Composition
1 g ointment for external use contains: Active ingredient: Â betamethasone (in the form of dipropionate) – 0.640 mg, which corresponds to 0.500 mg of betamethasone, salicylic acid-30 mg; excipients: Â mineral oil, vaseline.
Pharmacological action
A combined drug, the action of which is determined by its components; it has anti-inflammatory, anti-edematous, anti-allergic, vasoconstrictor, antiproliferative, immunosuppressive, keratolytic, antimicrobial, antifungal and local hypothermic effects.
Inhibits the release of inflammatory mediators, prevents the marginal accumulation of neutrophils, reduces inflammatory exudate and cytokine production, reduces macrophage migration, leading to a decrease in infiltration and granulation processes.
Thanks to the presence of salicylic acid, it softens the skin, eliminates stratum corneum and promotes deeper penetration of corticosteroids.
The ointment has a water-repellent effect and forms a protective film that protects the skin from external moisture; having a deep fat-saturating effect, it is most suitable for treating patients with dry and fragile skin.
The lotion (fat-free solution) is easily distributed on the surface of the skin, does not stick or dry the hair, does not leave visible traces, and has a cooling effect on the skin.
Indications
Belosalic® is used for the treatment of subacute and chronic dermatoses that are sensitive to external corticosteroid therapy, accompanied by hyperkeratosis and peeling. Psoriasis, eczema (dyshidrotic, seborrheic, thylotic), neurodermatitis, lichen planus, chronic discoid lupus erythematosus, ichthyosis and ichthyotic skin lesions, etc.
Use during pregnancy and lactation
Topical use of Belosalic® in pregnant women is allowed in cases where the intended benefit to the mother exceeds the risk to the fetus. In such cases, the use of the drug should be short and limited to small areas of the skin. During breastfeeding, the use of the drug Belosalic® is possible according to strict indications, but the drug should not be applied to the skin of the breast before feeding.
Recommendations for use
Externally. The drug is applied in a thin layer to the affected areas of the skin 2 times a day (in the morning and at night). In some patients, the maintenance effect can be achieved with less frequent applications. The course of treatment should not exceed 3 weeks.
For children aged 2-12 years, apply a thin layer 1 or 2 times a day for no more than 5 days.
The duration, specifics of the treatment regimen, and repeated courses of treatment should be agreed with the doctor.
The drug should be used only according to the method of use and in the doses indicated in the instructions for use. If necessary, consult your doctor before using the medicine.
Contraindications
Hypersensitivity to betamethasone, salicylic acid or any of the auxiliary components of the drug.
Skin tuberculosis, skin manifestations of syphilis, chickenpox, viral skin infections, skin post-vaccination reactions, open wounds, trophic ulcers, rosacea, acne vulgaris, children under 6 months of age.
With caution: Liver failure, childhood, long-term therapy, especially with the use of occlusive dressings.
Side effects
Burning, irritation, dryness, hypopigmentation, itchy skin, folliculitis, hypertrichosis, acne.
When using occlusive dressings – maceration of the skin, striae, skin atrophy, secondary infection, sweating.
With prolonged use – skin atrophy, local hirsutism, telangiectasia, purpura, hypopigmentation, hypertrichosis.
When applied on large surfaces – systemic manifestations of side effects of corticosteroids (growth retardation, Itsenko-Cushing syndrome, benign increase in intracranial pressure after treatment, hyperglycemia, glucosuria, hypokalemia, increased blood pressure) or salicylates (pallor, fatigue, drowsiness, hyperventilation on the background of tachypnea, nausea, vomiting, hearing disorders, confusion).
Overdose
Long-term continuous use of a combination of betamethasone and salicylic acid, especially in children, on large skin surfaces, when applied to skin with impaired integrity or when used under an occlusive dressing, increased absorption of active components and the manifestation of systemic effects (secondary adrenal insufficiency, hypercorticism, Cushing’s syndrome) are not excluded.
In this case, discontinuation of the drug and symptomatic therapy are recommended.
Special instructions
Avoid contact with the eyes, do not apply on the skin near the eyes and on the mucous membranes.
Long-term use on the face is not recommended – rosacea-type dermatitis, perioral dermatitis, skin atrophy and acne may develop; avoid use in the anogenital area.
The use of the ointment in children should be as short as possible (there is a danger of systemic absorption of proportionally more active components due to the predominance of the skin area over body weight and insufficient maturity of the epidermis).
If there are signs of allergic reactions or local irritation, treatment should be discontinued.
