the action lasts 20-60 minutes. Benzocaine is absorbed in minimal amounts and has virtually no systemic effect on the body. Its local analgesic effect occurs 1-2 minutes after the suspension is applied.
Indications
-peptic ulcer of the stomach and duodenum in the acute phase; – acute gastritis, chronic gastritis with increased and normal secretory function in the acute phase; duodenitis, enteritis;- hiatal hernia, reflux esophagitis;- functional bowel disorders, colitis;- discomfort and pain in the epigastrium due to errors in the diet, after drinking coffee, nicotine, alcohol;- prophylactically in the treatment of corticosteroids and NSAIDs. In the complex of therapeutic measures Almagel A is prescribed for diabetic patients.
Use during pregnancy and lactation
During pregnancy, it can be used for no longer than 3 days. Breast-feeding should be discontinued for the duration of treatment.
Recommendations for use
For adults. 10 ml (1 sachet) 3-4 times a day for 10-15 minutes before meals. The maximum duration of treatment is 7 days, then switch to treatment with Almagel. Before use, the suspension must be thoroughly homogenized by kneading the sachet!
Contraindications
-hypersensitivity to the components of the drug; – severe renal impairment;— Alzheimer’s disease— – early childhood (up to 1 month). Do not prescribe simultaneously with sulfonamides due to the presence of anesthetic in the preparation!
Side effects
when using the drug in the recommended doses, side effects are rare. On the part of the digestive system: constipation (passes after reducing the dose), nausea, vomiting, stomach cramps, changes in taste sensations. From the immune system: hypersensitivity reactions, including skin rashes, pruritus, urticaria, angioedema and anaphylactic reactions. Metabolic and alimentary disorders: hypermagnesemia, hyperaluminemia, hypophosphatemia. In patients with renal insufficiency and patients undergoing dialysis, long-term use of high-dose aluminum and magnesium salts may lead to the development of encephalopathy, dementia, microcytic anemia, or worsen the course of dialysis-induced osteomalacia, and due to the development of hypophosphatemia, lead to increased bone resorption processes and hypercalciuria with an increased risk of osteomalacia.
Interaction
It can absorb some medications, thus reducing their absorption, so when taking other medications at the same time, they should be taken 1-2 hours before or after taking Almagel® A. Almagel® A reduces the acidity of gastric juice, and this can affect the effect of a large number of medications when taken simultaneously. Almagel® Reduces the effect of histamine H2-receptor blockers (cimetidine, ranitidine, famotidine), cardiac glycosides, iron salts, lithium preparations, quinidine, mexiletine, phenothiazine preparations, tetracycline antibiotics, ciprofloxacin, isoniazid and ketoconazole. When taking enteric drugs at the same time, an increased pH of gastric juice can lead to accelerated disruption of the US membrane and cause irritation of the stomach and duodenum. Almagel can affect the results of some laboratory and functional tests and tests: it reduces the level of gastric secretion when determining its acidity; changes the results of tests using technetium (TC₉₉), for example, bone scintigraphy and some tests for studying the esophagus; increases serum phosphorus levels, serum pH values and urine.
How to take, course of use and dosage
Treatmentadults with 5-10 ml (1-2 scoops) or 1 sachet 3-4 times a day 10-15 minutes before meals. Overdose
If the dose is exceeded once, there are no other signs of overdose other than constipation, flatulence, and a metallic taste sensation. With prolonged use of high doses, nephrocalcinosis, severe constipation, mild drowsiness, hypermagnesemia are possible. There may also be signs of metabolic alkalosis: changes in mood or mental activity, numbness or pain in the muscles, irritability and rapid fatigue, slow breathing, unpleasant taste sensations. In these cases, it is necessary to immediately take measures to quickly remove the drug from the body(induction of vomiting, taking activated charcoal).
Special instructions
The interval between taking Almagel A and other medications should be 1-2 hours. It is not recommended to take the drug in doses exceeding 16 tablespoons per day, or, if such a dose is used, the duration of treatment should not exceed 2 weeks. When taking the drug for a long time, it is necessary to ensure sufficient intake of phosphorus with food.
