Composition

Active ingredient: meldonium dihydrate (trimethylhydrazinium propionate dihydrate) – 500 mg. Auxiliary substances: potato starch, colloidal silicon dioxide (aerosil), calcium stearate.

Pharmacological action

Meldonium, an analog of gamma-butyrobetaine, inhibits gamma-butyrobetaine hydroxynase, reduces carnitine synthesis and transport of long-chain fatty acids through cell membranes, and prevents the accumulation of activated forms of non-oxidized fatty acids-derivatives of acylcarnitine and acyl coenzyme A. As a result of a decrease in the carnitine concentration, gamma-butyrobetaine, which has vasodilating properties, is intensively synthesized. Under conditions of ischemia, it restores the balance of oxygen delivery and its consumption in cells, prevents disruption of ATP transport; at the same time, it activates glycolysis, which proceeds without additional oxygen consumption. The mechanism of action determines the variety of its pharmacological effects: increased performance, reduction of symptoms of mental and physical overstrain, activation of tissue and humoral immunity, cardioprotective effect. In the case of acute ischemic damage to the myocardium, it slows down the formation of a necrotic zone, shortens the rehabilitation period. In heart failure, it increases myocardial contractility, increases exercise tolerance, and reduces the frequency of angina attacks. In acute and chronic ischemic disorders of the cerebral circulation, it improves blood circulation in the focus of ischemia, promotes blood redistribution in favor of the ischemic area. It is effective in the case of vascular and dystrophic fundus pathology. It is also characterized by a tonic effect on the central nervous system, elimination of functional disorders of the somatic and autonomic nervous systems in patients with alcoholism during abstinence.

Indications

As part of the combined therapy of ischemic heart disease, chronic heart failure, cardialgia against the background of dyshormonal myocardial dystrophy.

Use during pregnancy and lactation

It is not recommended during pregnancy (the effectiveness and safety of use has not been studied). If it is necessary to prescribe the drug during lactation, stop breastfeeding.

Contraindications

Hypersensitivity to the drug, increased intracranial pressure (with impaired venous outflow and intracranial tumors), pregnancy, lactation, age up to 18 years (efficacy and safety have not been established).

Side effects

Rarely-allergic reactions (redness of the skin, itching, edema), dyspeptic phenomena, tachycardia, agitation, decreased arterial pressure.

Interaction

Increases the effect of coronary dilating and some antihypertensive drugs, cardiac glycosides. It can be combined with antianginal agents, anticoagulants, antiplatelet agents, antiarrhythmics, diuretics, bronchodilators. Due to the possible development of moderate tachycardia and arterial hypotension, caution should be exercised when combined with peripheral vasodilators and antihypertensive agents (including nifedipine, alpha-blockers).

How to take, course of use and dosage

Inside, before meals.

Stable angina, chronic heart failure – as part of a complex therapy of 500-1000 mg per day, using the entire dose at once or dividing it by 2 times. The course of treatment is 4-6 weeks.

Cardialgia on the background of dyshormonal myocardial dystrophy-250 mg 2 times a day (morning and evening). The course of treatment is 12 days.

Disorders of blood supply to the brain (stroke, cerebrovascular insufficiency) – 500-1000 mg per day, preferably in the morning. The course of treatment is 4-6 weeks. Alcohol withdrawal syndrome-500 mg 4 times a day. The course of treatment is 7-10 days.

In case of reduced working capacity and chronic physical overstrain, in the postoperative period to speed up the rehabilitation process – 250 mg 4 times a day. The course of treatment is 10-14 days. If necessary, the treatment is repeated after 2-3 weeks. Athletes are prescribed 500-1000 mg 2 times a day before training.
