Pharmacological action
It stimulates alpha-adrenergic receptors: in low concentrations it has mainly an α2-adrenomimetic effect, in high concentrations it also acts on α1-adrenergic receptors.
Narrows blood vessels at the application site. When applied intranasally, it reduces swelling of the mucous membrane of the upper respiratory tract, facilitates nasal breathing, and reduces blood flow to the venous sinuses. When instilled in the conjunctival sac, it reduces the swelling of the conjunctiva.
The action begins 15 minutes after application, duration-6-8 hours
. Pharmacokinetics
When applied topically (spray), oxymetazoline undergoes a small degree of systemic absorption, so its concentration in blood plasma is insignificant.
It is mainly excreted unchanged in the urine and feces. T1 / 2 is 5-8 days.
Indications
Acute rhinitis (including allergic rhinitis), vasomotor rhinitis, paranasal sinusitis, eustachiitis, otitis media; for narrowing the vessels of the mucous membrane for diagnostic purposes (under the supervision of a doctor).
Use during pregnancy and lactation
The drug is contraindicated for use during pregnancy and lactation.
Contraindications
- atrophic rhinitis;
- pregnancy;
- lactation (breastfeeding);
- children under 6 years of age;
- hypersensitivity to the components of the drug.
With caution, the drug should be prescribed for chronic heart failure, arrhythmia, severe atherosclerosis, arterial hypertension, thyrotoxicosis, diabetes mellitus, chronic renal failure, angle-closure glaucoma.
Side effects
Local reactions: Â transient dryness and burning of the nasal mucosa, dry mouth and throat, sneezing. With prolonged use – tachyphylaxis, reactive hyperemia of the mucous membrane and atrophy of the nasal mucosa.
Systemic reactions: Â increased blood pressure, increased anxiety, nausea, dizziness, headache, insomnia, palpitations, sleep disorders.
The interaction
slows down the absorption of local anesthetic drugs, prolongs their effect.
Co-use of other vasoconstrictor medications increases the risk of side effects.
With simultaneous use of monoamine oxidase blockers and tricyclic antidepressants – increased blood pressure.
How to take, course of use and dosage
Nesopin is prescribed intranasally.
Adults and children over 12 years of age – 2-3 injections in each nasal passage with an interval of 10-12 hours.
Children from 6 to 12 years – 1 injection in each nasal passage.
Overdose
Symptoms: Â tachycardia, increased blood pressure, central nervous system depression.
Treatment: Â symptomatic.
Special instructions
In the recommended dosage, you can use it for no more than 3 days without consulting a doctor.
Avoid contact with the drug intended for intranasal use in the eyes.
The drug has an effect on vision (if it gets into the eyes), it may reduce the reaction rate.
Dosage levels that exceed the recommended values may affect the ability to drive a vehicle or equipment.
