Pharmacological action

Amlovas – antispasmodic, antihypertensive, vasodilating, antianginal.

Blocks the flow of calcium ions through cell membranes to smooth muscle cells of the myocardium and blood vessels. It has a direct relaxing effect on vascular smooth muscles, dilates peripheral arterioles, which leads to a decrease in OPSS, reduces the load on the heart and reduces the need for oxygen in the myocardium, expands the coronary arteries, increasing oxygen supply to the myocardium.

Pharmacokinetics

After oral use, it is rapidly and almost completely (90%) absorbed from the gastrointestinal tract. Bioavailability is 60-65%, plasma cmax is reached in 6-12 hours, and steady-state concentration is reached in 7-8 days.

The volume of distribution is about 20 l / kg, binding to plasma proteins is more than 95%, penetrates through the BBB and into breast milk. It is mainly metabolized in the liver with the formation of inactive metabolites, T_1/2 — 35-45 h. It is excreted by the kidneys (10% — unchanged and 60% – in the form of metabolites), as well as with bile and through the intestines.

Indications

Arterial hypertension (monotherapy and in combination with thiazide diuretics, ACE inhibitors or alpha-and beta-blockers), angina pectoris (stable and unstable, including Prinzmetal or variant), dilated (non-ischemic) cardiomyopathy with severe chronic heart failure.

Contraindications

Hypersensitivity, hypotension.

Side effects

From the nervous system and sensory organs: dizziness, headache, drowsiness, fatigue; rarely-mood changes, asthenia, convulsions, myalgia, visual impairment, paresthesia.

Cardiovascular and blood disorders (hematopoiesis, hemostasis): palpitations, hot flashes.

From the digestive tract: nausea, abdominal pain, dyspeptic phenomena; rarely-gum hyperplasia, increased liver transaminases, jaundice.

From the genitourinary system: increased urination, edema, impotence.

From the skin: allergic reactions (skin rash, pruritus), rarely-erythema multiforme.

Other: rarely-shortness of breath, gynecomastia.

How to take, course of use and dosage

Inside, the initial dose is 5 mg / day, if necessary, the daily dose can be increased to 10 mg. The maximum daily dose is 10 mg.

Overdose

Symptoms: severe hypotension.

Treatment: maintenance of the cardiovascular system, monitoring of the heart and lungs, elevated position of the limbs, control of BCC and diuresis, use of vasoconstrictors, use of calcium gluconate (IV).

Special instructions

It is necessary to refrain from using the drug in pediatric practice, since the safety and effectiveness of its use in children have not been determined.

Caution should be used in patients with impaired liver function, aortic stenosis, or chronic heart failure.
