Pharmacological action

Pharmacological action – analgesic, antiplatelet, antipyretic, anti-inflammatory.

Nonsteroidal anti-inflammatory drug (NSAID).

It has anti-inflammatory, analgesic and antipyretic effects associated with the suppression of cyclooxygenase 1 and 2, which regulates the synthesis of prostaglandins.

Reduces platelet aggregation, adhesiveness, and thrombosis by suppressing the synthesis of thromboxane A2 in platelets.

The antiplatelet effect persists for 7 days after a single dose (more pronounced in men than in women).

Dissolving in water, the Upsarin Upsa tablet forms a buffer solution, which after taking the drug supports the active substances in dissolved form, not allowing them to pass back into the undissolved form when in contact with the acidic environment of the stomach and precipitate as solid particles on its walls.

As a result, faster and more complete absorption of the drug is provided, as well as its better tolerability compared to conventional acetylsalicylic acid tablets.

Indications

Moderate or mild pain syndrome of various origins in adults:

• Headache (including those associated with alcohol withdrawal syndrome).
• Toothache.
• A migraine.
• Neuralgia.
• Thoracic radicular syndrome.
• Myalgia and joint pain.
• Pain during menstruation.

Elevated body temperature in colds and other infectious and inflammatory diseases (in adults and children over 15 years of age).

Use during pregnancy and lactation

The drug is contraindicated in the first and third trimesters of pregnancy.

If it is necessary to use the drug during lactation, breastfeeding should be discontinued.

Use in children

Contraindication:

children under 15 years of age with acute respiratory diseases caused by viral infections due to the risk of developing Reye’s syndrome (encephalopathy and acute fatty liver with acute development of liver failure).

Contraindications

• erosive and ulcerative lesions of the gastrointestinal tract in acute phase, gastrointestinal bleeding;
• severe liver dysfunction;
• severe renal dysfunction;
• “aspirin” asthma;
• hemorrhagic diathesis (hemophilia, von Willebrand disease, telangiectasia, hypoprothrombinemia, thrombocytopenia, thrombocytopenic purpura);
• dissecting aneurysm of the aorta;
• portal hypertension;
• a vitamin K deficiency;
• deficiency of glucose-6-phosphate dehydrogenase;
• phenylketonuria;
• I and III trimester of pregnancy;
• lactation (breastfeeding);
• children under 15 years of age with acute respiratory diseases caused by viral infections, because of the risk of Reye’s syndrome (encephalopathy and acute fatty liver with acute hepatic insufficiency);
• hypersensitivity to the components of the drug;
• hypersensitivity to other NSAIDs.

With caution, the drug should be used for hyperuricemia, urate nephrolithiasis, gout, peptic ulcer of the stomach and/or duodenum (in the anamnesis), decompensated heart failure.

Side effects

A

Allergic reactions: rarely-skin rash, bronchospasm, angioedema, formation of an “aspirin triad” based on the hapten mechanism (a combination of bronchial asthma, recurrent nasal and paranasal sinus polyposis, and intolerance to acetylsalicylic acid and pyrazolone preparations).

From the digestive system: rarely-nausea, vomiting, epigastric pain, diarrhea, gastrointestinal bleeding (coffee grounds vomiting, black tarry stools), decreased appetite, increased activity of hepatic transaminases.

From the urinary system: rarely-impaired renal function.

From the hematopoietic system: rarely-thrombocytopenia, anemia, leukopenia, hyperbilirubinemia.

From the side of the blood coagulation system: rarely-hemorrhagic syndrome (nosebleeds, bleeding gums), increased blood clotting time.

At the recommended doses, the drug is usually well tolerated.

The patient should be warned that in case of adverse reactions, it is necessary to immediately stop taking the drug and inform the attending physician.

Interaction

Acetylsalicylic acid increases the toxicity of methotrexate, the effects of opioid analgesics, other NSAIDs, oral hypoglycemic drugs, heparin, indirect anticoagulants, thrombolytics and platelet aggregation inhibitors, sulfonamides (including co-trimoxazole), triiodothyronine.

Acetylsalicylic acid reduces the toxicity of uricosuric drugs (benzbromarone, sulfinpyrazone), antihypertensive agents and diuretics (spironolactone, furosemide).

CORTICOSTEROIDS, ethanol and ethanol-containing drugs increase the damaging effect of acetylsalicylic acid on the gastrointestinal mucosa, increase the risk of gastrointestinal bleeding.

Acetylsalicylic acid increases the concentration of digoxin, barbiturates and lithium preparations in the blood plasma.

Antacids containing magnesium and / or aluminum hydroxide slow and impair the absorption of acetylsalicylic acid.

How to take, course of use and dosage

The drug is intended for adults and children over 15 years of age.

Take orally.

• Adults and children over 15 years of age: take 1 tablet (500 mg) up to 6 times a day. For severe pain and high fever – 2 tablets (1 g); the maximum daily dose should not exceed 6 tablets (3 g).
• Elderly people: take 1 tablet (500 mg) up to 4 times a day; the maximum daily dose should not exceed 4 tablets (2 g).

How to use: dissolve the tablet/tablets in a glass (200 ml) of water or juice.

Frequency and time of use: the interval between doses of the drug should be at least 4 hours.

Regular adherence to the drug regimen allows you to avoid a sharp increase in temperature and reduce the intensity of pain.

The duration of treatment (without consulting a doctor) should not exceed 5 days when prescribed as an analgesic and more than 3 days – as an antipyretic.

Overdose

Symptoms: in the initial stage of poisoning, symptoms develop dizziness, vomiting, central nervous system agitation, decreased hearing acuity, severe headache, visual impairment, nausea, increased breathing, and later respiratory failure occurs, depression of consciousness up to coma, and disorders of water and electrolyte metabolism.

Treatment: induce vomiting or gastric lavage, prescribe a laxative and activated charcoal.

Treatment should be carried out in a specialized department.

Special instructions

Acetylsalicylic acid reduces the excretion of uric acid from the body, which can cause an acute attack of gout in predisposed patients.

With prolonged use of the drug, a general blood test and a fecal occult blood test should be performed periodically, and the functional state of the liver should be monitored.

Before surgery, to reduce bleeding during the operation and in the postoperative period, you should stop taking the drug for 5-7 days and inform the doctor.

Children should not be prescribed medications containing acetylsalicylic acid, because in the case of a viral infection, the risk of Reye’s syndrome increases. Symptoms of Reye’s syndrome include: prolonged vomiting, acute encephalopathy, and enlarged liver.

Patients who follow a salt-free or low-salt diet should take into account that each tablet of the drug contains 388.5 mg (16.9 meq) of sodium.
