Pharmacological action
Antiviral agent for external use, a synthetic analogue of purine nucleoside. Thymidine kinase of virus-infected cells actively converts acyclovir through a series of sequential reactions to mono -, di-and triphosphate of acyclovir. The latter interacts with viral DNA polymerase and is embedded in the DNA that is synthesized for new viruses. Thus, “defective” viral DNA is formed, which leads to suppression of replication of new generations of viruses. Acyclovir is active against Herpes simplex virus types 1 and 2, Varicella zoster virus, Epstein-Barr virus and cytomegalovirus. Pharmacokineticsfor external use, systemic absorption is minimal.
Indications
Treatment of skin infections caused by Herpes simplex viruses types 1 and 2, including primary and recurrent genital herpes, as well as herpes localized on the lips.
Use during pregnancy and lactation
The use of acyclovir during pregnancy is possible in cases where the intended benefit to the mother exceeds the potential risk to the fetus.
If it is necessary to use during lactation, the question of stopping breastfeeding should be decided.
Contraindications
Hypersensitivity to acyclovir and valacyclovir; children and adolescents under 18 years of age. With caution: pregnancy; lactation (breastfeeding); dehydration; renal failure.

Local reactions: Â infrequently-there may be a burning sensation at the application site, which passes with time, dry skin, itching; rarely-erythema, contact dermatitis at the application site. If it gets on the mucous membranes, inflammation may occur.
From the immune system: Â very rarely – anaphylactic reactions, including angioedema and urticaria.
Interaction
When applied topically at the recommended doses, there was no systemic interaction of Gerperax ointment with other drugs.
How to take, course of use and dosage
Externally. Apply 4-6 times / day (every 4 hours) in a thin layer on the affected and adjacent skin areas. The drug is applied either with a cotton swab or with clean hands to avoid additional infection of the affected areas. Therapy should be continued until a crust forms on the vesicles, or until they completely heal. The duration of therapy is on average 5 days and should not exceed 10 days.
It is important to start treatment of recurrent infection during the prodromal phase or at the very beginning of the infection manifestation.
Duration of treatment – at least 5 days. If there is no healing, treatment can be continued for up to 10 days. If symptoms persist for more than 10 days, you should consult your doctor.
Overdose
Symptoms (if accidentally swallowed): headache, neurological disorders, shortness of breath, nausea, vomiting, diarrhea, impaired renal function, lethargy, convulsions, coma.
Treatment: maintenance of vital functions, hemodialysis.
Special instructions
Gerperax ointment should not be applied to the mucous membranes of the mouth, eyes, nose and vagina. The earlier treatment is initiated (at the first sign of infection), the more effective it will be.
To realize the therapeutic effect of acyclovir, the state of the body’s immune system is important. Patients with reduced immunity (with HIV/If the drug is used in patients with AIDS or after bone marrow transplantation) against the background of topical application of acyclovir ointment, systemic use of the drug should be prescribed, as well as in cases of severe recurrent herpes infection.
Acyclovir does not prevent sexual transmission of herpes, so during treatment it is necessary to refrain from sexual contact, even in the absence of clinical manifestations.
