Composition
100 g of cream contains: Active ingredient: Â dexpanthenol-5,000 g; chlorhexidine bigluconate* – 0.776 g. Auxiliary substances: Â propylene glycol-4,500 g; lanolin-3,000 g; liquid paraffin-14,000 g; vaseline-5,000 g; dimethicone-1,000 g; cetostearyl alcohol-6,500 g; macrogol stearate-2,500 g; pantolactone-0.500 g; water-up to 100.00 g. * Chlorhexidine bigluconate is used as a 20% solution.
Pharmacological action
Dexpanthenol+Chlorhexidine is a combination drug. Dexpanthenol is a derivative of pantothenic acid. Pantothenic acid – a water-soluble B vitamin (vitamin B5) – is a component of coenzyme A (Co-A); it stimulates skin regeneration, normalizes cellular metabolism, accelerates mitosis, and increases the strength of collagen fibers. It has a weak anti-inflammatory effect.
Chlorhexidine – an antimicrobial drug, is an antiseptic. It is effective against gram-positive and Gram-negative bacteria, including Staphylococcus spp., Enterococcus spp., Pseudomonas aeruginosa, E. coli, Streptococcus spp., as well as yeast-like fungi and dermatophytes. It remains active in the presence of blood, pus, various secretions and organic substances.
The drug protects the wound surface from infections, suppresses the course of the infectious process and accelerates healing.
Pharmacokinetics
When applied externally, dexpanthenol is rapidly absorbed by the skin and converted to pantothenic acid, binds to plasma proteins (mainly beta-globulin and albumin). Its concentration in the blood is 0.5-1 mg/l, in the blood serum-100 mcg/l. Pantothenic acid is not metabolized in the body (except for inclusion in Co-A), it is excreted unchanged by the kidneys.
Chlorhexidine with external use is practically not absorbed.
Indications
Treatment and treatment of small wounds with a risk of infection, such as abrasions, scratches, small cuts and scratches, cracks, light burns; infected superficial skin lesions; cracked nipples during breastfeeding. For chronic wounds, for example, trophic ulcers of the lower leg, bedsores, as well as for surgical wounds.
Use during pregnancy and lactation
The drug Dexpanthenol+chlorhexidine can be used during pregnancy and during breastfeeding. However, its use on large skin surfaces should be avoided. If the drug is used to treat the nipples during lactation, it should be washed off before breastfeeding.
Contraindications
Hypersensitivity to the components of the drug; should not be applied to the damaged eardrum; in case of severe, deep or heavily contaminated wounds (such injuries require medical intervention).
Side effects
Allergic reactions such as contact dermatitis, allergic dermatitis, pruritus, erythema, eczema, rash, urticaria, skin irritation, anaphylactic reaction and anaphylactic shock. Interaction
As a precautionary measure regarding possible interactions (antagonism or inactivation), do not use concomitantly with other antiseptics. Chlorhexidine is incompatible with soap and other anionic compounds.
How to take, course of use and dosage
Externally. The cream is applied in a thin layer on a previously cleaned wound surface or inflamed skin area once or several times a day. Wound treatment can be carried out both in an open way and with the use of bandages. Nursing mothers should lubricate the surface of the nipple with ointment after each breast-feeding. Use the drug only according to the method of use and in the doses indicated in the instructions. If necessary, please consult your doctor before using the medicine.
Overdose
Dexpanthenol is well tolerated and non-toxic even in high doses. Cases of hypervitaminosis are not known. In cases of chlorhexidine poisoning, cases of increased aminotransferase activity have been described. In case of accidental ingestion, it is practically not absorbed. If necessary, symptomatic therapy is performed.
Special instructions
Avoid contact with the eyes, ears and mucous membranes, do not take orally (if you take large amounts by mistake, you should immediately consult a doctor).
Large, heavily soiled and deep wounds, as well as bitten and stab wounds require medical intervention (danger of tetanus).
If the size of the wound remains consistently large for some time, or the wound does not heal within 10-14 days, then you should consult a doctor. This is also necessary if the edges of the wound are very hyperemic, the wound suddenly swells and severe pain appears, or the damage is accompanied by fever (risk of sepsis).
The drug should not be used to treat skin irritations that are not prone to infection (for example, sunburn).
Frequent application to the same area of skin can lead to irritation. The drug is intended for the treatment and treatment of small wounds. avoid applying in large quantities.
Influence on the ability to drive vehicles and mechanisms.
The drug does not affect the ability to drive vehicles or engage in other potentially dangerous activities that require increased concentration of attention and speed of psychomotor reactions.
