Pharmacological action
Anticongestive agent – alpha-adrenomimetic
Product Code: R01AA04
Pharmacological Properties
A drug for the symptomatic local treatment of rhinitis. Phenylephrine hydrochloride is an alpha-1-adrenergic receptor agonist (sympathomimetic), which has a vasoconstrictive effect by stimulating alpha-1 receptors in the nasal mucosa, reduces mucosal swelling and tissue hyperemia, congestion in the nasal mucosa, and also improves the patency of the nasal airways. Restoration of nasopharyngeal air patency improves the patient’s well-being and reduces the risk of possible complications caused by stagnation of mucosal secretions.
Pharmacokinetics
When applied topically, systemic absorption is low.
Indications
Symptomatic treatment: to facilitate nasal breathing in cases of colds, flu, hay fever or other allergic diseases of the upper respiratory tract, accompanied by acute rhinitis or sinusitis.
Contraindications
- Increased individual sensitivity to the ingredients of the drug,
- diseases of the cardiovascular system (including severe atherosclerosis, angina pectoris, tachycardia)
- diseases of the thyroid gland (thyrotoxicosis)
- diabetes mellitus
- arterial hypertension
- concomitant use of MAO inhibitors (as well as 2 weeks after their withdrawal)
- children under 4 years of age.
Use caution for children under 6 years of age.
Side effects
Local reactions: burning, tingling, or tingling sensation in the nose.
System effects: headache, dizziness, palpitations, arrhythmia, high blood pressure, sweating, pallor, tremor, sleep disturbance.
Interaction
Monoamine oxidase inhibitors (procarbazine, selegiline), tricyclic antidepressants, maprotiline, guanedrel, and guanethidine enhance the pressor effect and arrhythmogenicity of phenylephrine (with systemic absorption).
Thyroid hormones with systemic absorption of phenylephrine increase the (mutually) associated risk of coronary insufficiency (especially in coronary atherosclerosis).
How to take, course of use and dosage
Intranasally.
For children aged 6-12 years-2-3 injections in each nasal passage, no more than every 4 hours.
The duration of treatment without consulting a doctor is no more than 3-5 days.
Overdose
It can be manifested by cardiac arrhythmias, increased blood pressure, and agitation.
In case of overdose, you should consult a doctor.

On the safety and effectiveness of nasal vasoconstrictor drugs in pediatric practice. / Voprosy prakticheskoi pediatrii [Questions of Practical pediatrics],2010, vol. 5, no. 5, p. 6569. Phenylephrine has a selective agonism to 1-adrenergic receptors, has a mild vasoconstrictor effect, presumably due to the predominant effect on the obturator choke vessels (superficial arterioles, nasal mucosa), and does not cause a significant decrease in blood flow in it.
Special instructions
In children, systemic absorption of phenylephrine and the associated risk of side effects are higher than in adults. The duration of use of the drug should not exceed 3 days, while maintaining nasal breathing difficulties, further use of the drug should be agreed with the doctor.
Phenylephrine should not be prescribed to patients within 2 weeks after discontinuation of monoamine oxidase inhibitors, as they may increase the severity of the adrenergic effects of sympathomimetic agents and increase the risk of side effects from the cardiovascular system.
It is not recommended to use the same bottle for several people, in order to avoid the spread of infection. When the bottle is compressed in an upright position, the solution is released from it in the form of a spray, when the bottle is compressed in an inverted position – drop by drop.
