Composition

1 capsule. contains: Active ingredients: diclofenac sodium – 50 mg, pyridoxine hydrochloride-50 mg, thiamine hydrochloride (vit. B₁) – 50 mg, cyanocobalamin-0.25 mg. Auxiliary substances: lysine hydrochloride – 110.75 mg, microcrystalline cellulose-150 mg, colloidal anhydrous silicon dioxide-4 mg, magnesium stearate-5 mg.

Pharmacological action

The mechanism of action of diclofenac is associated with inhibition of cyclooxygenase activity – the main enzyme of arachidonic acid metabolism, which is a precursor of prostaglandins, which play a major role in the pathogenesis of inflammation, pain and fever. Thiamine (vitamin B1) in the human body as a result of phosphorylation processes is converted to cocarboxylase, which is a coenzyme of many enzyme reactions. Vitamin B-1 plays an important role in carbohydrate, protein and fat metabolism. It is actively involved in the processes of nerve excitation in synapses. Pyridoxine (vitamin B6) is essential for the normal functioning of the central and peripheral nervous system. In its phosphorylated form, it is a coenzyme in the metabolism of amino acids (decarboxylation, transamination). It acts as a coenzyme of the most important enzymes acting in nerve tissues. Cyanocobalamin (vitamin B12) is necessary for normal hematopoiesis and maturation of red blood cells, and also participates in a number of biochemical reactions that ensure the vital activity of the body – in the transfer of methyl groups, in the synthesis of nucleic acids, protein, in the exchange of amino acids, carbohydrates, lipids.

Indications

Indications for use of the drug Clodiphene Neuro are:  – pain syndrome in non – rheumatic inflammations (after injuries, surgical and dental interventions; in gynecological diseases-primary algodismenorrhea, adnexitis; in inflammatory diseases of ENT organs – pharyngitis, tonsillitis, otitis);– inflammatory and degenerative diseases of the spine and joints (rheumatoid arthritis, ankylosing spondylitis-Ankylosing spondylitis, spondyloarthritis, spondyloarthrosis, osteoarthritis); – neuritis and neuralgia (cervical syndrome, lumbago, sciatica);– acute gouty arthritis;– rheumatic diseases of the periarticular soft tissues.

Contraindications

Contraindications to the use of the drug Clodiphene Neuro are:  hypersensitivity to the components of the drug;  peptic ulcer of the stomach and duodenum 12; III trimester of pregnancy; children under 12 years of age.

Side effects

Side effects depend on individual sensitivity, the amount of dose taken, and the duration of treatment. From the gastrointestinal tract: sometimes there may be nausea, vomiting, diarrhea, rarely-lack of appetite, abdominal cramps, a feeling of fullness of the stomach, the development of ulcers in the gastrointestinal tract, which in some cases leads to an admixture of blood in the stool. From the central nervous system: sometimes there may be headaches, dizziness, rarely-sleep disorders, excitability. Dermatological reactions: sometimes itching, rash, urticaria are observed. From the side of the kidneys: in some cases, there were violations of renal function, proteinuria, hematuria and kidney damage (interstitial nephritis, nephrotic syndrome), so regular monitoring of renal function is necessary. From the liver: sometimes there is an increase in the level of transaminases in the serum. With prolonged use, regular monitoring of liver function is recommended. From the hematopoietic system: in rare cases, anemia, leukopenia, agranulocytosis, thrombocytopenia were observed. Other: hypersensitivity reactions to diclofenac may occur in the form of swelling of the face, tongue, larynx, rapid heartbeat, increased blood pressure.

Interaction

When used simultaneously Clodifen Neuro: – digoxin, phenytoin or lithium preparations may increase the level of these drugs in plasma; – diuretics and antihypertensive agents may weaken their effect;- potassium-sparing diuretics may develop hyperkalemia; – other NSAIDs or glucocorticoids increase the risk of side effects from the gastrointestinal tract;- acetylsalicylic acid decreases the level of diclofenac concentration in the blood;- cyclosporine may increase the toxic effect of the latter on the kidneys; – antidiabetic agents may develop hypo – and hyperglycemia; with this combination of drugs, blood sugar control is necessary;- anticoagulants require regular monitoring of blood clotting;- levodopa reduces the effectiveness of the antiparkinsonian drug. When using methotrexate for 24 hours before or after taking Clodifene Neuro, it is possible to increase the concentration of methotrexate and increase its toxic effect. Ethanol dramatically reduces the absorption of thiamine (blood levels can drop by 30%). Long-term treatment with anticonvulsants can lead to thiamine deficiency. The use of colchicine and biguanides reduces the absorption of cyanocobalamin.

How to take, course of use and dosage

Clodifen Neuro capsules should be taken orally with meals, without chewing and washed down with plenty of liquid. For adults, the drug is prescribed 1 capsule 3 times a day at the beginning of treatment, as a maintenance dose,1 capsule 1-2 times a day. The duration of therapy depends on the nature and severity of the disease.

Overdose

Symptoms of drug overdose Clodifen Neuro: vomiting, dizziness, headache, shortness of breath, blurred consciousness; in children-myoclonic convulsions, nausea, abdominal pain, bleeding, impaired liver and kidney function. Treatment: gastric lavage, use of activated charcoal. Conduct symptomatic therapy, forced diuresis. Hemodialysis is ineffective.

Special instructions

Application Clodiphene Neuro is only possible under strict medical supervision for the following conditions:  induced porphyria; I and II trimesters of pregnancy; breast-feeding period. Careful medical monitoring is required:  with a history of diseases of the gastrointestinal tract and / or inflammatory ulcerative diseases of the gastrointestinal tract (ulcerative colitis, Crohn’s disease); in patients with kidney damage; with high blood pressure and / or heart failure; in elderly patients; immediately after a major surgical intervention. With great caution, Clodifen Neuro can be used in patients with asthma, allergic (“hay”) runny nose, nasal polyps, as well as in patients with hypersensitivity to NSAIDs due to the risk of an asthma attack, angioedema or urticaria. During the treatment period, care should be taken when driving vehicles and engaging in other potentially dangerous activities that require increased concentration of attention and speed of psychomotor reactions.

Store in a place protected from moisture and light at a temperature not exceeding 25°C. Keep out of reach of children!
