Micardis is hypotensive.
Blocks angiotensin II receptors (type AT1) and eliminates its vasoconstrictive effect. Reduces the system time Blood pressure, the concentration of aldosterone in plasma.
Pharmacokinetics
When taken orally, it is rapidly absorbed from the gastrointestinal tract. Bioavailability — 50%. When taken concomitantly with food, the decrease in AUC ranges from 6% (at a dose of 40 mg) to 19% (at a dose of 160 mg).3 hours after use, the plasma concentration is equalized regardless of taking the drug on an empty stomach or with food. Cmax and AUC were 3 and 2 times higher in women compared to men, respectively, without a significant effect on concentration. Binding to plasma proteins is 99.5%, mainly with albumin and alpha-1 glycoprotein. The average apparent volume of distribution at the equilibrium stage is 500 liters. It is metabolized by conjugation with glucuronic acid. The metabolites are pharmacologically inactive. T1/2 — more than 20 hours. Total plasma Cl is high (900 ml / min) compared to hepatic blood flow (about 1500 ml / min). It is excreted through the intestines in unchanged form, excretion by the kidneys — less than 2%.
The maximum effect develops in 3 hours after a single dose. The antihypertensive effect persists for more than 24 hours, including the last 4 hours before taking the next dose. A stable clinical result is achieved after 4-8 weeks of course application and is maintained for a long time. When abruptly stopping taking blood pressure gradually (over several days) returns to its original values without the manifestation of “rebound” hypertension.
Arterial hypertension.
Hypersensitivity, impaired biliary tract patency, severe liver or kidney function disorders, hereditary fructose intolerance, pregnancy, breastfeeding, childhood and adolescence.
Nervous system disorders: Â headache, dizziness, fatigue, insomnia, anxiety, depression, seizures.
Respiratory system disorders: Â upper respiratory tract infections (including pharyngitis, sinusitis, bronchitis), cough.
From the cardiovascular system:  pronounced decline Blood pressure, bradycardia, tachycardia, chest pain.
From the digestive system: Â nausea, dyspepsia, diarrhea, abdominal pain, increased activity of “hepatic” transaminases.
From the musculoskeletal system: Â myalgia, arthralgia, low back pain, tendinitis-like symptoms.
From the urinary system: Â peripheral edema, urinary tract infections, hypercreatininemia.
Allergic reactions: Â skin rash, etc.
Laboratory parameters: Â rarely-hyperkalemia, anemia, or hyperuricemia.
Other services: Â flu-like syndrome, rarely-erythema, pruritus, syncope, dyspnoea, eosinophilia, thrombocytopenia, angioedema, urticaria.
Thiazide diuretics (e. g. hydrochlorothiazide) enhance the antihypertensive effect of telmisartan. Telmisartan enhances the antihypertensive effect of other antihypertensive drugs, increases the concentration of digoxin in the blood.
Inside, regardless of food intake. Adults — 40 mg 1 time a day. In some patients, a therapeutic effect can be achieved by using a dose of 20 mg / day. If there is no reduction Blood pressure to the desired level, the dose can be increased to 80 mg once a day. The maximum effectiveness of the antihypertensive effect of the drug is usually noted 4-8 weeks after the start of treatment.
Patients with severe arterial hypertension — up to 160 mg / day or in combination with hydrochlorothiazide 12.5-25 mg / day.
Symptoms: Â pronounced lowering of blood pressure.
Treatment: Â symptomatic therapy; hemodialysis is ineffective.
Impact on the ability to drive a car and machinery
A special study of the effect of the drug on the ability to drive a car and machinery has not been conducted.
However, when driving a car and machinery, you should be aware of the possibility of developing dizziness and drowsiness when using drugs used to treat arterial hypertension.
