Pharmacological action
The drug Festal compensates for the insufficiency of the external secretory function of the pancreas due to pancreatin, and the biliary function of the liver due to the bile component. Festal has proteolytic, amylolytic and lipolytic effects.
The enzymes included in pancreatin: amylase, lipase and protease facilitate the digestion of carbohydrates, fats and proteins, which contributes to their more complete absorption in the small intestine. And in this respect, Festal practically does not differ from other enzyme preparations.
The presence of bile acids in Festal gives the drug additional opportunities for correcting biliary insufficiency, which often accompanies chronic pancreatitis. Bile extract acts as a choleretic agent, facilitates the absorption of fats and fat-soluble vitamins A, E and K, promotes the release of lipase by the pancreas.
The enzyme hemicellulase promotes the breakdown of plant fiber, which also improves digestive processes, reduces the formation of gases in the intestines.
Indications
Insufficiency of external secretory function of the pancreas in chronic pancreatitis in combination with biliary insufficiency due to various pathological conditions, clinically manifested by impaired digestion of food, flatulence, a tendency to constipation, as part of combination therapy for:
- diffuse liver diseases – alcoholic and toxic liver damage, cirrhosis of the liver;
- large losses of bile acids (in patients after cholecystectomy);
- violation of bile acid circulation, observed in dyskinesia. biliary tract diseases, dysbiosis, malabsorption;
- violation of neurohumoral regulation of bile formation and bile secretion in chronic diseases of the gastrointestinal tract -chronic gastritis, chronic duodenitis, chronic cholecystitis
To improve food digestion in patients with normal gastrointestinal tract function in case of nutritional errors, as well as in cases of masticatory disorders, forced prolonged immobilization, and a sedentary lifestyle.
Preparation for X-ray and ultrasound examination of the abdominal organs.
Use during pregnancy and lactation
It is possible to use the drug during pregnancy and lactation (breastfeeding )according to indications.
Contraindications
- Hypersensitivity to the components of the drug Festal,
- acute pancreatitis,
- exacerbation of chronic pancreatitis,
- liver failure,
- hepatic coma or precoma,
- hepatitis,
- hyperbilirubinemia,
- mechanical jaundice,
- cholelithiasis,
- gallbladder empyema,
- intestinal obstruction,
- tendency to diarrhea,
- children under 3 years of age (for this dosage form).
With caution: cystic fibrosis, pregnancy.
Side effects
Allergic reactions (hyperemia of the skin, sneezing, lacrimation), nausea, diarrhea, abdominal pain (including intestinal colic).
With prolonged use in high doses, hyperuricemia and hyperuricuria may develop.
Children (when taken in large doses) may develop perianal irritation and irritation of the oral mucosa.
Interaction
When used concomitantly with Festal, the absorption of PASC (p-aminosalicylic acid), sulfonamides, and antibiotics increases; the absorption of iron preparations may decrease.
Concomitant use of antacids containing calcium carbonate and / or magnesium hydroxide may reduce the effectiveness of Festal. Cimetidine enhances the effect of the drug.
How to take, course of use and dosage
Inside.
The drug Festal is taken orally, without chewing, during or immediately after a meal, with a small amount of liquid. Assign 1-2 pills 3 times a day.
Higher doses are prescribed only by a doctor.
The dosage and dosage regimen for children is determined by the doctor.
The duration of treatment can vary from several days (if the digestive process is disrupted due to dietary errors) to several months or years (if permanent replacement therapy is required).
Before X-ray or ultrasound examination – 2 tablets 2-3 times a day for 2-3 days before the study.
Overdose
Overdose symptoms include hyperuricemia and hyperuricuria.
If symptoms of overdose occur, discontinue the drug and conduct symptomatic therapy.
Special instructions
With caution, the drug is prescribed to patients with a history of chronic pancreatitis.
It should be borne in mind that with high activity of lipase contained in pancreatin, the likelihood of developing severe constipation in children increases. In clinical practice, cases of fibrotic colonopathy have been observed in children with cystic fibrosis who received daily preparations containing pancreatin in high doses.
Digestive disorders may occur in patients with hypersensitivity to pancreatin, or in patients with a history of meconium ileus or intestinal resection.
When using pancreatin in high doses, in some cases, perianal irritation may occur.
