Pharmacological action
Participates in the regulation of redox processes, carbohydrate metabolism, blood clotting, tissue regeneration, in the synthesis of steroid hormones; increases the body’s resistance, reduces vascular permeability, reduces the need for vitaminsB1, B2, A, E, folic acid, pantothenic acid. It satisfies the body’s increased needs for vitamin C in diseases accompanied by fever.
Indications
Prevention and treatment of hypo-and vitamin C deficiency;
As an auxiliary agent: hemorrhagic diathesis, nasal, uterine, pulmonary and other bleeding, including those caused by radiation sickness; overdose of anticoagulants, sluggish healing of wounds and bone fractures, dystrophy, increased physical and mental stress, during recovery from severe, long-term diseases.
Use during pregnancy and lactation
The minimum daily requirement for ascorbic acid in the II-III trimesters of pregnancy is about 60 mg.
The minimum daily requirement during breastfeeding is 80 mg. A mother’s diet containing an adequate amount of ascorbic acid is sufficient to prevent vitamin C deficiency in an children (it is recommended that the nursing mother does not exceed the maximum monthly requirement for ascorbic acid).
Contraindications
Hypersensitivity to the components of the drug, thrombophlebitis, tendency to thrombosis, diabetes mellitus, kidney stone disease, childhood.
With caution:
Hyperoxalaturia, renal failure, hemochromatosis, thalassemia, polycythemia, leukemia, sideroblastic anemia, glucose-6-phosphate dehydrogenase deficiency, sickle cell anemia, progressive malignant diseases, pregnancy.
Side effects
From the central nervous system (CNS): headache, fatigue, with prolonged use of large doses – increased excitability of the central nervous system, sleep disorders.
From the digestive system: irritation of the gastrointestinal mucosa, nausea, vomiting, diarrhea, stomach cramps.
From the endocrine system: inhibition of the function of the insular apparatus of the pancreas (hyperglycemia, glucosuria).
From the urinary system: when used in high doses –hyperoxalaturia and the formation of urinary stones from calcium oxalate.
From the cardiovascular system: thrombosis, when used in high doses – increased blood pressure, the development of microangiopathies, myocardial dystrophy.
Allergic reactions: skin rash, rarely-anaphylactic shock.
Laboratory parameters: thrombocytosis, hyperprothrombinemia, erythropenia, neutrophilic leukocytosis, hypokalemia.
Other: hypervitaminosis, fever, prolonged use of high doses – sodium (Na+) and fluid retention, impaired metabolism of zinc (Zn2+), copper (Cu2+).
Intensive consumption of chewable tablets or resorption of oral forms can cause damage to tooth enamel.
Interaction
Increases the concentration of benzylpenicillin and tetracyclines in the blood; at a dose of 1 g/day, it increases the bioavailability of ethinyl estradiol. Improves intestinal absorption of iron preparations (converts trivalent iron to bivalent); may increase iron excretion when used concomitantly with deferoxamine.
Acetylsalicylic acid (ASA), oral contraceptives, fresh juices and alkaline drinks reduce absorption and absorption. When used concomitantly with ASA, the urinary excretion of ascorbic acid increases and the excretion of ASA decreases.
ASA reduces the absorption of ascorbic acid by about 30%. Increases the risk of crystalluria during treatment with short-acting salicylates and sulfonamides, slows down the excretion of acids by the kidneys, increases the excretion of drugs that have an alkaline reaction (including alkaloids), reduces the concentration of oral contraceptives in the blood.
Increases the total clearance of ethanol, which, in turn, reduces the concentration of ascorbic acid in the body. Preparations of the quinoline series (fluoroquinolones, etc. ), calcium chloride, salicylates, glucocorticosteroids with prolonged use deplete the reserves of ascorbic acid. When used concomitantly, it reduces the chronotropic effect of isoprenaline.
With prolonged use or use in high doses, the disulfiram-ethanol interaction may be disrupted. In high doses, it increases the renal excretion of mexiletine. Barbiturates and primidone increase the excretion of ascorbic acid in the urine.
Reduces the therapeutic effect of antipsychotics (phenothiazine derivatives), tubular reabsorption of amphetamine and tricyclic antidepressants.
Reduces the effectiveness of heparin, indirect anticoagulants.
How to take, course of use and dosage
The drug is taken orally after a meal. The tablet should be chewed and washed down with a small amount of water.
For prevention: adults 200 mg (1 tablet) per day.
For medicinal purposes: adults take 1-2 tablets 1-2 times a day (the maximum therapeutic dose is -1 g per day).
Overdose
If you take more than 1 g per day, heartburn, diarrhea, difficulty urinating and/or staining the urine red, hemolysis (in patients with glucose-6-phosphate dehydrogenase deficiency) are possible.
If any side effects occur, you should stop taking the drug and consult a doctor.
Special instructions
Due to the stimulating effect of ascorbic acid on the synthesis of corticosteroid hormones, it is necessary to monitor the function of the adrenal glands and blood pressure.
With prolonged use of large doses, the function of the pancreatic insular apparatus may be inhibited, so it should be regularly monitored during treatment.
In patients with high iron content in the body, ascorbic acid should be used in minimal doses. Prescribing ascorbic acid to patients with rapidly proliferating and intensely metastatic tumors may worsen the course of the process.
Ascorbic acid, as a reducing agent, can distort the results of various laboratory tests (blood glucose, bilirubin, transaminase activity, LDH).
Influence on the ability to drive vehicles and mechanisms
The use of the drug in accordance with the instructions does not adversely affect the psychomotor functions associated with driving a car or controlling machines and mechanisms.
With prolonged use of the drug in high doses, side effects such as headache, increased excitability of the central nervous system, increased blood pressure may occur. In this case, care should be taken when driving vehicles and mechanisms that require concentration of attention and speed of psychomotor reactions.
