Pharmacological action
Clotrimazole is a derivative of imidazole. A broad-spectrum antifungal agent. The effect of the drug is associated with a violation of the synthesis of ergosterol, which is part of the fungal cell membrane, which causes a change in its structure and properties and leads to cell lysis. Dermatophytes, yeast fungi (genus Candida, Torulopsis glabrata, genus Rhodotorula), mold fungi, as well as the pathogen Pityriasis versicolor (multicolored lichen) and the pathogen zritrasma are sensitive to clotrimazole.
In addition, it has an antimicrobial effect against gram-positive (staphylococci and streptococci) and Gram-negative bacteria (Bacteroides, Gardnerella vaginalis), as well as against Trichomonas vaginalis, Malasseria furfur, Corinebacterium minussimum.
Indications
-Fungal lesions of the skin and mucous membranes. – Mycoses of the feet/hands, smooth skin of the trunk, limbs, folds, beard area, mustache, scalp caused by dermatophytes. – Pityriasis versicolor, zritrasma, candidiasis of the skin, genital mucosa, paronychia.
Use during pregnancy and lactation
It should not be used in the first trimester of pregnancy (adequate and strictly controlled studies have not been conducted). With intravaginal use in women in the second and third trimesters of pregnancy, no adverse effects on the fetus were detected, but the use of a vaginal applicator is undesirable.
With caution — during breastfeeding (it is not known whether clotrimazole penetrates into breast milk).
Contraindications
Hypersensitivity to clotrimazole, first trimester of pregnancy.
Side effects
Allergic reactions (pruritus, urticaria).
When applied topically to the skin: erythema, blistering, swelling, burning and tingling, irritation and flaking of the skin.
When applied topically for the treatment of urogenital infections: itching, burning, hyperemia and swelling of the mucous membrane, vaginal discharge, frequent urination, intercurrent cystitis, burning sensation in the penis of a partner, pain during sexual intercourse.
When applied topically in the oral cavity: redness of the oral mucosa, burning and tingling sensation at the application site, irritation.
Interaction
Reduces the effect of polyene antibiotics (amphotericin, nystatin, natamycin, etc. ). Propyl ether of para-hydroxybenzoic acid in high concentrations enhances the antifungal effect of clotrimazole, dexamethasone — reduces.
When used concomitantly with nystatin, clotrimazole activity may decrease.
How to take, course of use and dosage
A few drops of the solution should be easily rubbed into the affected areas of the skin 2-3 times a day. After the acute symptoms of the disease are eliminated, treatment should be continued for at least another 4 weeks.
For dermatomycosis of the feet, the drug should be used for another 2 weeks after all symptoms of the disease have been eliminated in order to prevent relapses of the disease. It is recommended to apply the solution after each foot wash.
The drug should not be used with an occlusive dressing.
Special instructions
To prevent the spread of urogenital infection, simultaneous treatment of sexual partners is necessary. In trichomoniasis, other drugs that have a systemic effect (for example, metronidazole inside) should be prescribed together with clotrimazole for more successful treatment. In the case of an unintended method of using the drug (orally), the following symptoms may occur: anorexia, nausea, vomiting, gastralgia, impaired liver function, rarely drowsiness, hallucinations, pollakiuria, allergic skin reactions. In these situations, it is necessary to take activated charcoal and consult a doctor.
If allergic reactions or irritation develop, treatment should be stopped and another therapy should be chosen. For dermatomycosis of the feet, it is recommended to use the solution after each foot wash.
It is not recommended to apply the drug on the skin in the eye area. If there is no clinical improvement after 4 weeks from the start of treatment, it is necessary to confirm the diagnosis microbiologically and exclude another cause of the disease.
