Composition
100 g of gel contains: Active ingredients: sodium heparin in terms of dry matter – 0.417 g, dexpanthenol-2.5 g, dimethyl sulfoxide-15.0 g. Auxiliary substances: carbomer 940 (carbopol 980), trometamol (trometamine), lavender oil, cremophor RH 40 (macrogolglycerol hydroxystearate, polyoxyl 40 hydrogenated castor oil), isopropanol (isopropyl alcohol), purified water.
Pharmacological action
Pharmacotherapy group: Direct-acting anticoagulant for topical use + other DRUGSPharmacodynamics : The action of the drug is due to the active substances included in its composition: sodium heparin, dexpanthenol, dimethyl sulfoxide. Dimethyl sulfoxide has an anti-inflammatory and anti-edematous effect due to the inactivation of hydroxyl radicals and the improvement of metabolic processes in the focus of inflammation. It has a local analgesic effect by reducing the speed of nociceptive (pain) impulses in peripheral neurons. Dimethyl sulfoxide penetrates through biological membranes (from 50% or more), including through the skin, contributing to a deeper penetration of other ingredients of the drug into the tissues. Heparin is a direct-acting anticoagulant, has an anti-inflammatory effect, promotes the regeneration of connective tissue by inhibiting the activity of hyaluronidase; prevents thrombosis, activates the fibrinolytic properties of blood; improves local blood flow. Dexpanthenol has an anti-inflammatory, dermatoprotective effect. By improving metabolic processes, it promotes the regeneration of damaged tissues. Pharmacokinetics: The normal physiological concentration of dimethyl sulfoxide (DMSO) in blood plasma is 40 ng / ml. 6 hours after application of the drug, the maximum concentration in blood plasma reaches 120 ng / ml and persists for up to 12 hours after application. 12-25% of absorbed DMSO is eliminated within the first 24 hours and 37-48% is eliminated within 7 days unchanged through the kidneys or as a metabolite (dimethyl-sulfone).3.5-6% of all DMSO is excreted through the lungs in the form of dimethyl sulfide 6-12 hours after use of the drug. The half-life of DMSO is 11-14 hours. Heparin for external use is slightly absorbed. Dexpanthenol when applied externally after absorption is converted to pantothenic acid, which is part of coenzyme A, which plays an important role in the processes of acetylation and oxidation.
Indications
-Edema, hematomas and inflammation of soft tissues, muscles, tendons, tendon sheaths; – closed injuries, bruises;- joint injuries with sprains and tendons;- epicondylitis of the shoulder (“tennis player’s elbow”), tendinitis (inflammation of the tendons), tendovaginitis (inflammation of the tendon sheaths), bursitis (inflammation of the mucous bag of the joint);- periarthritis of the shoulder joint; – acute neuralgia.
Use during pregnancy and lactation
Due to the fact that the experience of using dimethyl sulfoxide in pregnant women is still insufficient, the use of the drug during pregnancy is contraindicated. Dimethyl sulfoxide penetrates into breast milk, so during the use of the drug, you should stop breastfeeding the baby.
Contraindications
-Hypersensitivity to the components of the drug;- bronchial asthma;- severe renal and / or hepatic insufficiency; – severe disorders of the cardiovascular system;- pregnancy, breast-feeding period; – children’s age (up to 5 years);- open wounds at the site of application.
Side effects
Local skin reactions (redness, itching, and burning sensation at the site of gel application) are possible, which usually gradually disappear during treatment. Sometimes an allergic skin reaction may occur. In some cases, allergic reactions of an immediate type may occur (urticaria, angioedema). In rare cases, during the use of the drug, some patients may smell garlic from the mouth. The smell is caused by dimethyl sulfide, which is a product of dimethyl sulfoxide metabolism. It is possible to change the taste sensations that disappear within a few minutes after applying the gel. Very rarely, when the drug is applied to large areas of the body, nausea, diarrhea, difficulty breathing, headache, chills may occur. In case of adverse reactions when using the drug, it is recommended to consult a doctor.
Interaction
Topical application of other drugs simultaneously with the drug helps to increase their permeability through the skin. Severe toxic reactions (peripheral neuropathy) may occur when Heparidex® is co-administered with medicinal products containing sulindac (a nonsteroidal anti-inflammatory drug).
How to take, course of use and dosage
The drug is applied with a thin surface layer on the skin over the area of soft tissue, joints or tendons (edema, hematoma, inflammation, trauma) and evenly distributed with light rubbing movements. The procedure is repeated 2-4 times a day for 7-10 days or until such symptoms of the disease as pain, swelling, swelling, and inflammation disappear. If there is no clear improvement within 10 days of treatment, you should consult a doctor. Heparidex® can be used for iontophoresis. Keep in mind that electric shock treatment can cause local skin irritation reactions. Therefore, it is necessary to carefully follow the instructions of the device manufacturers so that the use of the drug does not increase these local reactions. Heparidex® can be used as a contact gel in ultrasound therapy (phonophoresis). The active ingredients of the gel (dimethyl sulfoxide and heparin) complement the therapeutic effect of ultrasound waves. It is not recommended to use the drug for more than 8 days without consulting a doctor.
Overdose

Special instructions
Before applying the gel, it is necessary to thoroughly clean the skin area from other medications and dirt. Applying bandages is possible only after the penetration of most of the gel into the skin and evaporation of the alcohol contained in its composition (after a few minutes). Heparidex® should not be applied to the mucous membranes of the eyes, nose, mouth, open wounds or damaged skin (due to radiation, severe sunburn, postoperative scars). During treatment with the drug, the light sensitivity of the skin may increase, so during its use, intensive sun baths and visits to the solarium should be limited. If skin reactions occur, treatment should be discontinued. Due to the high absorption of dimethyl sulfoxide, Heparidex® should not be used in combination with other ointments and gels. When using the drug independently, do not exceed the recommended doses and maximum terms of use of the drug. If there is no reduction or if the symptoms of the disease become more severe, it is recommended to consult a doctor. Influence on the ability to drive vehicles and mechanisms: The drug does not affect the ability to drive vehicles and engage in other potentially dangerous activities that require increased concentration of attention and speed of psychomotor reactions.
