Pharmacological action
Glucose enhances redox processes in the body, improves the antitoxic function of the liver, covers part of the body’s energy expenditure, as it is a source of easily digestible carbohydrates. Entering the tissues, it is phosphorylated, turning into glucose-6-phosphate, which is actively involved in many parts of the body’s metabolism.
The 400 mg/ml solution is hypertonic. When a hypertonic solution is injected into a vein, the osmotic pressure of the blood increases, the contractile activity of the heart muscle increases, the vessels dilate, the detoxification function of the liver improves, and the diuresis increases.
Pharmacokinetics
It is completely absorbed by the body, not excreted by the kidneys (the appearance in the urine is a pathological sign).
Indications
Hypoglycemia, insufficient carbohydrate nutrition, toxicoinfection, intoxication in liver diseases (hepatitis, dystrophy and atrophy of the liver, including liver failure), hemorrhagic diathesis; intoxication; collapse, shock. As a component of various blood-substituting and anti-shock fluids; for the preparation of drug solutions for intravenous use.
Contraindications
Hypersensitivity, hyperglycemia, hyperlactacidemia, hyperhydration, postoperative glucose utilization disorders; circulatory disorders that threaten brain and lung edema; brain edema, pulmonary edema, acute left ventricular failure, hyperosmolar coma, childhood.
With caution
Decompensated chronic heart failure, chronic renal failure (oligo -, anuria), hyponatremia, diabetes mellitus.
Side effects
Ion balance disorders, hyperglycemia, glucosuria, fever, hypervolimia, acute left ventricular failure.
At the injection site – the development of infection, thrombophlebitis.
Interaction
When combined with other medicinal products, it is necessary to visually monitor compatibility (possible invisible pharmaceutical or pharmacodynamic incompatibilities).
How to take, course of use and dosage
Intravenous drip – up to 30 drops/min (1.5 ml / min); the maximum daily dose for adults is 250 ml.
For a more complete assimilation of dextrose, administered in large doses, simultaneously with it, insulin is prescribed at the rate of 1 UNIT of insulin per 4-5 g of dextrose.
In diabetic patients, dextrose is administered under the control of its content in the blood and urine.
Overdose
Symptoms: hyperglycemia, glucosuria, hyperglycemic hyperosmolar coma, hyperhydration, impaired water and electrolyte balance. Treatment: stop glucose use, enter insulin, symptomatic therapy.
Special instructions
Glucose solution should not be administered quickly or for a long time. If chills occur during the infusion of the solution, the use should be stopped immediately. To prevent thrombophlebitis, the solution should be administered slowly, through large veins. When combined with other drugs, it is necessary to visually monitor compatibility. Patients with diabetes mellitus are given dextrose under the control of its content in the blood and urine.
