Composition
Timolol (in the form of maleate) – 5 mg
Pharmacological action
Non-selective beta-blocker for topical use in ophthalmology. It does not have internal sympathomimetic and membrane-stabilizing activity.
The drug reduces both normal and elevated intraocular pressure by reducing the formation of intraocular fluid. It has no effect on pupil size and accommodation.
The effect of the drug is manifested 20 minutes after instillation in the conjunctival sac. The maximum decrease in intraocular pressure develops in 1-2 hours and persists for 24 hours.
Indications
-increased intraocular pressure (ophthalmohypertension)
— – open-angle glaucoma;
– secondary glaucoma;
– to reduce intraocular pressure in closed-angle glaucoma (as an additional agent in combination with myotics);
— congenital glaucoma (if other therapeutic measures are insufficient).
Contraindications
— asthma or other severe chronic obstructive airway disease;
sinus bradycardia;
— AV-block II and III degree;
acute heart failure;
— severe chronic heart failure (III and IV functional class NYHA classification);
— cardiogenic shock;
— allergic reactions, generalized skin eruptions;
— atrophic rhinitis;
— dystrophic diseases of the cornea;
— the neonatal period;
— hypersensitivity to the components of the drug.
With caution, the drug is prescribed to patients with pulmonary insufficiency, emphysema, severe cerebrovascular insufficiency, heart failure (NYHA functional class I and II), sinoatrial blockade, arterial hypertension, diabetes mellitus, hypoglycemia, thyrotoxicosis, myasthenia gravis, Reye’s syndrome, while prescribing other beta-blockers, as well as children.
Side effects
From the side of the visual organ: Â irritation and hyperemia of the conjunctiva, eyelid skin, burning and itching in the eyes, lacrimation or decreased lacrimation, photophobia, corneal epithelial edema, spot surface keratopathy, corneal hypesthesia, diplopia, ptosis, dry eyes, short-term visual acuity, blepharitis, conjunctivitis, keratitis. When performing surgical interventions for glaucoma, detachment of the choroid of the eye may develop in the postoperative period.
From the cardiovascular system: Â chest pain, heart failure, bradycardia, bradiarrhythmia, decreased blood pressure, collapse, AV block, cardiac arrest, transient cerebral circulatory disorders.
Respiratory system disorders: Â rhinitis, shortness of breath, bronchospasm, and pulmonary insufficiency.
From the central nervous system and peripheral nervous system: Â headache, dizziness, weakness, depression, paresthesia, myasthenia gravis, drowsiness, hallucinations, ringing in the ears, slowing down the speed of psychomotor reaction.
From the digestive system: Â nausea, vomiting, diarrhea.
Allergic reactions: Â urticaria, eczema.
Other services: Â nosebleeds, decreased potency, alopecia.
Interaction
Joint use of the drug Ocumed with eye drops containing epinephrine can cause dilation of the pupil.
The decrease in intraocular pressure increases with the simultaneous use of eye drops containing epinephrine and pilocarpine. Instill in the eyes of two beta-blockers should not be.
The decrease in blood pressure and slowing of the heart rate may be increased when the drug is co-administered with slow calcium channel blockers, reserpine and other beta-blockers.
Concomitant use with insulin or oral antidiabetic agents may result in hypoglycemia.
Timolol enhances the effect of peripheral muscle relaxants (it is necessary to cancel the drug 48 hours before the planned surgical intervention using general anesthesia).
How to take, course of use and dosage
Adults and children over 10 years of age are instilled 1 drop (0.25% eye drops) into the conjunctival sac of the affected eye 2 times/day, with insufficient effectiveness – 1 drop (0.5% eye drops) 2 times/day. With normalization of intraocular pressure, the maintenance dose is 1 drop (0.25% eye drops) 1 time/day.
Children under 10 years of age are prescribed 1 drop (0.25% eye drops) 2 times a day.
The drug is intended for long-term continuous use.
The patient should be warned that only the doctor can change the dose or temporarily cancel the drug.
Overdose
Symptoms: Â due to local absorption, systemic effects characteristic of beta-blockers may develop (dizziness, headache, arrhythmia, bradycardia, bronchospasm, nausea, vomiting).
Treatment: Â immediately flush eyes with water or saline solution. Conduct symptomatic therapy.
Special instructions
The patient should be warned about the need for regular measurement of intraocular pressure and corneal examination, as well as about the need to stop using the drug and consult a doctor if side effects occur.
When wearing soft contact lenses, Okumed eye drops should not be used, as the preservative contained in them can be deposited in soft contact lenses and have an adverse effect on the eye tissue.
Hard contact lenses should be removed before instillation of the drug and re-installed after 15 minutes.
When transferring patients to treatment with Okumed, refractive correction may be necessary (after the effects of previously used myotics disappear).
Okumed should be discontinued 48 hours prior to general anesthesia surgery.
Okumed should not be used simultaneously with antipsychotic (neuroleptics) and anxiolytic (tranquilizers) drugs.
During the period of use of the drug, it is not recommended to drink alcohol (a sharp decrease in blood pressure is possible).
Use in pediatrics
There is no sufficient experience in using the drug in children, so the drug can only be prescribed in cases where the intended benefit of therapy exceeds the potential risk of side effects.
Influence on the ability to drive motor vehicles and manage mechanisms
Immediately after using the drug, visual clarity may decrease and psychomotor reactions may slow down, which may reduce the ability to engage in potentially dangerous activities that require increased attention (especially when drinking alcohol at the same time).
