Pharmacological action
Pharmacodynamics
Calcium ions are involved in the transmission of nerve impulses, the contraction of smooth and striated muscles, the functioning of the myocardium, blood clotting; they are necessary for the formation of bone tissue, the functioning of other systems and organs.
The concentration of calcium ions in the blood decreases with many pathological processes; severe hypocalcemia contributes to the occurrence of tetany. In addition to eliminating hypocalcemia, calcium compounds reduce vascular permeability, have anti-allergic, anti-inflammatory, and hemostatic effects.
Pharmacokinetics
After parenteral use, the drug is evenly distributed in all tissues and organs. In the blood plasma, calcium is in an ionized state. Passes through the placental barrier, is released into breast milk. It is mainly excreted from the body by the kidneys.
Indications
- The parathyroid insufficiency, increased output of calcium from the body (in particular during prolonged dehydration),
- as an adjuvant for allergic diseases (serum sickness, urticaria, angioneurotic edema) and allergic complications of drug therapy,
- to reduce vascular permeability in pathological processes of various Genesis (exudative phase of the inflammatory process, hemorrhagic vasculitis, radiation sickness);

Use during pregnancy and lactation
Pregnant women who take calcium supplements together with vitamin D are more likely to develop hypercalcemia.
At the recommended doses, taking calcium supplements does not cause any negative consequences for the fetus, and it is safe to use it during breast-feeding.

Side effects

Interaction
of Calcium gluconate eliminates the inhibition of neuromuscular transmission after the use of antibiotics of a number of aminoglycosides. When combined with thiazide diuretics, it leads to the development of hypercalcemia.
During treatment with cardiac glycosides, parenteral use of the drug increases the cardiotoxic effects of cardiac glycosides. When used concomitantly with phenigidine, calcium supplements weaken its effect. Calcium supplements increase the toxicity of quinidine.
Thiazide diuretics reduce urinary calcium excretion, which increases the risk of hypercalcemia.
Calcium supplements are contraindicated in patients receiving cardiac glycosides.
Calcium supplements can reduce the absorption of tetracycline drugs in the gastrointestinal tract, and therefore the interval between taking these drugs should be at least 3 hours.
How to take, course of use and dosage
Intravenously or intramuscularly, adults and children over 14 years of age are administered 5-10 ml of Calcium gluconate daily, every other day or after 2 days, depending on the nature of the disease and the patient’s condition.
The ampoule with the solution is heated to body temperature before use. The solution is administered slowly, for 2-3 minutes.
Children are given intravenously, depending on their age, a 10% solution of calcium gluconate in the following doses: under the age of 6 months – 0.1-1 ml,6-12 months-1-1.5 ml,1-3 years – 1.5–2 ml,4-6 years – 2-2.5 ml,7-14 years – 3-5 ml.
For the introduction of a solution in an amount less than 1 ml, a single dose is adjusted to the appropriate volume (volume of the syringe) with 0.9% isotonic sodium chloride solution or 5% glucose solution.
Overdose
Symptoms: Hypercalcemia may occur.
Treatment: Calcitonin is used as an antidote, which is administered intravenously at the rate of 5-10 IU per 1 kg of body weight per day (the drug is diluted in 500 ml of 0.9% sodium chloride solution, administered dropwise for 6 hours in 2-4 doses).
Special instructions
Patients with hypercalciuria, reduced glomerular filtration rate, or a history of nephrourolithiasis should be prescribed the drug with caution and under the control of the content of calcium in the urine.
To reduce the risk of developing nephrourolithiasis, it is recommended to drink a sufficient amount of liquid inside.
Before filling the syringe with a solution of calcium gluconate, it is necessary to make sure that there are no ethyl alcohol residues in it, because due to their interaction, calcium gluconate precipitates.
Ability to influence the speed of reactions when driving vehicles or working with other mechanisms. There is no evidence that Calcium gluconate can affect the speed of psychomotor reactions.
Calcium supplements should be used with caution in patients with heart disease or sarcoidosis. It is necessary to carefully monitor the amount of calcium in the blood and urinary calcium excretion when prescribing high doses of the drug, especially in children. Treatment should be suspended if blood calcium levels exceed 2.625-2.75 mmol / l or if urinary calcium excretion exceeds 5 mg / kg. High vitamin D intake should be avoided during calcium therapy.
Patients with mild hypercalciuria, reduced glomerular filtration rate, or a history of nephrolithiasis should be treated with caution and under the control of urinary calcium. To reduce the risk of developing nephrolithiasis, it is recommended to drink plenty of water.
