Cardiket is a vasodilator.
Pharmacodynamics
Isosorbide dinitrate causes relaxation of the smooth muscle wall of blood vessels, thus inducing vasodilation. Isosorbide dinitrate acts as a peripheral vasodilator, affecting both arteries and veins. These effects contribute to the deposition of venous blood and reduce venous return to the heart, reducing end dBP and left ventricular volume (reduce preload).
A long-acting cardiket is characterized by a combination of rapid onset of effect and duration of action.
The advantages of the drug are determined by the fact that the Active ingredient contained in the Cardiket®tablet of prolonged action quickly enters the bloodstream. As a result, the drug provides a rapid onset of effect in the treatment of angina pectoris and retains its protective effect for a long time.
Pharmacokinetics
When taken orally, isosorbide dinitrate is rapidly absorbed. Oral bioavailability is 22% (the effect of “first pass” through the liver). The onset of action is in 15-30 minutes, thecmax of the substance in the blood plasma is reached in 15 minutes — 1-2 hours. As a result of passing through the liver, isosorbide dinitrate is metabolized to form isosorbide-2-mononitrate and isosorbide-5-mononitrate, which have T1/2of 1.5-2 and 4-6 hours, respectively. Both metabolites are pharmacologically active. It is excreted by the kidneys (almost entirely in the form of metabolites). Binding to plasma proteins is 30%.
From the cardiovascular system: Â at the beginning of treatment-severe headache (due to dilation of the venous bed of the brain vessels); possibly-tachycardia, orthostatic hypotension.
From the central nervous system : Â drowsiness.
From the gastrointestinal tract: Â in some cases-nausea, vomiting.
Dermatological reactions: Â in some cases – erythematous rashes.
Simultaneous use of Cardiket retard with antihypertensive drugs, cyclic antidepressants, MAO inhibitors may increase its hypotensive effect.
When used simultaneously, Kardiket retard potentiates the action of dihydroergotamine and ethanol.
The dose is selected individually, since it depends on the stage of the disease and the individual need for nitrates.
Tablets of 20 mg are prescribed for 1 tab. 2-3 times /day, Kardiket should be taken after a meal, without chewing and with a small amount of liquid.
When prescribing more than 1 tab. /day, the interval between doses should be at least 8 hours (to ensure the full effect of the drug).
Symptoms: Â headache, palpitations, dizziness, hypotension, nausea, vomiting. Treatment: Â symptomatic.
Cardiket retard should not be prescribed for the purpose of stopping an angina attack.
Severe headache that occurs at the beginning of therapy, in most cases significantly decreases or completely disappears with continued use of the drug.
Alcohol consumption should be avoided during treatment with Cardiket.
Influence on the ability to drive motor vehicles and manage mechanisms
The possibility of engaging in potentially dangerous activities should be considered only after evaluating the individual patient’s response to the drug.
