procaine hydrochloride (novocaine) 100 mg;
excipients: bases for suppositories: solid fat (vitepsol, supposir) – a sufficient amount before receiving a suppository weighing 1.25 g
Pharmacological action
Pharmacotherapy group: Local anesthetic ATX code: N01baPharmacodynamics : A local anaesthetic agent with moderate anaesthetic activity and a wide range of therapeutic effects. Being a weak base, it blocks sodium channels and prevents the generation of impulses in the endings of sensitive nerves and the conduction of impulses along nerve fibers. Changes the action potential in nerve cell membranes without a pronounced effect on the resting potential. Suppresses carrying out not only painful but also impulses of other modality.
Upon absorption and direct vascular introduction into the blood stream reduces the excitability of peripheral cholinergic systems reduces the formation and release of acetylcholine from preganglionic endings (has a weak ganglioblocking effect) eliminates smooth muscle spasm reduces the excitability of the myocardium and motor areas of the cerebral cortex.
Pharmacokinetics: Poorly absorbed through the mucous membranes. It is mainly excreted by the kidneys in the form of metabolites in unchanged form, no more than 2% is excreted.
Indications
Hemorrhoid cracks in the anus.
Contraindications
Hypersensitivity (including to paraaminobenzoic acid and other local anesthetic esters).
With caution:
Children’s age (up to 12 years) in elderly patients (over 65 years) pregnancy and lactation.
Side effects

Interaction
Local anesthetics enhance the depressing effect on the central nervous system of other drugs.
Anticoagulants (ardeparin dalteparin danaparoid enoxaparin heparin warfarin) they increase the risk of bleeding.
Use with monoamine oxidase inhibitors (furazolidone procarbazine selegiline) increases the risk of hypotension.
Enhance and lengthen the effect of muscle-relaxing drugs.
Vasoconstrictors (epinephrine methoxamine phenylephrine) prolong the local anesthetic effect.
Procaine reduces the antimiasthenic effects of drugs, especially when used in high doses, which requires additional correction of the treatment of myasthenia gravis.
Cholinesterase inhibitors (antimiasthenic drugs cyclophosphamide demecarin ecothiophate thiotep) reduce the metabolism of local anesthetic drugs.
The procaine metabolite (paraaminobenzoic acid) is a sulfonamide antagonist.
How to take, course of use and dosage
Rectally.
Having previously released the suppository from the contour cell packaging, it is inserted deep into the anus (after a cleansing enema or spontaneous release of the intestine) 1-2 times a day.
Overdose
Symptoms: pallor of the skin and mucous membranes dizziness nausea vomiting “cold” sweat increased breathing tachycardia decreased blood pressure up to collapse apnea methemoglobinemia. The effect on the central nervous system is manifested by a feeling of fear hallucinations convulsions motor excitement.
Treatment: maintenance of adequate pulmonary ventilation with oxygen inhalation intravenous use of short-acting drugs for general anesthesia in severe cases-detoxification and symptomatic therapy.
