Pharmacological action
A broad-spectrum antibacterial drug from the group of quinoxaline derivatives, it has chemotherapeutic activity in infections caused by Proteus vulgaris, Pseudomonas aeruginosa, Klebsiella pneumoniae, Escherichia coli, Shigella dysenteria, Shigella flexneri, Shigella boydii, Shigella sonnei, Salmonella spp., Staphylococcus spp., Streptococcus spp., a pathogenic anaerobe (Clostridium perfringens), acts on bacterial strains that are resistant to other chemotherapeutic agents, including antibiotics.

It is possible to develop drug resistance of bacteria. Treatment of burn and purulent-necrotic wounds promotes faster cleaning of the wound surface, stimulates reparative regeneration and marginal epithelialization, and favorably affects the course of the wound process.
When applied topically, it is partially absorbed from the wound or burn surface, and is excreted by the kidneys.
Indications
Purulent bacterial infections caused by sensitive microflora when other chemotherapeutic agents are ineffective or poorly tolerated.

Intracavitary use – purulent processes in the chest and abdominal cavity, with purulent pleurisy, pleural empyema, lung abscesses, peritonitis, cystitis, wounds with deep purulent cavities (soft tissue abscesses, pelvic cellulose phlegmons, postoperative wounds of the urinary and biliary tract, purulent mastitis, etc. ), for the prevention of infectious complications after bladder catheterization. Topical application – treatment of acute purulent otitis media with perforation and exacerbation of chronic purulent otitis media, treatment of acute tonsillopharyngitis, treatment of exacerbation of chronic tonsillitis (compensated form).
External use-superficial and deep wounds of various localization, long-term non-healing wounds and trophic ulcers, soft tissue phlegmons, infected burns, purulent wounds in osteomyelitis.
Use during pregnancy and lactation
During pregnancy and lactation, the use of Dioxydin is contraindicated.
Contraindications
Individual intolerance to the drug, adrenal insufficiency (including in the anamnesis), pregnancy, lactation, children’s age (up to 18 years).
Side effects
When administered intravenously, headache, chills, fever, dyspeptic disorders (nausea, vomiting, diarrhea), muscle cramps, allergic reactions (rash, urticaria), photosensitizing effect (the appearance of pigmented spots on the body when exposed to sunlight) are possible. When applied externally – itching, near-wound dermatitis.
How to take, course of use and dosage
Dioxydin is prescribed in a hospital setting. Apply externally, intracavitarily. 1% Dioxydin solution should not be used for intravenous use, due to the instability of the solution during storage at low temperatures.
Intracavitary use. In the purulent cavity, depending on its size,10 to 50 ml of 1% Dioxydin solution is administered per day. The dioxydin solution is introduced into the cavity through a catheter, drainage tube, or syringe. The maximum daily dose for oral use is 70 ml of a 1% solution. The drug is usually injected into the cavity once a day. If indicated, the daily dose may be administered in two divided doses. If well tolerated, the drug can be administered daily for 3 weeks or more. If necessary, repeat courses are conducted after 1-1.5 months.

Local application. In the treatment of acute purulent otitis media with perforation and exacerbation of chronic purulent otitis media, a 0.25% solution of dioxidine is used. To obtain a 0.25% solution, ampoule solutions of the drug are diluted to the desired concentration with a sterile 0.9% sodium chloride solution or water for injection in a ratio of 1: 3 (1 part Dioxidine solution and 3 parts sodium chloride solution or water for injection). The solution is prepared in a glass or plastic container. The resulting 0.25% solution is pipetted into the external ear canal for 5 drops of the solution 3 times a day. The duration of treatment for acute purulent otitis media and exacerbation of chronic purulent otitis media is 10 days (if clinical improvement is achieved, the course of treatment may be 6-8 days). The solution should be instilled in the patient lying on his side; after the procedure, the patient should remain in the same position for several minutes, it is allowed to place a cotton turunda in the external ear canal.
In the treatment of acute tonsillopharyngitis and exacerbation of chronic tonsillitis, a 0.1% solution of dioxidine is used. To obtain a 0.1% solution, ampoule solutions of the drug are diluted to the desired concentration with a sterile 0.9% sodium chloride solution or water for injection in a ratio of 1: 9 (1 part of the solution Dioxidine and 9 parts sodium chloride solution or water for injection). The resulting 0.1% solution is used for washing the tonsils with a syringe or rinsing 3 times a day. For one procedure, use 50 ml of the solution. The course of treatment is 7 days.

External use. Apply 0.1-1% Dioxidine solutions. To obtain 0.1 – 0.2% solutions, ampoule solutions of the drug are diluted to the desired concentration with a sterile 0.9% sodium chloride solution or water for injection. For the treatment of superficial infected purulent wounds, napkins moistened with 0.5-1% Dioxydin solution are applied to the wound. Deep wounds after treatment are loosely tamponed with tampons moistened with a 1% solution of Dioxidin, and in the presence of a drainage tube,20 to 100 ml of a 0.5% solution of the drug is injected into the cavity.

For the treatment of deep purulent wounds with osteomyelitis (wounds of the hand, foot),0.5-1% solutions of the drug are used in the form of baths or special treatment of the wound with a solution of the drug is carried out for 15-20 minutes (introduction of a solution into the wound for this period), followed by the application of dressings with a 1% solution of Dioxin.
Dioxydin in the form of 0.1-0.5% solutions can be used for prevention and treatment.
Overdose
An overdose of Dioxydin may lead to acute adrenal insufficiency, which requires immediate discontinuation of the drug and appropriate hormone replacement therapy.

Special instructions
Dioxydin is prescribed only for adults. Before starting the course of treatment, a drug tolerance test is performed, for which 10 ml of a 1% solution is injected into the cavity. If there are no side effects within 3-6 hours (dizziness, chills, fever), start a course of treatment.
Dioxydin is prescribed only for severe forms of infectious diseases or when other antibacterial drugs, including cephalosporins of II-IV generations, fluoroquinolones, carbapenems, are ineffective. In chronic renal failure, the dose is reduced. When pigmented spots appear, increase the duration of dose use to 1.5-2 hours, reduce the dose, prescribe antihistamines or cancel Dioxydin.
In case of crystals falling out during storage (at temperatures below 15°C). C) they are dissolved by heating the ampoules in a boiling water bath with shaking until the crystals are completely dissolved (clear solution). If the crystals do not fall out again when cooled to 36-38°C, the drug is suitable for use.
