Pharmacological action
Pharmacodynamics
A drug for the treatment of psoriasis.
Calcipotriol is a synthetic analog of the active metabolite of vitamin D, stimulates morphological differentiation and suppresses the proliferation of keratinocytes, which is the basis of its therapeutic effect in psoriasis.
Calcipotriol is a potent inhibitor of interleukin I-induced T-cell activation.
The effect of calcipotriol on calcium metabolism is 100 times weaker thanthat of vitaminD3.
Pharmacokinetics
The transdermal absorption of calcipotriol ranges from 1% to 5%, depending on the dose used. Calcipotriol undergoes rapid biotransformation in the liver to form pharmacologically inactive metabolites. T 1/2 is very short.
Indications
Psoriasis vulgar, including chronic psoriasis of the scalp.
Use during pregnancy and lactation
The safety of using the drug during pregnancy and lactation has not been established.
It is not known whether calcipotriol is excreted in breast milk.
Contraindications
- Hypersensitivity to the components of the drug.
- Diseases associated with impaired calcium metabolism.
With caution, the drug should be used for: hypercalcemia, hypercalciuria, hypervitaminosis D, nephrolithiasis. In children due to lack of experience with the use. In patients over 65 years of age.
Side effects
Dermatological reactions: possible-local skin irritation, erythema and itching of the skin; rarely-dermatitis, exacerbation of psoriasis; in some cases-hyperpigmentation, photosensitization.
From the side of metabolism: hypercalciuria, hypercalcemia.
Interaction
Divonex should not be used simultaneously with topical preparations containing salicylic acid.
How to take, course of use and dosage
The ointment is applied in a thin layer to the affected areas of the skin 2 times / day. The therapeutic effect is usually observed 1-2 weeks after the start of treatment. The duration of the course of treatment is no more than 6-8 weeks.
- For adults: the maximum daily dose is 15 g, the maximum dose per week is not more than 100 g.
- For children over 12 years of age: the maximum dose per week is 75 g.
- For children aged 6-12 years: the maximum dose per week is not more than 50 g.
Overdose
Symptoms: when using the drug in doses higher than recommended, it is possible to increase the concentration of calcium in the blood.
Treatment: discontinuation of the drug.
Special instructions
It is not recommended to apply the ointment on the face.
To avoid contact of the drug with the skin of the face, hands should be thoroughly washed after each application of the ointment.
Hypercalcemia develops when the maximum dose of calcipotriol is exceeded per week. When the drug is discontinued, the concentration of calcium in the blood quickly returns to normal.
Treatment with Divonex can be combined with PUVA therapy.
