Pharmacological action
of Festal – lipolytic, amylolytic, proteolytic, normalizing the functions of the pancreas, choleretic.
Pharmacodynamics
Compensates for the insufficiency of the external secretory function of the pancreas due to pancreatin and biliary function of the liver due to the bile component.
It has proteolytic, amylolytic and lipolytic effects. The enzymes that make up pancreatin — amylase, lipase and protease-facilitate the digestion of carbohydrates, fats and proteins, which contributes to their more complete absorption in the small intestine. In this respect, it practically does not differ from other enzyme preparations.
The presence of bile acids provides additional opportunities for correcting biliary insufficiency, which often accompanies chronic pancreatitis.
Bile extract acts as a choleretic agent, facilitates the absorption of fats and fat-soluble vitamins A, E and K, promotes the release of lipase by the pancreas.
The enzyme hemicellulase promotes the breakdown of plant fiber, which also improves digestive processes, reduces the formation of gases in the intestines.
Indications
- insufficiency of exocrine pancreatic function in chronic pancreatitis combined with biliary insufficiency due to various pathological conditions, clinically manifested by a violation of digestion, flatulence, tendency to constipation, as part of combination therapy in diffuse liver disease — alcoholic and toxic liver damage, cirrhosis of the liver;
- large losses of bile acids (in patients after cholecystectomy); violation of circulation of bile acids observed in biliary dyskinesia, disbacteriosis, malabsorption; breach of neurohumoral regulation of the processes of bile production and secretion in chronic gastrointestinal diseases, chronic gastritis, chronic duodenitis, chronic cholecystitis;
- to improve food digestion in patients with normal function of the digestive tract in the case of errors in the diet, and in violation of the chewing function, forced prolonged immobilization, sedentary lifestyle;
- preparation for x-ray and ultrasound of the abdominal cavity.
Use during pregnancy and lactation
It is possible to use the drug during pregnancy and lactation (breastfeeding )according to indications.
Contraindications
- hypersensitivity to the components of the drug;
- acute pancreatitis, exacerbation of chronic pancreatitis;
- liver failure, hepatic coma or precoma;
- hepatitis;
- hyperbilirubinemia;
- mechanical jaundice, gallstone disease;
- empyema of the gallbladder, intestinal obstruction;
- tendency to diarrhea;
- children under 3 years of age (for this dosage form).
With caution: Â cystic fibrosis, pregnancy.
Side effects
Allergic reactions (hyperemia of the skin, sneezing, lacrimation), nausea, diarrhea, abdominal pain (including intestinal colic).
With prolonged use in high doses, hyperuricemia and hyperuricuria may develop.
Children (when taken in large doses) may develop perianal irritation and irritation of the oral mucosa.
Interaction
When used concomitantly, it enhances absorption PASC, sulfonamides, antibiotics; may reduce the absorption of iron preparations.
Concomitant use of antacids containing calcium carbonate and / or magnesium hydroxide may reduce the effectiveness of Festal®.
Cimetidine enhances the effect of the drug.
How to take, course of use and dosage
Inside, without chewing, during or immediately after a meal, with a small amount of liquid.
Assign 1-2 pills 3 times a day. Higher doses are prescribed only by a doctor.
Before X-ray or ultrasound examination — 2 tablets 2-3 times a day for 2-3 days before the study.
The dosage regimen for children is determined by the doctor.
The duration of treatment can vary from several days (if the digestive process is disrupted due to dietary errors) to several months or years (if permanent replacement therapy is required).
Overdose
Symptoms: Â hyperuricemia and hyperuricuria.
Treatment: Â symptomatic therapy, drug withdrawal.
Special instructions
With caution, the drug is prescribed to patients with a history of chronic pancreatitis.
It should be borne in mind that with high activity of lipase contained in pancreatin, the likelihood of developing severe constipation in children increases. In clinical practice, cases of fibrotic colonopathy have been observed in children with cystic fibrosis who received daily preparations containing pancreatin in high doses.
Digestive disorders may occur in patients with hypersensitivity to pancreatin, or in patients with a history of meconium ileus or intestinal resection.
When using pancreatin in high doses, in some cases, perianal irritation may occur.
