Cialis is a treatment for erectile dysfunction (impotence, erectile dysfunction) in men and is part of a group of drugs called PDE-5 (phosphodiesterase type 5) inhibitors.
The drug dilates the blood vessels of the penis during sexual arousal.
As a result, blood circulation in the penis increases, and this, in turn, contributes to the onset of a stable erection.
Erectile dysfunction
Side effects are usually minor or moderate in severity, transient and decrease with continued use of the drug.
Most often: Â headache (11%), dyspepsia (7%).
Possible: Â back pain, myalgia, nasal congestion, flushes of blood to the face.
Rarely: Â swelling of the eyelids, pain in the eyes, hyperemia of the conjunctiva. dizziness.
Very rare: Â hypersensitivity reactions (including rash, urticaria and facial edema, Stevens-Johnson syndrome and exfoliative dermatitis), hypotension (in patients who have already taken antihypertensive drugs), hypertension and syncope, abdominal pain and gastroesophageal reflux, hyperhidrosis (increased sweating), priapism and delayed erection, blurred vision, non-arterial anterior ischemic pain neuropathy of the optic nerve, retinal vein occlusion, visual field disorders.
From the cardiovascular system: Â in patients with cardiovascular risk factors such as myocardial infarction, sudden cardiogenic death, stroke, chest pain, palpitations, and tachycardia, it is not possible to determine whether these events are directly related to these risk factors, tadalafil, sexual arousal, or a combination of these or other factors.

Symptoms: Â with a single dose of 500 mg/day given to healthy volunteers and multiple doses of up to 100 mg/day given to patients with erectile dysfunction, the undesirable effects were the same as with lower doses of the drug.
Treatment: Â conducting standard symptomatic therapy. During hemodialysis, tadalafil is practically not excreted.
Caution should be exercised when prescribing Cialis to patients taking alpha-1 blockers, for example, doxazosin, since concomitant use in some cases may lead to symptomatic hypotension.
In a clinical pharmacology study,18 healthy volunteers who received a single dose of tadalafil did not experience symptomatic hypotension with simultaneous use of the alpha-1-blocker tamsulosin.
During Cialis treatment, patients with moderate renal insufficiency (creatinine clearance from 31 to 50 ml/min) were more likely to have back pain compared to patients with mild renal insufficiency (creatinine clearance from 51 to 80 ml/min) or healthy volunteers.
Patients with creatinine clearance < 50 ml / min should be treated with caution. Sexual activity has a potential risk for patients with cardiovascular diseases. Therefore, treatment of erectile dysfunction, including with the use of Cialis, should not be carried out in men with heart diseases in which sexual activity is not recommended.
The potential risk of sexual activity complications in patients with cardiovascular diseases should be considered: myocardial infarction within the last 90 days, unstable angina or angina that occurs during sexual intercourse, chronic heart failure (NYHA functional class II and higher) that has developed within the last 6 months, uncontrolled cardiac arrhythmias, hypotension (blood pressure less than 90/50 mm Hg) or uncontrolled arterial hypertension, stroke, transferred within the last 6 months.
Cialis should be used with caution in patients with a predisposition to priapism (for example, sickle cell anemia, multiple myeloma or leukemia) or in patients with anatomical deformity of the penis (for example, angulation, cavernous fibrosis or Peyronie’s disease). The patient should be informed about the need to immediately consult a doctor in case of an erection lasting 4 hours or more.
Untimely treatment of priapism leads to damage to the tissues of the penis, as a result of which there may be a long-term loss of potency. No cases of priapism have been reported with Cialis. The safety and efficacy of combining Cialis with other treatments for erectile dysfunction have not been studied. Therefore, the use of such combinations is not recommended.
Like other PDE5 inhibitors, tadalafil has systemic vasodilating properties, which can lead to a transient decrease in blood pressure. Before prescribing Cialis, doctors should carefully consider whether patients with existing cardiovascular disease will be exposed to undesirable effects due to such vasodilating effects.
Patients with impaired liver function do not require special dose adjustment. Since there are no data from controlled clinical trials on the safety and efficacy of Cialis in patients with severe hepatic insufficiency, the drug should be used in this category of patients with extreme caution and only if absolutely necessary.
In patients with impaired renal function (creatinine clearance >30 ml/min), no special dose adjustment is required. During Cialis treatment, patients with moderate renal insufficiency (creatinine clearance from 31 to 50 ml/min) were more likely to have back pain compared to patients with mild renal insufficiency (creatinine clearance from 51 to 80 ml/min) or healthy volunteers. Patients with creatinine clearance<50 ml / min should be treated with caution.
Since there are no data from controlled clinical trials on the safety and efficacy of Cialis in patients with severe renal insufficiency (creatinine clearance < 30 ml/min), the drug should be used in this category of patients with extreme caution and only if absolutely necessary.
Cialis is not used in patients under 18 years of age.
