Pharmaceutical action:
Locoid lipocrem is a synthetic non-halogenated glucocorticoid preparation for external use. Esterification of the hydrocortisone molecule with butyric acid residue (butyrate) allowed a radical increase in the activity of the drug compared to native hydrocortisone. It has a fast-acting anti-inflammatory, decongestant, and antipruritic effect.
The use of Locoid Lipocrem® in the recommended doses does not cause suppression of the hypothalamic-pituitary-adrenal system. Although the use of the drug in high doses for a long time, especially when using occlusive dressings, can lead to an increase in the content of cortisol in blood plasma, this is usually not accompanied by a decrease in the reactivity of the pituitary-adrenal system, and the withdrawal of the drug leads to a rapid normalization of cortisol production.

Absorption After application, the Active ingredient accumulates in the epidermis, mainly in the granular layer. Systemic absorption is negligible. A small amount of hydrocortisone 17-butyrate is absorbed into the systemic circulation unchanged.
Metabolism Most of hydrocortisone 17-butyrate is metabolized to hydrocortisone and other metabolites directly in the epidermis and, subsequently, in the liver.
Excretionmetabolites and a small portion of unchanged hydrocortisone 17-butyrate are excreted by the kidneys and through the intestines.

Local reactions: skin irritation; in rare cases-other local reactions characteristic of corticosteroid therapy.
Systemic reactions: long-term use, application to large surfaces and/or use of occlusive dressings may cause side effects that are characteristic of systemic corticosteroids.
The risk of local and systemic side effects with Locoid Lipocrem® is lower than with fluoridated steroids.
Locoid is applied topically.
The affected skin surface is smeared with a thin layer no more than 3-4 times a day. The course of treatment is 1-3 weeks. The dose of the drug used for 1 week should not exceed 30-60 g.
If there is itching in the anus, before applying the drug, wash the affected area with warm water and soap, then with water and dry the skin surface with a towel or napkin.
For wet skin diseases, lesions of the scalp, a lotion and an emulsion for external use are used.
Avoid contact with the ointment in the eyes.
For children under 12 years of age, the drug is prescribed only under strict medical supervision. If after 7 days of use there is no improvement or deterioration of the condition, as well as if symptoms resume several days after discontinuation, the drug should be discontinued and a doctor should be consulted.
Do not use the drug without consulting a doctor in cases where itching in the genital area in women is accompanied by vaginal discharge.
For long-term treatment and / or application on large surfaces, it is recommended to prescribe a diet with a limited Na+ and high K+ content; introduce a sufficient amount of protein into the body. It is necessary to monitor blood pressure, the concentration of glucose and cortisol in the blood, blood clotting, diuresis, and the patient’s body weight.
