Almagel® is a drug that is a balanced combination of algeldrate (aluminum hydroxide) and magnesium hydroxide. It neutralizes free hydrochloric acid in the stomach, reduces the activity of pepsin, which leads to a decrease in the digesting activity of gastric juice.
It has an enveloping, adsorbing effect. Protects the gastric mucosa by stimulating the synthesis of prostaglandins (cytoprotective effect). This protects the mucosa from inflammatory and erosive-hemorrhagic lesions resulting from the use of irritating and ulcerogenic agents, such as ethyl alcohol and non-steroidal anti-inflammatory drugs (for example, Indometacin, diclofenac, aspirin, acetylsalicylic acid, corticosteroids). The therapeutic effect after taking the drug occurs in 3-5 minutes.
The duration of action depends on the rate of gastric emptying. When taken on an empty stomach, the action lasts up to 60 minutes. When taken an hour after a meal, the antacid effect can last up to 3 hours. It does not cause secondary hypersecretion of gastric juice.

Indications
Treatment
- Acute gastritis, chronic gastritis with increased and normal secretory function of the stomach (in the phase of exacerbation); acute duodenitis, enteritis, colitis;

- heartburn and epigastric pain after errors in nutrition, excessive use of ethanol, nicotine, coffee, taking medications that irritate the gastric mucosa.

Use during pregnancy and lactation

Contraindications

Side effects
Almagel can cause constipation, which resolves after reducing the dose.
In rare cases, nausea, vomiting, stomach cramps, changes in taste sensations, allergic reactions and hypermagnesemia (increased levels of magnesium in the blood) are observed.
With prolonged use of the drug in patients with renal insufficiency and on dialysis, mood and mental activity may change; With prolonged use of high doses of the drug, along with a lack of phosphorus in food, osteomalacia may occur.
Interaction
It can absorb some medications, thus reducing their absorption, so when taking other medications at the same time, they should be taken 1-2 hours before or after taking Almagel.
Almagel reduces the acidity of gastric juice, and this can affect the effect of a large number of medications when taken simultaneously.
Almagel reduces the effect of histamine H2-receptor blockers (cimetidine, ranitidine, famotidine), cardiac glycosides, iron salts, lithium preparations, quinidine, mexiletine, phenothiazine preparations, tetracycline antibiotics, ciprofloxacin, isoniazid and ketoconazole.
When taking enteric drugs at the same time, an increased pH of gastric juice can lead to accelerated disruption of the US membrane and cause irritation of the stomach and duodenum. Almagel can affect the results of some laboratory and functional tests and tests: it reduces the level of gastric secretion when determining its acidity; changes the results of tests using technetium (TS??), for example, bone scintigraphy and some tests for studying the esophagus, increases serum phosphorus levels, serum pH values and urine.
How to take, course of use and dosage
Treatment Adults and children over 15 years of age: 5-10 ml (1-2 scoops) or 1 sachet 3-4 times a day. If necessary, the single dose can be increased to 15 ml (3 scoops). Children from 10 to 15 years old Apply in a dose equal to half the dose for adults-1 measuring spoon 2-4 times a day or 2 measuring spoons 1-2 times a day or 1 sachet 1-2 times a day. The drug is taken 45-60 minutes after a meal and in the evening before going to bed. After achieving the therapeutic effect, the daily dose is reduced to 5 ml (1 measuring spoon) 3-4 times a day or 1 sachet 1-2 times a day for 15-20 days. It is not recommended to take liquids within 15 minutes after taking Almagel. Before use, the suspension must be thoroughly homogenized by shaking the bottle or kneading and shaking the sachet. For prevention,5-15 ml (1-3 scoops) or 1 sachet 15 minutes before taking medications with an irritating effect.
Overdose
If the dose is exceeded once, there are no other signs of overdose other than constipation, flatulence, and a metallic taste in the mouth.
With prolonged use of high doses, the formation of kidney stones, severe constipation, mild drowsiness, hypermagnesemia are possible.
There may also be signs of metabolic alkalosis: changes in mood or mental activity, numbness or pain in the muscles, irritability and rapid fatigue, slow breathing, unpleasant taste sensations.
In these cases, it is necessary to immediately take measures to quickly remove the drug from the body – gastric lavage, stimulation of vomiting, taking activated charcoal.
Special instructions

With prolonged use of the drug (more than 20 days), regular medical monitoring of serum magnesium levels is necessary in the treatment of patients with renal insufficiency.
