Pharmacological action
Pharmaco-therapeutic group: Antidiarrheal agent
Pharmacological action
An antidiarrheal agent. Reduces the tone and motility of intestinal smooth muscles, apparently due to binding to opioid receptors in the intestinal wall. Inhibits the release of acetylcholine and prostaglandins, reducing peristalsis and increasing the time of passage of contents through the intestine.
Increases the tone of the anal sphincter. The action comes quickly and lasts 4-6 hours.
Indications
- Symptomatic treatment of acute and chronic diarrhea (genesis: allergic, emotional, drug, radiation; with changes in the diet and quality of food, with impaired metabolism and absorption). As an auxiliary drug for diarrhea of infectious origin.
- Regulation of stool in patients with ileostoma.
Use during pregnancy and lactation
Contraindicated in the first trimester of pregnancy. In the second and third trimesters of pregnancy, loperamide can be prescribed in cases where the expected benefit of therapy for the mother exceeds the potential risk to the fetus.
Since a small amount of loperamide is found in breast milk, taking it during breastfeeding is not recommended.
Contraindications
- Hypersensitivity to the drug,
- lactose intolerance,
- lactase deficiency or glucose-galactose malabsorption,
- diverticulosis,
- intestinal obstruction,
- ulcerative colitis in the acute stage,
- diarrhea on the background of acute pseudomembranous enterocolitis,
- as monotherapy dysentery and other infections of the gastrointestinal tract;
- pregnancy (I trimester),
- the period lactations,
- loperamide capsules are not indicated for children under 6 years.
With caution: liver failure.
Side effects
From the digestive system: Â abdominal pain, bloating, nausea, constipation, dry mouth.
From the central nervous system: Â increased fatigue, drowsiness, dizziness.
Allergic reactions: Â skin rash.
Interaction
It is believed that simultaneous use of colestyramine may reduce the effectiveness of loperamide.
When used concomitantly with co-trimoxazole, ritonavir, the bioavailability of loperamide increases, which is due to inhibition of its metabolism during the “first pass” through the liver.
In case of loperamide overdose, naloxone is used as an antidote.
How to take, course of use and dosage
Inside, washed down with water. Adults and children over 6 years of age: Acute diarrhea: the initial dose is 2 capsules (4 mg) for adults and 1 capsule (2 mg) for children, then one capsule (2 mg) after each act of defecation in case of loose stools. Chronic diarrhea: the initial dose is -2 capsules (4 mg) per day for adults and 1 capsule (2 mg) for children; then the initial dose should be adjusted so that the frequency of normal stool is 1-2 times a day, which is usually achieved with a maintenance dose of 1 to 6 capsules (2-12 mg) per day. The maximum daily dose for acute and chronic diarrhea in adults is 8 capsules (16 mg); the maximum daily dose in children is calculated based on body weight (3 capsules per 20 kg of body weight of the child), but should not exceed 8 capsules (16 mg). If the stool is normalized or if there is no stool for more than 12 hours, the drug is canceled.
Overdose
Symptoms: Â central nervous system depression (stupor, impaired coordination, drowsiness, miosis, muscle hypertension, respiratory depression), intestinal obstruction. Treatment: the antidote is naloxone; given that the duration of action of loperamide is longer than that of naloxone, repeated use of the latter is possible. Symptomatic treatment: activated charcoal, gastric lavage, artificial ventilation. Medical supervision is required for at least 48 hours.
Special instructions
Use with caution in patients with hepatic insufficiency.
It should not be used in clinical situations where suppression of intestinal motility is required.
If there is no effect after 2 days of using loperamide, it is necessary to clarify the diagnosis and exclude the infectious genesis of diarrhea.
Use with caution in patients with impaired liver function.
In case of loperamide overdose, naloxone is used as an antidote.
