Composition
Active ingredient: Indometacin – 100 mg. Auxiliary substances: colloidal silicon dioxide (aerosil), solid fat, castor oil, urea (urea), propyl parahydroxybenzoate (nipazole), polysorbate (tween 80).
Pharmacological action
Indometacin has anti-inflammatory, analgesic, antipyretic and antiplatelet effects. Suppresses the activity of pro-inflammatory factors, reduces platelet aggregation.
By inhibiting cycloxygenase 1 and 2, it disrupts arachidonic acid metabolism, reduces the amount of prostaglandins (Rd) both in the inflammatory focus and in healthy tissues, and suppresses the exudative and proliferative phases of inflammation.
Causes weakening or disappearance of rheumatic and non-rheumatic pain syndrome (including joint pain at rest and during movement, reduction of morning stiffness and joint swelling, helps to increase the volume of movement; in inflammatory processes that occur after operations and injuries, quickly relieves both spontaneous pain and pain during movement, reduces inflammatory edema at the wound site).
Indications
Inflammatory and degenerative diseases of the musculoskeletal system: rheumatoid, psoriatic, juvenile chronic arthritis, arthritis in Paget and Reiter’s disease, neuralgic amyotrophy (Personage-Turner disease), ankylosing spondylitis (Ankylosing spondylitis), gouty arthritis (with an acute attack of gout, fast-acting dosage forms are preferred), rheumatism.
Pain syndrome: headache (including menstrual syndrome) and toothache, lumbago, sciatica, neuralgia, myalgia, after injuries and surgical interventions, accompanied by inflammation, bursitis and tendinitis (most effective when localized in the shoulder and forearm).
Algodismenorrhea; Bartter’s syndrome (secondary hyperaldosteronism); pericarditis (symptomatic treatment); childbirth (as an analgesic and tocolytic agent); pelvic inflammatory processes, including Adnexitis.
Non-blocking of the Botall bayou.
Infectious and inflammatory diseases of ENT organs with severe pain syndrome (as part of complex therapy): pharyngitis, tonsillitis, otitis media.
Febrile syndrome (including with lymphogranulomatosis, other lymphomas and hepatic metastases of solid tumors) – in case of ineffectiveness of ASA and paracetamol. The drug is intended for symptomatic therapy, reducing pain and inflammation at the time of use, and does not affect the progression of the disease.
Contraindications
Hypersensitivity;
bronchial asthma, urticaria or acute rhinitis, triggered by the reception of ask or other NSAIDs;
gastric ulcer and 12 duodenal ulcer, inflammatory bowel disease (ulcerative colitis, Crohn’s disease), bleeding (including intracranial, from the digestive tract), congenital heart disease (severe coarctation of the aorta, the pulmonary artery atresia, severe tetralogy of Fallot), period after coronary artery bypass grafting;
disorders of blood clotting (including hemophilia, prolonged bleeding time, bleeding tendency), liver failure, active liver disease;
Chronic renal failure (CC less than 30 ml/min), progressive kidney disease, disorders of the blood (leukopenia and anemia), confirmed hyperkalemia;
pregnancy, lactation;
for rectal use, rectal bleeding, proctitis, hemorrhoids, children’s age (under 14 years).
With caution. IHD, cerebrovascular diseases, CHF, dyslipidemia, hyperlipidemia, diabetes mellitus, thrombocytopenia, diabetes mellitus, peripheral arterial diseases, arterial hypertension, smoking, CRF (creatinine clearance 30-60 ml/min), cirrhosis of the liver with portal hypertension, hyperbilirubinemia, a history of gastrointestinal ulceration, the presence of H. pylori infection, long-term use of NSAIDs, frequent alcohol consumption, severe somatic diseases; concomitant therapy with the following drugs: anticoagulants (including warfarin), antiplatelet agents (including acetylsalicylic acid, clopidogrel), oral corticosteroids (including prednisone), selective serotonin reuptake inhibitors (including citalopram, fluoxetine, paroxetine, sertraline); mental disorders, epilepsy, parkinsonism, depression, elderly age.
Side effects
From the digestive system: Â nausea, anorexia, vomiting, abdominal pain and discomfort, constipation or diarrhea, erosive and ulcerative lesions, bleeding and perforation of the gastrointestinal tract; rarely-intestinal strictures, stomatitis, gastritis, flatulence, bleeding from the sigmoid colon or diverticula, jaundice, hepatitis.
From the central nervous system and peripheral nervous system: Â dizziness, headache, depression, fatigue; rarely-anxiety, fainting, drowsiness, seizures, peripheral neuropathy, muscle weakness, involuntary muscle movements, sleep disorders, mental disorders (depersonalization, psychotic episodes), paresthesia, dysarthria, parkinsonism.
From the cardiovascular system: Â edema, increased blood pressure, tachycardia, chest pain, arrhythmia, palpitation, hypotension, congestive heart failure, hematuria.
Allergic reactions: Â rarely-pruritus, urticaria, angiitis, erythema nodosum, skin rash, exfoliative dermatitis, Stevens-Johnson syndrome, erythema multiforme, toxic epidermal necrolysis, hair loss, acute respiratory distress, sharp drop in blood pressure, anaphylactic reactions, angioedema, dyspnoea, bronchial asthma, pulmonary edema.
From the hematopoietic system: Â rarely-leukopenia, petechiae or ecchymoses, purpura, aplastic and hemolytic anemia, thrombocytopenia, DIC syndrome.
From the side of the senses: Â rarely – impaired visual clarity, diplopia, orbital and periorbital pain, tinnitus, hearing disorders, deafness.
From the urinary system: Â rarely-proteinuria, nephrotic syndrome, interstitial nephritis, impaired renal function, renal failure.
From the side of metabolism: Â rarely – hyperglycemia, glucosuria, hyperkalemia.
Other services: Â rarely-vaginal bleeding, hot flashes, increased sweating, nosebleeds, enlarged and strained mammary glands, gynecomastia.
Local reactions: Â at the site of intravenous use, in some cases – the formation of an infiltrate, abscess; with rectal use, irritation of the rectal mucosa, tenesmus, exacerbation of chronic colitis are possible.
For external use: Â itching, redness, rash at the application site.
Interaction
When used concomitantly, Indometacin may reduce the effects of saluretics, beta-blockers, and enhance the effects of indirect anticoagulants.
With the simultaneous use of Indometacin and diflunizal, there is a risk of severe bleeding from the gastrointestinal tract.
When used concomitantly with probenecid, the concentration of Indometacin in blood plasma may increase.
Indometacin can reduce the tubular secretion of methotrexate, which leads to an increase in its toxicity.
Concomitant use with NSAIDs increases the toxicity of cyclosporine.
Indometacin at a dose of 50 mg 3 times / day increases the concentration of lithium in blood plasma and reduces the clearance of lithium from the body in patients with mental illnesses.
When Indometacin is co-administered with digoxin, it is possible to increase the concentration of digoxin in blood plasma and increase the half-life of digoxin.
How to take, course of use and dosage
Set individually, taking into account the severity of the disease. For adults, when taken orally, the initial dose is 25 mg 2-3 times a day. If the clinical effect is not sufficiently pronounced, the dose is increased to 50 mg 3 times / day. Dosage forms of prolonged action are used 1-2 times / day. Maximum daily dose: 200 mg.
If the effect is achieved, treatment is continued for 4 weeks at the same or reduced dose. With prolonged use, the daily dose should not exceed 75 mg. Take after meals.
For the treatment of acute conditions or relief of exacerbation of the chronic process,60 mg is administered intravenously 1-2 times a day. The duration of intravenous use is 7-14 days. Then Indometacin is administered orally or rectally at 50-100 mg 2 times a day, while the maximum daily dose should not exceed 200 mg. For maintenance treatment, apply rectally 50-100 mg 1 time/day at night.
