Composition
1 bottle of powder contains:
Active ingredients:
a mixture of benzathine benzylpenicillin,
benzylpenicillin sodium (or potassium) salt and benzylpenicillin novocaine salt 600 thousand units.
Pharmacological action
Combined antibacterial drug of the group of penicillins of prolonged action, destroyed by penicillinase.
The mechanism of action is based on a violation of the synthesis of peptidoglycan, a mucopeptide of the cell wall, which leads to inhibition of the synthesis of the cell wall of the microorganism, suppression of bacterial growth and reproduction.
Active in relation to:
- Gram-positive bacteria: Staphylococcus spp. (not forming penicillinase). Streptococcus spp. (including Streptococcus pneumoniae). Corynebacterium diphtheriae. Bacillus anthracis.
- Gram-negative bacteria: Neisseria gonorrhoeae. Neisseria meningitidis. Actinomyces israelii.
- Treponema spp.
- Anaerobic spore-forming rods.
- Leishmania.
Staphylococcus spp., which produces penicillinase, is resistant to the drug.
Indications
- Treatment of infectious diseases caused by penicillin-sensitive pathogens (especially in cases where it is necessary to create a long-term therapeutic concentration in the blood).
- Syphilis.
- Yawns.
- Streptococcal infections (acute tonsillitis, scarlet fever, wound infections, erysipelas), with the exception of infections caused by group B streptococci.
- Prevention of rheumatism.
- Leishmaniasis.
Use during pregnancy and lactation
Data on the use of the drug Bicillin-3 during pregnancy and lactation (breastfeeding) are not provided.
Contraindications
- Hypersensitivity to penicillins and procaine (novocaine).
- Bronchial asthma.

- Other allergic diseases.
Side effects
Allergic reactions: Â urticaria, angioedema, erythema multiforme, anaphylactic shock, arthralgia, fever, exfoliative edema.
From the digestive system: Â stomatitis, glossitis.
From the side of the blood coagulation system: Â anemia, thrombocytopenia, leukopenia, hypocoagulation.
Other services: Â with prolonged use – superinfection, mycoses.
Interaction
When used concomitantly with bactericidal antibiotics (including cephalosporins, cycloserine, vancomycin, rifampicin, aminoglycosides), synergism of action is noted; with bacteriostatic antibiotics (including macrolides, chloramphenicol, lincosamides, tetracyclines) – antagonism.
Bicillin-3 reduces the effectiveness of oral contraceptives and ethinyl estradiol (risk of breakthrough bleeding).
Concomitant use of Bicillin-3 with allopurinol increases the risk of allergic reactions.
Pharmacokinetic interaction: concomitant use of diuretics, allopurinol, phenylbutazone, NSAIDs reduces tubular secretion and increases the concentration of penicillin.
How to take, course of use and dosage
of Bicillin-3 is injected deep into the upper outer quadrant of the buttock in a dose of no more than 1.2 million units. If necessary,2 injections are made in different buttocks. Repeated injections are performed 4 days after the previous injection.
The drug in a dose of 600,000 units is administered 1 time in 6 days.
In the treatment of primary and secondary syphilis
A single dose of the drug is 1.8 million units. The course of treatment is 7 injections. The first injection is carried out at a dose of 300,000 units, the second injection – after 1 day in a full dose (1.8 million units). Subsequent injections are carried out 2 times a week.
In the treatment of secondary recurrent and latent early syphilis
For the first injection, a dose of 300,000 units is used, for subsequent injections-1.8 million units. Injections are carried out 2 times a week. The course of treatment is 14 injections.
Intravenous use of the drug is prohibited.
Rules for preparation and use of the solution
- Bicillin-3 suspension is prepared immediately before use.
- 2-3 ml of sterile water for injection or isotonic sodium chloride solution is injected into the vial with the drug.
- The mixture in the vial is shaken for 30 seconds (in the direction of the longitudinal axis) until a homogeneous suspension (or suspension) is formed, which is injected deep in / m into the upper outer quadrant of the buttock.
- Rubbing the buttock after the injection is not recommended.
Overdose
Currently, no cases of Bicillin-3 overdose have been reported.
Special instructions
If allergic reactions develop, treatment should be stopped immediately.
If anaphylactic shock develops, anti-shock therapy is indicated (use of norepinephrine, corticosteroids, and mechanical ventilation).
Intravenous and endolumbal use is not allowed, as Uanye syndrome may develop (depression, anxiety, paresthesia, visual impairment).
In the treatment of sexually transmitted diseases, if syphilis is suspected, microscopic and serological studies should be performed before starting therapy and then for 4 months.
Due to the possible development of fungal lesions, it is advisable to prescribe antifungal drugs ( nystatin, levorin ), as well as ascorbic acid and B vitamins.
It should be taken into account that the use of the drug in insufficient doses or early termination of treatment leads to the appearance of resistant strains of pathogens.
