Pharmacological action
Fluimucil is a mucolytic drug with antioxidant properties. Dilutes sputum, increases its volume and facilitates its separation. The action of acetylcysteine is associated with the ability of its sulfhydryl groups to break intra – and intermolecular disulfide bonds of acidic mucopolysaccharides of sputum, which leads to depolarization of mucoproteins and a decrease in sputum viscosity.
It remains active in the presence of purulent sputum.
It has an antioxidant effect due to the presence of the SH group, which can neutralize electrophilic oxidative toxins.
It also has an anti-inflammatory effect (by suppressing the formation of free radicals and active oxygen-containing substances responsible for the development of inflammation in the lung tissue).
Indications
Diseases of the respiratory system, accompanied by the formation of sputum of increased viscosity (acute and chronic bronchitis, pneumonia, bronchiectasis, cystic fibrosis, bronchial asthma); to facilitate the discharge of mucus in sinusitis.

Use during pregnancy and lactation is only possible if the intended benefit to the mother exceeds the potential risk to the fetus or child.
Contraindications
Hypersensitivity to acetylcysteine, peptic ulcer of the stomach and duodenum in the acute stage.
Caution should be exercised when prescribing the drug to patients with liver and kidney diseases.
Side effects
From the digestive system: Â rarely – heartburn, nausea, vomiting, diarrhea, stomatitis.
Allergic reactions: Â rarely – skin rash, pruritus, urticaria, bronchospasm.
Other services: Â rarely-nosebleeds, tinnitus, collapse, decreased platelet aggregation.
Local reactions: Â with parenteral use, a slight burning sensation at the injection site is possible; with inhalation use, reflex cough, local irritation of the respiratory tract, stomatitis, rhinitis; rarely-bronchospasm (in this case, the appointment of bronchodilators is necessary).
Interaction
The combined use of acetylcysteine with antitussive agents may increase sputum congestion due to suppression of the cough reflex.
When used simultaneously with such antibiotics as tetracyclines (excluding doxycycline), ampicillin, amphotericin B, their interaction with the thiol group of acetylcysteine is possible, which leads to a decrease in the activity of both drugs. Therefore, the interval between taking these drugs should be at least 2 hours.
Concomitant use of acetylcysteine and nitroglycerin may cause a marked decrease in blood pressure and headache.
Concomitant use of acetylcysteine and carbamazepine may result in subtherapeutic levels of carbamazepine.
Activated carbon can reduce the effect of acetylcysteine.
Acetylcysteine eliminates the toxic effects of paracetamol.
Acetylcysteine may affect the results of colorimetric determination of salicylates.
Acetylcysteine may affect the analysis of ketones in the urine.
How to take, course of use and dosage
Granules for the preparation of a solution for oral use are dissolved in 1/3 cup of water.
Adults and children over 6 years of age-200 mg 2-3 times a day.
For newborns – only for vital indications at a dose of 10 mg / kg under strict medical supervision. Children of the first year of life are allowed to drink the resulting solution from a spoon or feeding bottle. Children from 1 to 2 years – 100 mg 2 times a day, from 2 to 6 years-200 mg 2 times a day or 100 mg 3 times a day.
The duration of treatment should be evaluated individually. In acute diseases, the duration of treatment is from 5 to 10 days and up to several months in the treatment of chronic diseases (on the recommendation of a doctor).
Overdose
Acetylcysteine when taken at a dose of 500 mg / kg / day does not cause signs and symptoms of overdose.
Symptoms The following symptoms may occur: nausea, vomiting, and diarrhea.
Treatment: There is no specific antidote, and the treatment is symptomatic.
Special instructions
Patients with bronchial asthma and obstructive bronchitis can only be prescribed Fluimucil if adequate sputum drainage is provided.
Dosage forms for oral use contain aspartame, so they should not be prescribed to patients with phenylketonuria.
When dissolving the drug, it is necessary to use glassware, avoid contact with metal and rubber surfaces.
