Indications
Shortness of sputum (bronchitis, tracheitis, bronchiolitis, pneumonia, bronchiectasis), cystic fibrosis, lung abscess, emphysema, laryngotracheitis, interstitial lung diseases, bronchial asthma, atelectasis (due to blockage of the bronchial mucous plug), catarrhal and purulent otitis media, sinusitis, including sinusitis, remove viscous secretion from the respiratory tract in post-traumatic and postoperative conditions, paracetamol poisoning (the antidote).
Contraindications
Hypersensitivity, peptic ulcer of the stomach and duodenum in the acute stage.
Side effects
From the digestive tract: Â nausea, vomiting, heartburn, feeling of fullness of the stomach, stomatitis.
Allergic reactions: Â skin rash, pruritus, urticaria, bronchospasm (mainly in patients with hyperreactivity of the bronchi).
Other services: Â drowsiness, fever; rarely-tinnitus; reflex cough, local irritation of the respiratory tract, rhinorrhea (with inhalation); burning sensation at the injection site (with parenteral use).
Interaction
The combined use of acetylcysteine with antitussive agents may increase sputum congestion due to suppression of the cough reflex.
When used simultaneously with such antibiotics as tetracyclines (excluding doxycycline), ampicillin, amphotericin B, their interaction with the thiol group of acetylcysteine is possible, which leads to a decrease in the activity of both drugs. Therefore, the interval between taking these drugs should be at least 2 hours.
Simultaneous use of acetylcysteine and nitroglycerin may lead to increased vasodilating and antiplatelet effects of the latter.
If concomitant use of acetylcysteine and nitroglycerin is necessary, patients should be monitored for hypotension, which may be severe, and they should be warned about the possibility of headaches. Activated charcoal can reduce the effectiveness of acetylcysteine.
Acetylcysteine eliminates the toxic effects of paracetamol.
How to take, course of use and dosage
For oral use.
Adults: 5 ml of oral solution 40 mg / ml in 1-3 doses (corresponds to 200-600 mg of acetylcysteine per day).
For children:
– children from 2 to 5 years: 5 ml of oral solution 20 mg / ml in 2-3 doses (corresponds to 200-300 mg of acetylcysteine per day)
– children and adolescents from 6 to 14 years: 5 ml of oral solution 20 mg / ml in 3-4 doses (corresponds to 300-400 mg of acetylcysteine per day) or 5 ml of oral solution 40 mg/ml in 2 doses (corresponds to 400 mg of acetylcysteine per day)
– children over 14 years of age: 5 ml of oral solution 40 mg / ml in 1-3 doses (corresponds to 200-600 mg of acetylcysteine per day).
In case of paracetamol overdose: in the first 10 hours after use, Fluimucil is administered as soon as possible at the rate of 140 mg/kg, more often at the rate of 70 mg/kg every 4 hours, for 1-3 days.
Overdose
Symptoms: Overdose may be manifested by gastrointestinal symptoms such as nausea, vomiting, and diarrhea.
Treatment: symptomatic therapy. There is no specific antidote.
Special instructions
In patients with asthma and obstructive bronchitis, acetylcysteine should be administered with caution due to the possibility of developing bronchospasm. If bronchospasm occurs, treatment with acetylcysteine should be stopped immediately. The use of acetylcysteine, mainly at the beginning of treatment, can cause dilution of bronchial secretions and increase its volume. If the patient is unable to effectively clear sputum, postural drainage or aspiration of sputum from the bronchi is necessary.
Fluimucil contains aspartame, a source of phenylalanine. This should be taken into account in patients suffering from phenylketonuria.
The drug also contains sorbitol and sucrose, which should not be used in patients with hereditary fructose intolerance, as well as glucose-galactose malabsorption and sucrose-isomaltase insufficiency.
It is recommended to use with caution in patients with a history of gastric and duodenal ulcers, especially in cases of concomitant use of other medications that irritate the gastric mucosa.
