Pharmacological action
Pharmacotherapy group. An analeptic agent. ATX code: R07AB02Pharmacological action. Pharmacodynamics . Analeptic agent of mixed action. The mechanism of action consists of central and peripheral effects on the nervous system. The central mechanism is associated with a direct effect on the vasomotor center of the medulla oblongata, leading to its excitation and an indirect increase in blood pressure (especially with the initial suppression of this center). Peripheral exposure is caused by the excitation of chemoreceptors of the carotid sinus, which leads to an increase in the frequency and depth of respiratory movements. It has no direct effect on the cardiovascular system. Pharmacokinetics. Absorption is high and does not depend on the route of use. It undergoes rapid metabolism in the liver with the formation of inactive metabolites, mainly excreted by the kidneys.
Indications
Acute and chronic circulatory disorders, decreased vascular tone, weakened respiration (including in patients with infectious diseases and in convalescents), acute collapse, asphyxia, shock states during and after operations, asphyxia of newborns, poisoning with drugs, sleeping pills, analgesics.
Use during pregnancy and lactation
There are no reliable data on the safety of using the drug during pregnancy and lactation. It is not recommended to use this medicine during pregnancy and lactation.
Contraindications
Hypersensitivity, epilepsy, a history of tonic-clonic seizures, hyperthermia in children.
Side effects
From the digestive system: vomiting. Nervous system disorders: anxiety, muscle contractions (starting with the circular muscles of the mouth). From the skin: itching, flushing of the face. From the cardiovascular system: arrhythmia. Other: pain at the injection site, allergic reactions.
Interaction
The convulsive effect of Cordiamine is enhanced by reserpine, the stimulating effect on the respiratory center is eliminated by means of anesthesia, the effect of the drug is weakened by PASC and saluside, aminazine and other phenothiazine derivatives lead to a weakening of the analeptic effect and convulsive effect.
Enhances the effects of psychostimulants and antidepressants. Reduces the effect of narcotic analgesics, sleeping pills, neuroleptics, tranquilizers, anticonvulsants.
How to take, course of use and dosage
The drug is administered intravenously, intramuscularly and subcutaneously. Adults and children over 14 years of age are administered subcutaneously, intramuscularly and intravenously at a dose of 1-2 ml 1-3 times a day. Intravenous use is carried out slowly. Children-subcutaneously, depending on age, in a dose of 0.1-0.75 ml. Since subcutaneous and intramuscular injections of Cordiamine are painful, procaine (Novocaine) 1 ml of 0.5% solution is pre-injected into the site of the intended injection to reduce pain, depending on the situation. The highest doses for adults subcutaneously: single – 2 ml, daily-6 ml.
Overdose
Symptoms of overdose: increased severity of dose-dependent side effects, generalized tonic-clonic seizures.
Measures to help with overdose: treatment is symptomatic (including anticonvulsant medications).
