Composition

1 tablet contains amoxicillin (in the form of trihydrate) 500 mg

Pharmacological action

Amosin is a broad-spectrum antibiotic of the semisynthetic penicillin group. It acts bactericidal. It inhibits transpeptidase, disrupts the synthesis of peptidoglycan (the cell wall support polymer) during division and growth, and causes bacterial lysis. Acid-resistant.

Active against aerobic gram-positive bacteria: Staphylococcus spp. (except for penicillinase-producing strains), Streptococcus spp. ; aerobic gram-negative bacteria: Neisseria gonorrhoeae, Neisseria meningitidis, Bacillus anthracis, Listeria monocytogenes, Helicobacter pylori, Klebsiella spp.

Penicillinase-producing microorganisms are resistant to amoxicillin. The action develops in 15-30 minutes after use and lasts for 8 hours

Indications

Contraindications

Side effects

Allergic reactions: possibly-urticaria, skin hyperemia, erythema, angioedema, rhinitis, conjunctivitis; rarely-fever, joint pain, eosinophilia, exfoliative dermatitis, erythema multiforme, Stevens-Johnson syndrome, reactions similar to serum sickness; in isolated cases – anaphylactic shock.

From the digestive system:  dysbiosis, taste changes, vomiting, nausea, diarrhea, stomatitis, glossitis, impaired liver function, moderate increase in the activity of hepatic transaminases; rarely – pseudomembranous enterocolitis.

From the central nervous system and peripheral nervous system:  agitation, anxiety, insomnia, ataxia, confusion, behavior changes, depression, peripheral neuropathy, headache, dizziness, convulsive reactions.

From the urinary system:  rarely-interstitial nephritis.

From the hematopoietic system:  leukopenia, neutropenia, thrombocytopenic purpura, anemia.

Other services:  shortness of breath, tachycardia, vaginal candidiasis, superinfection (especially in patients with chronic diseases or reduced body resistance).

Interaction

Antacids, glucosamine, laxatives, food, antibiotics of the aminoglycoside group, when used simultaneously, slow down and reduce the absorption of amoxicillin; ascorbic acid increases its absorption.

Bactericidal antibiotics (including aminoglycosides, cephalosporins, cycloserine, vancomycin, rifampicin) show synergy with amoxicillin; bacteriostatic drugs (macrolides, chloramphenicol, lincosamides, tetracyclines, sulfonamides) – antagonism.

Amoxicillin, when used simultaneously, increases the effectiveness of indirect anticoagulants (suppressing the intestinal microflora, reduces the synthesis of vitamin K and the prothrombin index); reduces the effectiveness of estrogen-containing oral contraceptives, ethinyl estradiol (the risk of intermenstrual bleeding), as well as drugs that produce para-aminobenzoic acid during metabolism.

Diuretics, allopurinol, oxifenbutazone, phenylbutazone, NSAIDs and drugs that block tubular secretion, when used simultaneously with Amosin, reduce tubular secretion, increase the concentration of amoxicillin.

Concomitant use of Amosine with allopurinol increases the risk of skin rash.

Amoxicillin, when used concomitantly, reduces the clearance and increases the toxicity of methotrexate.

Amoxicillin, when used concomitantly, increases the absorption of digoxin.

How to take, course of use and dosage

Amosin is taken orally, before or after a meal. The dosage regimen is set individually, taking into account the severity of the disease, the sensitivity of the pathogen to the drug, and the patient’s age.

Adults and children over 10 years of age (with a body weight >40 kg) are prescribed 500 mg 3 times a day, with a severe course of the disease-0.75-1 g 3 times a day.

Children aged 5 to 10 years are prescribed 250 mg 3 times a day, at the age of 2 to 5 years – 125 mg 3 times a day, at the age of 2 years-20 mg / kg / day in 3 doses. The course of treatment is 5-12 days. For children under 5 years of age, the drug is prescribed in the form of a suspension.

For the treatment of acute uncomplicated gonorrhea Amosin is prescribed at a dose of 3 g once; in the treatment of women, it is recommended to repeat the specified dose.

For acute infectious diseases of the gastrointestinal tract (paratyphoid, typhoid fever) and biliary tract, for gynecological infectious diseases, adults are prescribed 1.5-2 g 3 times a day or 1-1.5 g 4 times a day.

With leptospirosis, adults are prescribed 500-750 mg 4 times a day for 6-12 days.

In case of Salmonella transmission to adults-1.5-2 g 3 times a day for 2-4 weeks.

For the prevention of endocarditis with minor surgical interventions, adults are prescribed a dose of 3-4 g 1 hour before the procedure. If necessary, a second dose is prescribed after 8-9 hours. In children, the dose is halved.

Overdose

Symptoms:  nausea, vomiting, diarrhea, water-electrolyte balance disorders (as a result of vomiting and diarrhea).

Treatment:  gastric lavage, activated charcoal, saline laxatives; measures aimed at maintaining water and electrolyte balance; hemodialysis.

Special instructions

During the course of treatment with Amosin, it is necessary to monitor the state of function of the hematopoietic organs, liver and kidneys.

It is possible to develop superinfection due to the growth of microflora insensitive to amoxicillin, which requires a corresponding change in antibacterial therapy.

When treating patients with bacteremia, a bacteriolysis reaction (Yarish-Gerksheimer reaction) rarely develops.

In patients with hypersensitivity to penicillins, cross-allergic reactions with cephalosporin antibiotics are possible.

In the treatment of mild diarrhea with Amosin, antidiarrheal medications that reduce intestinal motility should be avoided; kaolin-or attapulgite-containing antidiarrheal agents can be used. In case of severe diarrhea, it is necessary to conduct a differential diagnosis and prescribe appropriate therapy.

Treatment should be continued for another 48-72 hours after the disappearance of clinical signs of the disease.

When using estrogen-containing oral contraceptives and amoxicillin at the same time, additional methods of contraception should be used, if possible.
