Pharmacological action

Ichthyol ointment has a pronounced local anti-inflammatory and antiseptic effect, has an antipruritic and keratostatic effect.

Mechanism of action. The effect of ichthyol is due to the presence of thiophenol components (the main one is thiophene) containing up to 10.5% organically bound sulfur. Ichthyol thiophenes have a bactericidal effect against gram-positive microorganisms (including Staphylococcus aureus, Streptococcus pyogenes), as well as yeast-like fungi due to denaturation of the transmembrane proteins of their cells. Ichthyol has no effect on gram-negative microorganisms.

Immediately after applying the ointment, its weakly irritating effect is manifested in the form of a feeling of warmth, hyperemia at the site of applying the drug. It is associated with the direct activation of pain and thermoreceptors in the tissue by thiophenol components.

After stimulation of pain receptors, impulses are transmitted along sensitive nerve fibers to the spinal cord, where they include trophic reflexes of the sympathetic nervous system. Sympathetic pulsation stimulates blood circulation in soft tissues and skin, enhances the processes of regeneration and metabolism in the skin. Subsequently (after 1-2 hours), the anti-inflammatory and analgesic effect of the ointment on the underlying layers of soft tissues at the site of application is manifested.

The local anti-inflammatory effect of ichthyol is associated with a decrease in the synthesis and release of inflammatory mediators, acceleration of their metabolism. Ichthyol inhibits the migration of leukocytes to the inflammatory site in response to chemotactic factors – C5 and the compliment component, leukotriene B4 and the tripeptide Met-Leu-Phe.

Eliminates itching of the skin and excessive keratinization of the skin, due to the violation of the inclusion of new protein molecules in the composition of keratin. At the same time, the skin becomes less dense, its elasticity improves, excessive peeling disappears, keratinized dense growths in places of pressure and friction.

Ichthyol has a photoprotective effect and reduces the skin’s sensitivity to UV radiation.

Pharmacokinetics

After applying the podoclusal dressing to undamaged skin, it is slowly absorbed. Systemic absorption is 5-10%. The absorbed part of the thiophenol components of ichthyol is released into the bile and removed from the body through the intestines.

Indications

• Burns, erysipelas, eczema;
• diskoidna lupus erythematosus, rosacea, eczema solar Ismatova smallpox as a sunscreen composition of complexdatatype;
• infiltrative-suppurative form of Microsporum and trihofitii;
• the streptococcal (impetigo), stafilodermii (ostiofollikulit, sycosis, boils and hidradenitis);
• arthritis and neuralgia traumatic and vospitatelnogo;
• prostatitis, metritis, parametritis, salpingitis and drogeological diseases of the pelvic organs.

Contraindications

Hypersensitivity.

Interaction

It enhances the effect of other photoprotective agents(phenylsalicylate, PABA and dioxomethyltetrahydropyrimidine), however, it is pharmaceutically incompatible with zinc oxide (it forms a precipitate), so during photoprotective therapy it should not be used in conjunction with creams and ointments containing zinc oxide.

Incompatible in solutions and lotions with iodide salts, alkaloids, and heavy metal salts.

Dimethyl sulfoxide, ethyl alcohol, and glycerin, when applied simultaneously with ichthyol ointment, enhance the systemic absorption of ichthyol and increase its depth of penetration into soft tissues in the area of application.

How to take, course of use and dosage

Externally, the ointment is applied to the affected areas of the skin 1-2 times a day, if necessary, use a bandage.
