Composition
Active ingredient: hydrocortisone acetate-0.5 g; Excipients: methylparahydroxybenzoate (methylparaben), petroleum jelly.
Pharmacological action
Pharmacodynamics Hydrocortisone is a synthetic glucocorticoid drug for external use. It has anti-inflammatory, decongestant, and antipruritic effects. It inhibits the release of cytokines (interleukins and interferon) from lymphocytes and macrophages, inhibits the release of inflammatory mediators by eosinophils, disrupts arachidonic acid metabolism and prostaglandin synthesis. Reduces inflammatory cell infiltrates, reduces the migration of white blood cells, including lymphocytes to the area of inflammation. Pharmacokineticsabsorption After application, the Active ingredient accumulates in the epidermis, mainly in the granular layer, and systemic absorption is insignificant. A small amount of hydrocortisone is absorbed into the systemic circulation unchanged. Metabolism Most of hydrocortisone is metabolized directly in the epidermis, and then in the liver. Excretionmetabolites and a small portion of unchanged hydrocortisone is excreted by the kidneys and through the intestines.
Indications
Superficial, uninfected, topical corticosteroid-sensitive skin diseases:
- dermatitis, including atopic, contact, seborrheic;
- eczema;
- psoriasis.

Use during pregnancy and lactation
Studies on the safety and efficacy of hydrocortisone in pregnant and lactating women have not been conducted. There are no exact data on the penetration of glucocorticosteroids into breast milk when applied topically. However, the risk cannot be completely excluded. The use of hydrocortisone in pregnant and lactating women is allowed only as prescribed by the attending physician, when the expected therapeutic effect for the mother exceeds the potential risk to the fetus and child.
Experimental studies have shown that corticosteroids can cause fetal developmental disorders. Currently, there is no clear confirmation of these data in humans.
Contraindications
Hypersensitivity to hydrocortisone and any of the excipients of the drug; wound, ulcerative skin lesions, bacterial, viral and fungal skin infections; tuberculosis, syphilis; rosacea, acne vulgaris, perioral dermatitis; skin tumors, post-vaccination reactions, children under 2 years of age. With caution
Diabetes mellitus, tuberculosis (systemic damage), application to the skin of the face and intertriginous skin, use of occlusive dressings, application to large areas of the skin, and/or long-term treatment (especially in children), pregnancy (application to large areas of the skin, long-term treatment), lactation (use in large doses or/and for a long time).
Side effects
Hyperemia, swelling, itching and irritation of the skin at the places where the ointment is applied. With prolonged use of a glucocorticosteroid, secondary infectious skin lesions, atrophic changes, and hypertrichosis may develop. When applied on large surfaces and / or using occlusion, symptoms of hypercorgicism (hyperglycemia, glucosuria, reversible suppression of the adrenal cortex, manifestation of Itsenko-Cushing syndrome) may develop, as a manifestation of the resorptive effect of hydrocortisone. Interaction
Barbiturates weaken the effect, diuretics increase hypokalemia. Reduces the effect of hypoglycemic agents.
With prolonged use of the drug with possible resorption of the drug into the general bloodstream, hydrocortisone reduces the effectiveness of insulin, oral hypoglycemic drugs, antihypertensive agents, anticoagulants, reduces the concentration of salicylates in the blood, reduces the concentration of praziquantel in the blood serum. Increase the risk of side effects of hydrocortisone: androgens, estrogens, oral contraceptives, anabolic steroids (hirsutism, acne); antipsychotics, carbutamide, azathioprine (cataracts); holinoblockers, antihistamines, tricyclic antidepressants, nitrates (glaucoma); diuretics (hypokalemia). When used with cardiac glycosides, digitalis intoxication may develop. Simultaneous use with lead and silver preparations leads to mutual inactivation.
How to take, course of use and dosage
Externally. The drug is preferably used for acute and subacute skin processes (without wetting). The drug is applied to the affected areas of the skin in a thin layer 1-3 times a day. To improve penetration, the drug is applied with light massaging movements. If there is a positive trend, the frequency of use of the drug can be reduced to 2-3 times a week. The course of treatment is 1-3 weeks. In children, the course of treatment should not exceed 7 days. If there is no improvement after treatment or new symptoms appear, you should consult your doctor. Use the drug only according to the method of use and in the doses indicated in the instructions. If necessary, please consult your doctor before using the medicine.
Overdose
Symptoms: Â excessive and frequent use during the day or use in children may cause systemic effects of the drug.
Treatment: Â when the drug is discontinued, the overdose phenomena go away on their own.
Special instructions
It is not recommended to exceed the course of treatment. If used for more than 2 weeks and there is a history of open – or closed-angle glaucoma, monitoring of intraocular pressure is necessary.
Application in pediatrics: Â it is possible to use Hydrocortisone for the treatment of children as prescribed by the attending physician, if the expected therapeutic effect exceeds the risk of possible side effects. Children are more likely to develop systemic effects of hydrocortisone than adults. Therefore, in children, the drug should be used carefully, if possible, in a short course (5-7 days).
Influence on the ability to drive motor vehicles and manage mechanisms: Â the drug does not limit psychophysical activity, the ability to drive vehicles and maintain moving mechanisms.
Patients who temporarily lose clear vision after applying the drug are not recommended to drive a car or work with complex equipment immediately after applying the drug.
