Thrombophlebitis of superficial veins; localized infiltrates and edema of soft tissues; injuries of tendons and joints, bruises of soft tissues and joints.
Hypersensitivity to the components of the drug, childhood, ulcerative-necrotic processes in thrombophlebitis, reduced blood clotting, thrombocytopenia, violation of the integrity of the skin (due to the possibility of increased bleeding).
Heparin sodium is contraindicated in hemorrhagic diathesis, leukemia, anemia, increased vascular permeability, polyps, malignant neoplasms and ulcerative lesions of the gastrointestinal tract, severe liver and kidney function disorders, brain and spine operations.
Benzyl nicotinate is contraindicated in paroxysmal tachycardia.
Benzocaine (anastezine) is contraindicated in patients with hypersensitivity to this drug.
With caution
Caution in the use of heparin is required for ulcerative and neoplastic lesions of the gastrointestinal tract, cachexia, high blood pressure, in the immediate postpartum and postoperative period (during the first 3-8 days).
Restriction to the use of benzyl nicotinate is a prostate adenoma with a violation of the outflow of urine.
Restrictions on the use of benzocaine (anastezine) are: breastfeeding (should be suspended), infancy (up to 1 year).

Combined preparation for external use, the action of which is determined by the properties of its constituent components. Direct-acting anticoagulant. Gradually released from the ointment, sodium heparin reduces the inflammatory process and has an antithrombotic effect. Promotes resorption of existing ones and prevents the formation of new blood clots. Blocks the synthesis of thrombin, reduces platelet aggregation. Inhibits the activity of hyaluronidase, activates the fibrinolytic properties of blood.
Benzyl nicotinate has a vasodilating, antispasmodic effect. Directly reduces the contractile activity of smooth muscles of blood vessels and internal organs. Benzyl nicotinate, expanding the surface vessels, promotes the absorption of heparin.
Local antiseptic benzocaine (anastezine), reducing the permeability of the cell membrane for sodium ions, blocks the conduction of nerve impulses. Prevents the occurrence of pain impulses in the endings of sensitive nerves and their conduction along nerve fibers, reduces the severity of pain. When applied to the skin, it has a local analgesic effect.
Pharmacokinetics
When applied topically, only a small amount of sodium heparin is absorbed into the systemic circulation, which is mainly excreted by the kidneys.
The pharmacokinetics of the active ingredients – benzyl nicotinate and benzocaine (anastezine) – when applied topically have not been studied.

The use of the drug during pregnancy and lactation is possible only under strict indications and under careful medical supervision.
The drug is not prescribed in the immediate postpartum period (during the first 3-8 days) due to the possibility of increased bleeding as a result of the action of sodium heparin and benzyl nicotinate.
Breastfeeding should be suspended if the use of the drug is necessary during this period (due to the presence of benzocaine (anastezine) in the composition.

Possible allergic reactions – skin rash, angioedema. If such symptoms occur, the drug is discontinued and desensitizing drugs are prescribed.
When using heparin, it is necessary to take into account the possibility of developing hemorrhages. Along with hemorrhages, the use of heparin can lead to the development of thrombocytopenia, osteoporosis, alopecia and hypoaldosteroism.
When using benzyl nicotinate, there may be a transient increase in the activity of hepatic transaminases, tachycardia, leukopenia, and sometimes impaired renal function.
When using benzocaine (anesthesin), contact dermatitis is occasionally possible; decreased sensitivity at the site of application; rarely, with excessive use – methemoglobinemia (especially in children and young children).
The effect of heparin is enhanced by anticoagulation drugs, acetylsalicylic acid and other nonsteroidal anti-inflammatory drugs (increases the risk of bleeding), weakened by cardiac glycosides (digitalis preparation), tetracyclines, H1 – histamine receptor blockers, nicotinic acid, etacric acid. Benzyl nicotinate increases the likelihood of tachyarrhythmias on the background of sympathomimetics and the risk of cardiac depression under the influence of potassium-withdrawing drugs. Cholinesterase inhibitors inhibit the metabolism of benzocaine (anaesthesin).
Externally. The ointment is applied in a thin layer to the affected area and gently rubbed into the skin, without massaging,2-3 times a day until the inflammatory phenomena disappear, on average from 3 to 14 days. The maximum daily dose of the drug is 25.0 g. The possibility of a longer course of treatment is determined by the doctor.
For thrombosis of external hemorrhoids, rectal swabs soaked in ointment are used, which are inserted into the anus. Or the ointment is applied to a calico or linen pad, which is applied directly to the thrombosed nodes and fixed. The ointment should be applied daily, until symptoms disappear, on average from 3 to 14 days.
With trophic ulcers of the lower leg, the ointment is applied only to the surrounding areas of the skin with undisturbed integrity of the skin.
Since the drug in therapeutic doses has a predominantly local effect, cases of overdose are unlikely. With prolonged use and on large surfaces, hemorrhagic complications (heparin) are possible. In these cases, the drug is discontinued and symptomatic therapy is prescribed. Symptoms of overdose of the components included in the composition when applied topically have not been studied.
The ointment is white with a yellowish tinge.
Do not apply the ointment to open wounds, mucous membranes, in the presence of purulent processes on the skin. The use of the drug is not recommended for deep venous thrombosis.
With prolonged use of heparin, blood clotting time monitoring is necessary.
With prolonged use of benzyl nicotinate, monitoring of liver function and a general blood test is necessary.
Influence on the ability to drive vehicles and mechanisms
The drug does not affect the ability to drive vehicles and engage in other potentially dangerous activities that require increased concentration of attention and speed of psychomotor reactions.
Ointment for external use in aluminum tubes with bouchons with an internal varnish coating of 10.0 g or 25.0 g. Each tube together with the instructions for use is placed in a cardboard pack.
