Patients with severe CRF (creatinine clearance less than 30 ml / min) and severe hepatic insufficiency:Â 5 mg 1 time a day daily or 10 mg 1 time a day every other day. Elderly patients-no dose adjustment is required.

Erythromycin and cstoconazole (inhibitors of the CYP3A4 isoenzyme), cimetidine (inhibitor of the CYP3A4 and CYP2D6 isoenzymes) increase the Cmax of loratadine and its active metabolite in the blood. There are no clinically significant changes. Loratadine reduces the Cmax of erythromycin in plasma by 15%.

Symptoms: headache, drowsiness, tachycardia. Extrapyramidal disorders and palpitations were observed in children weighing less than 30 kg when ingested at a dose of more than 10 mg.
Treatment: gastric lavage, taking activated charcoal; symptomatic and supportive therapy. For children,0.9% sodium chloride solution is used for washing, for adults, water can be used for washing.
Saline laxative solutions and enterosorbents are indicated for rapid elimination of loratadine from the gastrointestinal tract. Loratadine is not eliminated by hemodialysis or peritoneal dialysis.
Loratadine should be taken with caution in patients with severe hepatic insufficiency, as loratadine clearance may be reduced in such patients. The safety and efficacy of loratadine in children under 3 years of age is unknown, so the drug is contraindicated for use in this age group.
Treatment with loratadine should be discontinued at least 48 hours prior to skin allergy testing, as antihistamines may hide or reduce positive skin reactions. Loratadine can cause dry mouth in some patients, so people at risk should have oral hygiene because of the risk of tooth decay. Children under 12 years of age with a body weight of less than 30 kg should use loratadine in lower doses.
Influence on the ability to drive motor vehicles and manage mechanisms
When using loratadine, caution should be exercised when driving vehicles and operating complex mechanisms, given that during use, side effects such as drowsiness and dizziness may develop, affecting the ability to concentrate and psychomotor reactions.
