Pharmacodynamics
Locoid® is a synthetic non-halogenated corticosteroid for external use. Esterification of the hydrocortisone molecule with butyric acid residue (butyrate) allowed a radical increase in the activity of the drug compared to native hydrocortisone. Locoid®has a rapidly advancing anti-inflammatory, anti-edematous, antipruritic effect.
The use of Locoid® in the recommended doses does not cause suppression of the hypothalamic-pituitary-adrenal system. Although the use of high doses of the drug for a long time, especially when using occlusive dressings, can lead to an increase in the content of cortisol in blood plasma, this is usually not accompanied by a decrease in the reactivity of the pituitary-adrenal system, and the withdrawal of the drug leads to a rapid normalization of cortisol production.
Pharmacokinetics
Suction
After application, the Active ingredient accumulates in the epidermis, mainly in the granular layer, and systemic absorption is insignificant. A small amount of hydrocortisone 17-butyrate is absorbed into the systemic circulation unchanged.
Metabolism
Most of hydrocortisone 17-butyrate is metabolized to hydrocortisone and other metabolites directly in the epidermis and subsequently in the liver.
Deduction
Metabolites and a small portion of unchanged hydrocortisone 17-butyrate are excreted by the kidneys and through the intestines.
Local reactions: Â skin irritation, in rare cases-other local reactions characteristic of local steroid therapy.
Systemic reactions: Â long-term use, application to large surfaces, and/or the use of occlusive dressings may cause side effects that are characteristic of systemic corticosteroids.
The risk of local and systemic side effects with Locoid® is lower than with fluoridated steroids.
Externally. Locoid® in the form of an ointment is used for subacute and chronic processes, especially in the presence of lichenification, infiltration, dryness.
The drug is applied to the affected areas of the skin in a thin layer 1-3 times a day. If there is a positive trend, the frequency of use of the drug can be reduced to 2-3 times a week.
To improve penetration, the drug is applied with massaging movements.
In cases of a resistant course of diseases, for example, with the localization of dense psoriatic plaques on the elbow areas, knees, the drug can be used under occlusive dressings.
The dose of the drug used during the week should not exceed 30-60 g.
Symptoms: Â with prolonged use of the drug in high doses, it is possible to increase the side effects described above. If symptoms of hypercorticism occur, the drug should be discontinued.
Locoid® should not be applied to the periorbital area due to the risk of glaucoma. If the conjunctiva is systematically exposed, there is a risk of increased intraocular pressure.
Any corticosteroids for external use in the form of an ointment should not be used on the scalp, as well as in acute inflammation (especially exudative). For the treatment of steroid-sensitive skin lesions located in the face, folds, genitals, as well as on areas of the skin with abundant hair, it is preferable to use Locoid®, cream and Locoid Crelo®.
The drug should not be used in the presence of atrophic skin changes.
The risk of local and systemic side effects increases when applied to large areas of the lesion, prolonged use, the use of occlusive dressings and in childhood.
If there is no effect during 2 weeks of continuous treatment, the diagnosis should be clarified.
As with the use of any corticosteroid, it is advisable to limit the duration of use and the course dose to the minimum sufficient to stop the skin process.
Application in pediatrics
It can be used in children from 6 months of age. If the drug is used in children, in the face area or under occlusive dressings, the duration of treatment should be reduced.
When used in children, the surface area of the skin on which the drug is applied should not exceed 20% of the total.
In childhood, suppression of the adrenal cortex can develop faster. In addition, there may be a decrease in the excretion of growth hormone. When using the drug for a long time, it is necessary to monitor body weight, height, and plasma cortisol levels. In a study in children who received 30-60 g of Locoid® per week for 4 weeks, in comparison with the use of 1% hydrocortisone ointment, no child was found to have impaired adrenal cortex function, i. e. no differences between hydrocortisone 17-butyrate in this indicator and conventional hydrocortisone were observed.
