Pharmacological action
Glidiab MV is a hypoglycemic drug, a second-generation sulfonylurea derivative. Glidiab stimulates the secretion of insulin by the pancreas, potentiates the insulin-secreting effect of glucose, and increases the sensitivity of peripheral tissues to insulin.
It stimulates the activity of intracellular enzymes of muscle glycogen synthetase.
Reduces the time period from the moment of food intake to the beginning of insulin secretion. Glidiab restores the early peak of insulin secretion (unlike other sulfonylurea derivatives, such as glibenclamide and chlorpropamide, which act mainly during the second phase of secretion).
Reduces the peak of hyperglycemia after a meal.
Indications
- non-insulin-dependent diabetes mellitus (type II) of moderate course (including complicated by microangiopathy);
- prevention of hemorheological disorders in patients with non-insulin-dependent diabetes mellitus (as part of combination therapy with other sulfonamide hypoglycemic drugs).
Use during pregnancy and lactation
The drug is contraindicated for use during pregnancy and lactation (breastfeeding).
Contraindications
Contraindications of the drug GLIDIAB® MV
- diabetes mellitus type 1;
- diabetic ketoacidosis;
- diabetic precoma, diabetic coma;
- hyperosmolar coma;
- severe hepatic and/or renal failure;
- extensive surgical intervention, extensive burns, trauma, and other conditions, requiring insulin;
- intestinal obstruction;
- paresis of the stomach;
- conditions associated with malabsorption of food, the development of hypoglycemia (infectious diseases);
- leukopenia;
- pregnancy;
- lactation (breastfeeding);
- hypersensitivity to the components of the drug.
The drug should be used with caution (the need for more careful monitoring and dose selection) in patients with febrile syndrome, thyroid diseases (with impaired thyroid function), and alcoholism.
Side effects
From the digestive tract: very rarely — dyspeptic phenomena (nausea, vomiting, abdominal pain), extremely rarely-jaundice.
From the cardiovascular system and blood: reversible cytopenia, eosinophilia, anemia.
From the skin: rarely – allergic skin reactions, photosensitization.
From the side of metabolism: hypoglycemia.
From the nervous system and sensory organs: weakness, headache, dizziness, changes in taste sensations.
Interaction
Hypoglycemic effects of the drug are enhanced when used concomitantly with ACE inhibitors, histamine H2-receptor blockers (cimetidine), antifungal drugs (miconazole, fluconazole), NSAIDs (phenylbutazone, Indometacin, diclofenac), fibrates (clofibrate, bezafibrate), anti-tuberculosis drugs (ethionamide), salicylates, indirect coumarin-type anticoagulants, anabolic steroids, beta-blockers, cyclophosphamide, chloramphenicol, MAO inhibitors, long-acting sulfonamides, with fenfluramine, fluoxetine, pentoxifylline, guanethidine, theophylline, with drugs that block tubular secretion, with reserpine, bromocriptine, disopyramide, pyridoxine, allopurinol, ethanol and ethanol-containing drugs, as well as other hypoglycemic drugs (acarbose, biguanides, insulin).
The hypoglycemic effect of the drug decreases when used simultaneously with barbiturates, corticosteroids, sympathomimetics (epinephrine, clonidine, ritodrin, salbutamol, terbutaline), phenytoin, slow calcium channel blockers, carbonic anhydrase inhibitors (acetazolamide), thiazide diuretics, chlortalidone, furosemide, triamtaren, asparaginase, baclofen, danazol, diazoxide, isoniazid, morphine, glucagon, rifampicin, with thyroid hormones, with lithium preparations, with nicotinic acid in high doses, with chlorpromazine, estrogens and oral contraceptives containing them. When interacting with ethanol, a disulfiram-like reaction can develop. Gliclazide increases the risk of ventricular extrasystole when taking cardiac glycosides. Beta-blockers, clonidine, reserpine, and guanethidine can mask the clinical manifestations of hypoglycemia.
How to take, course of use and dosage
The dose of the drug should be selected individually, depending on the clinical manifestations of the disease, fasting glucose levels and 2 hours after meals.
Initial daily dose (including for elderly patients over 65 years of age) it is 30 mg (1 tablet). In the future, if necessary, the dose of the drug is increased at intervals of at least 2 weeks. The maximum daily dose is 120 mg (4 tablets). The drug is taken orally 1 time / morning during breakfast.
With Glidiab® MB can be replaced with Glidiab® in doses from 1 to 4 tablets/
Glidiab® MB can be used in combination with other hypoglycemic agents: biguanides, alpha-glucosidase inhibitors, or insulin.
In patients with mild to moderate renal impairment (creatinine clearance from 15 to 80 ml / min), the drug is prescribed in the same doses.
Overdose
Symptoms: hypoglycemia is possible, up to the development of hypoglycemic coma.
Treatment: if the patient is conscious-take easily digestible carbohydrates (sugar) inside, if he loses consciousness-enter 40% dextrose (glucose) solution intravenously, enter 1-2 mg of glucagon intravenously. After regaining consciousness, the patient should be given food rich in easily digestible carbohydrates in order to avoid the re-development of hypoglycemia.
Special instructions
Treatment is carried out against the background of a low-calorie diet with a low carbohydrate content. It is necessary to regularly monitor the level of glucose in the blood on an empty stomach and after meals, the daily curve of glucose levels in the blood and urine. With physical and emotional overstrain, changing the diet, dose adjustment is necessary.
