Pharmacological action
Encephabol – metabolic, nootropic.
Pharmacodynamics
Pyritinol increases pathologically reduced metabolism in the brain by increasing glucose uptake and utilization, increases nucleic acid metabolism and acetylcholine release in nerve cell synapses, and improves cholinergic transmission between nerve tissue cells.
Pyritinol helps to stabilize the structure of the cell membrane of nerve cells and their function by inhibiting lysosomal enzymes, thereby preventing the formation of free radicals.
Pyritinol improves the rheological properties of blood, increases the plasticity of red blood cells by increasing the content of ATP in their membrane, which leads to a decrease in blood viscosity and improved blood flow.
Pyritinol improves blood circulation in the ischemic areas of the brain, increases their oxygen supply; increases glucose metabolism in the primary ischemic areas of the brain. As a result, memory indicators improve and disturbed metabolic processes in the nervous tissue are restored, which contributes to the full functioning of its cells.
Indications
- of primary degenerative dementia, vascular dementia and mixed forms;
- symptomatic treatment of chronic disorders of mental health;
- post-traumatic encephalopathy;
- symptomatic treatment of the syndrome of dementia (including primary degenerative dementia, vascular dementia and mixed forms), accompanied by impaired memory, thinking and ability to concentrate, fatigue, lack of meaning and motivation, affective disorders;
- cerebral arteriosclerosis;
- the consequences of encephalitis;
- mental retardation;
- cerebroasthenic syndrome in children;
- encephalopathy in children.
Contraindications
Absolute values: Â hyper-sensitivity.
Relative: a history of kidney disease, severe liver dysfunction, severe changes in the peripheral blood picture, acute autoimmune diseases, for example, systemic lupus erythematosus, myasthenia gravis, pemphigus.
Side effects
When taking Encephabol in accordance with the indications and in the recommended doses, no side effects should be expected.
Sometimes-hypersensitivity reactions of varying severity: rashes on the skin or mucous membranes, itching, nausea, vomiting, diarrhea, fever, sleep disorders.
Rarely — increased excitability, loss of appetite, headache, dizziness, fatigue, taste changes, liver function disorders (increased transaminases, cholestasis).
Interaction
Concomitant use of Encefabol may potentiate the side effects of penicillamine, gold preparations, and sulfasalazine.
No clinically significant interaction of Encefabol with other medicinal products has been established.
How to take, course of use and dosage
The dosage regimen is set individually, depending on the severity of the condition and the effectiveness of therapy.
Take the drug during or after a meal. For sleep disorders, the last daily dose should not be taken in the evening or at night. The duration of treatment depends on the clinical picture of the disease.
For adults, the average dose is 600 mg / day (2 tablets 3 times / day).
Newborns are prescribed Encephabol from 3 days after birth at 20 mg per day for a month, the drug should be given in the morning.
For children aged 2 months and older, the dose is increased by 20 mg every week until the daily dose reaches 100 mg.
Children aged 1 to 7 years are prescribed a daily dose of 50 mg to 300 mg, depending on the indications.
For children over 7 years of age, the daily dose is from 50 mg to 600 mg (1-2 tablets 1-3 times / day).
In acute conditions and the appointment of the drug in high doses, a noticeable therapeutic effect is achieved within a few hours or days.
In chronic diseases (including the consequences of traumatic brain injury or dementia), significant therapeutic success is achieved after 2-4 weeks of treatment. The optimal and stable effect usually occurs after 6-12 weeks. The duration of treatment for chronic diseases should be at least 8 weeks.
In newborns with a high risk of developing perinatal pathology, the average duration of treatment is 6 months, and after 3 months, you should check for indications for further treatment.
Overdose
Symptoms: increased side effects.
Treatment: gastric lavage, use of activated charcoal.
If necessary, conduct symptomatic therapy.
Special instructions
In patients with rheumatoid arthritis and other chronic joint diseases, there is an increased sensitivity to compounds that include the SH group, including pyritinol. These patients have an increased risk of hypersensitivity reactions, immunopathological reactions, as well as impaired taste sensitivity and liver function. During the treatment of this category of patients, systematic monitoring of general blood tests, urine, liver function, and immunological parameters is necessary.
Hypersensitivity reactions to the drug may occur in patients with hypersensitivity to D-penicillamine, since the latter is similar to pyritinol in chemical structure (thiol groups).
When using Encephabol for indications, as a rule, there are no restrictions for those activities that require increased attention, rapid psychomotor reactions. However, given the likelihood of individual differences in the response of individual patients to the drug, at the beginning of treatment and when increasing the dose, the possibility of a violation of the speed of psychomotor reactions should be taken into account.
