Adults are prescribed 20-40 mg per day (contents of 1-2 ampoules).
Treatment in children begins with a dose of 5 mg (0.25 ml). In the future, the dose is increased depending on the age of the child.
Children aged 1-12 months – 5 mg (0.25 ml).
Children aged 1-6 years – 10 mg (0.5 ml).
Children aged 7-18 years – 10-20 mg (0.5-1 ml).
The daily dose should not exceed 2 mg / kg of body weight.
If the allergy is severe, treatment should begin with an injectable form of chloropyramine. First, it is administered intravenously slowly, then they switch to intramuscular use and in the final phase of treatment they switch to taking tablets inside. The duration of treatment is set individually, depending on the clinical symptoms and the patient’s condition.
Special patient groups
Elderly, emaciated patients: the use of chloropyramine requires special caution, because in these patients, antihistamines often cause side effects (dizziness, drowsiness).
Patients with impaired liver function: it may be necessary to reduce the dose due to a decrease in the metabolism of the active component of the drug in liver diseases.
Patients with impaired renal function: it may be necessary to change the dosage regimen and reduce the dose due to the fact that the active component is mainly excreted through the kidneys.
With caution
The drug should be prescribed with caution to elderly patients, with insufficient liver function and/or heart disease, with peptic ulcer of the stomach and / or duodenum, when taking medications that depress the central nervous system (CNS), urinary retention, angle-closure glaucoma.

Pharmacodynamicachloropyramine is a blocker of H1-histamine receptors. According to its chemical structure, it belongs to the derivatives of ethylenediamine. Chloropyramine selectively blocks H1-histamine receptors and reduces capillary permeability. Prevents the development and eases the course of allergic reactions. It has a sedative, antihistamine, m-holinoblokiruyushchy and pronounced antipruritic effect. It has moderate antispasmodic activity and antiemetic effect. Pharmacokinetics are unevenly distributed in the body, including the central nervous system. Penetrates the blood-brain barrier. The relationship of chloropyramine with plasma proteins is 7.9%. The peak of binding was observed at pH 6,8-7,4. The therapeutic concentration is maintained for 4-6 hours. It is intensively metabolized in the liver. It is excreted mainly through the kidneys in the urine in the form of metabolites. Elimination of chloropyramine in children may occur faster than in adults.

From the central nervous system:  lethargy, weakness, drowsiness, dizziness, headache, euphoria, fatigue, irritability, tremor, convulsions, impaired coordination of movements, visual disturbances; children may have some stimulating effect on the central nervous system, manifested by anxiety, increased irritability, insomnia. From the digestive system:  dry mouth, nausea, vomiting, gastralgia, diarrhea/constipation, loss or increased appetite. From the side of the urethra:  difficulty urinating. From the cardiovascular system:  reduced blood pressure (more often in elderly patients), tachycardia, arrhythmia. From the hematopoietic system:  very rarely – leukopenia, agranulocytosis. From the side of the visual organs, increased intraocular pressure. Allergic reactions to the drug. Other services:  photosensitivity, muscle weakness.
Increases the effect of drugs for general anesthesia, sleeping pills, sedatives, tranquilizers, m-holinoblokatorov, opioid analgesics, local anesthetics. Tricyclic antidepressants and monoamine oxidase inhibitors enhance the m-holinoblocking and depressing effect on the central nervous system. Caffeine and phenamine reduce the depressing effect on the central nervous system. Concomitant use with ethanol is not recommended (risk of severe CNS depression).

Symptoms:  in children – agitation, anxiety, hallucinations, athetosis, ataxia, convulsions, mydriasis, immobility of the pupils, hyperemia of the facial skin, hyperthermia; then-collapse, coma. In adults-depression (lethargy, depression) or agitation of the central nervous system (psychomotor agitation), very rarely-hallucinations, coordination disorders, dilated pupils and lack of their reaction to light, dry mouth, hyperthermia and hyperemia of the skin, convulsions, in severe cases – coma. Treatment:  symptomatic therapy (including the use of antiepileptic drugs, caffeine, and phenamine), resuscitation measures, and artificial ventilation of the lungs – as indicated. There is no specific antidote.
During treatment, the use of alcoholic beverages is unacceptable. Taking the drug overnight may increase the symptoms of reflux esophagitis.
If during prolonged use of the drug there is an unreasonable increase in body temperature, laryngitis, pallor of the skin, jaundice, formation of ulcers of the oral mucosa, the appearance of hematomas, unusual and difficult-to-stop bleeding, you should consult a doctor, since prolonged therapy with antihistamines can lead to hematopoiesis disorders.
Antihistamines can distort the appearance of the reaction when setting skin allergy tests, so a few days before the planned test, the drug should be discontinued. When combined with ototoxic drugs, chloropyramine may mask early signs of ototoxicity.
Influence on the ability to drive vehicles and mechanisms
Patients using chloropyramine are advised to refrain from driving vehicles, working with mechanisms, and other activities that require increased concentration of attention and speed of psychomotor reactions.
