CORTICOSTEROIDS have anti-inflammatory and anti-allergic effects.
Reduces inflammatory cell infiltrates, reduces the migration of white blood cells and lymphocytes to the area of inflammation.
Stabilizes cellular and subcellular, including lysosomal and mast cell membranes. Reduces the binding of Ig to cell surface receptors and inhibits the synthesis or release of cytokines (interleukins and interferon) from lymphocytes and macrophages.
Reduces exudative reaction, helps to reduce capillary permeability. Reduces the severity of the early immunological response.
It has an antimetabolic effect and inhibits the development of connective tissue and scarring.
Indications
- allergic eye diseases (eyelid dermatitis, blepharitis, conjunctivitis and keratoconjunctivitis);
- inflammatory eye diseases of the anterior part of the eye in the absence of violation of the integrity of the corneal epithelium (blepharitis, conjunctivitis and keratoconjunctivitis);
- thermal and chemical burns (after complete epithelialization of corneal defects).
Contraindications
- purulent, viral, tuberculosis, fungal diseases of the eyes;
- glaucoma;
- trachoma;
- vaccination period;
- violation of the integrity of the membranes of the eye
Side effects
Allergic reactions, burning sensation, injection of sclera, short-term blurred vision.
With prolonged use for more than 10 days, the following symptoms may occur: :
– increased intraocular pressure with possible further development of steroid glaucoma with lesions of the optic nerve and impaired visual field (and therefore the application of preparations containing corticosteroids, more than 10 days it is necessary to regularly measure intraocular pressure);
– education of the rear subcapsular cataracts;
– slowing down the healing process (in diseases causing thinning of the cornea, its possible perforation). Secondary bacterial infection can occur as a result of suppressing the protective reaction of the patient’s body. In acute purulent diseases of the eye, glucocorticosteroids can mask or enhance the existing infectious process.
Fungal corneal lesions tend to occur especially frequently with prolonged use of glucocorticosteroids. The appearance of non-healing ulcers on the cornea after prolonged treatment with glucocorticosteroids may indicate the development of fungal invasion.
Interaction
With prolonged use of the drug with possible resorption of the drug into the general bloodstream, hydrocortisone reduces the effectiveness of insulin, oral hypoglycemic drugs, antihypertensive agents, anticoagulants, reduces the concentration of salicylates in the blood, reduces the concentration of praziquantel in the blood serum.
Increase the risk of side effects of hydrocortisone: androgens, estrogens, oral contraceptives, anabolic steroids (hirsutism, acne); antipsychotics, carbutamide, azathioprine (cataracts); holinoblockers, antihistamines, tricyclic antidepressants, nitrates (glaucoma); diuretics (hypokalemia). When used with cardiac glycosides, digitalis intoxication may develop.
Simultaneous use with lead and silver preparations leads to mutual inactivation.
How to take, course of use and dosage
Locally. 1-2 cm of eye ointment is injected into the conjunctival sac 2-3 times a day.
The course of treatment is 1-2 weeks. The course of treatment may be extended on the doctor’s recommendation.
Special instructions
Contact lenses are not recommended during treatment. When using eye drops, the time interval between instillation and application of the ointment is at least 15 minutes. If used for more than 10 days and there is a history of open – or closed-angle glaucoma, monitoring of intraocular pressure is necessary. Shelf life after opening the tube is 1 month. Influence on the ability to drive motor vehicles and manage mechanisms.
If the patient’s visual clarity is temporarily reduced after using the drug, it is not recommended to drive a car or engage in activities that require visual clarity until it is restored.
