Pharmacological action – analgesic, antipyretic, anti-inflammatory.
Pharmacodynamics
It selectively blocks COX-1 and -2 and has an inhibitory effect on PG synthesis.
The analgesic effect is most pronounced in inflammatory pain. The analgesic activity of the drug does not belong to the narcotic type. It shows antiplatelet activity.
The analgesic effect when using Faspik develops 10-45 minutes after use.
Pharmacokinetics
When taken orally, it is well absorbed from the gastrointestinal tract.
When using Faspik C, the max of ibuprofen in plasma is approximately 25 or 40 mcg/ml and is reached approximately 15-30 minutes after taking the drug on an empty stomach at a dose of 200 or 400 mg, respectively.
Approximately 99% binds to plasma proteins. It is slowly distributed in the synovial fluid and is removed from it more slowly than from the plasma.
It is metabolized in the liver mainly by hydroxylation and carboxylation of the isobutyl group. The metabolites are pharmacologically inactive. It has two-phase elimination kinetics. T1/2 from plasma is 1-2 hours. Up to 90% of the dose can be detected in the urine as metabolites and their conjugates.
Less than 1% is excreted unchanged in the urine and, to a lesser extent, in the bile.
Contraindicated in children under 12 years of age.
The drug is contraindicated for use during pregnancy and lactation (breastfeeding).
The use of Faspik may negatively affect female fertility and is not recommended for women planning pregnancy.
The effectiveness of furosemide and thiazide diuretics may be reduced due to sodium retention associated with inhibition of prostaglandin synthesis in the kidneys.
Faspik may enhance the effect of oral anticoagulants, so concomitant use is not recommended.
When administered concomitantly with acetylsalicylic acid, ibuprofen reduces its antiplatelet effect (it is possible to increase the frequency of acute coronary insufficiency in patients receiving small doses of acetylsalicylic acid as an antiplatelet agent).
Faspik may reduce the effectiveness of antihypertensive drugs.
In the literature, isolated cases of increased plasma concentrations of digoxin, phenytoin and lithium have been described with simultaneous use of ibuprofen.
Faspik (like other NSAIDs) should be used with caution in combination with acetylsalicylic acid or other NSAIDs and CORTICOSTEROIDS (this increases the risk of adverse effects of the drug on the gastrointestinal tract).
Faspik may increase the concentration of methotrexate in plasma.
Combined treatment with zidovudine and Faspik may increase the risk of hemarthrosis and hematoma in HIV-infected patients with hemophilia.
The combined use of Faspik and tacrolimus may increase the risk of nephrotoxic effects due to reduced PH synthesis in the kidneys.
Ibuprofen increases the hypoglycemic effect of oral hypoglycemic agents and insulin; dose adjustment may be necessary.
The daily dose of Faspik when taken orally is 0.6-1.2 g in 3-4 doses, the drug is also used in suppositories rectally and ointment externally.
Children are prescribed 20-40 mg per kg of body weight per day.
Symptoms:  abdominal pain, nausea, vomiting, lethargy, drowsiness, depression, headache, tinnitus, metabolic acidosis, coma, acute renal failure, decreased blood pressure Blood pressure, bradycardia, tachycardia, atrial fibrillation, respiratory arrest.
Treatment: Â gastric lavage (only within an hour after ingestion), use of activated charcoal, alkaline drinking, forced diuresis, symptomatic therapy (correction of the acid-base state, blood pressure).
Use with caution in patients with liver and kidney diseases.
Long-term use of Faspik without consulting a doctor is not recommended.
