Pharmacological action

An indirect anticoagulant. Causes hypoprothrombinemia associated with a violation of the prothrombin formation process in the liver, also causes a decrease in the formation of VII, IX, X blood clotting factors, a decrease in plasma tolerance to heparin, blood lipid content and increased vascular permeability. The effect occurs in 8-10 hours and reaches a maximum in 24-30 hours after taking phenindione.

Indications

Prevention of thromboembolism (including in case of myocardial infarction, after surgical interventions); thrombosis of coronary vessels, thrombophlebitis and deep vein thrombosis of the lower extremities; prevention of thrombosis after surgery for prosthetic heart valves (permanent admission).

Contraindications

Hemophilia, hypocoagulation, I trimester of pregnancy, hypersensitivity to phenindione.

Side effects

From the digestive system:  nausea, diarrhea, impaired liver function, toxic hepatitis.

From the hematopoietic system:  inhibition of bone marrow hematopoiesis.

From the urinary system:  impaired renal function.

From the cardiovascular system:  myocarditis.

From the side of the blood coagulation system:  with prolonged use – micro-and macrohematuria, bleeding from the oral cavity and nasopharynx, from the gastrointestinal tract, muscle hemorrhages.

From the central nervous system:  headache.

Allergic reactions:  skin rash, dermatitis, eosinophilia, fever.

Interaction

When used concomitantly with ACTH, the anticoagulant effect increases.

When used concomitantly with dipyridamole, cases of increased bleeding have been described, apparently due to a decrease in platelet adhesiveness and aggregation under the influence of dipyridamole.

When used concomitantly with clofibrate, the anticoagulant effect increases. It is believed that fibrates may increase the affinity of oral anticoagulants for the corresponding receptors or, possibly, disrupt the metabolism of anticoagulants.

When used concomitantly with liothyronine, the anticoagulant effect of phenindione increases. It is believed that thyroid hormones can increase the affinity of oral anticoagulants for the corresponding receptors.

When used concomitantly with miconazole, the anticoagulant effect of phenindione increases. The risk of bleeding increases.

When used concomitantly with cimetidine, the anticoagulant effect of phenindione increases due to a slowdown in its metabolism in the liver under the influence of cimetidine, which is an inhibitor of microsomal liver enzymes.

When used concomitantly with ethylestrenol, the anticoagulant effect increases, and there is a risk of bleeding.

How to take, course of use and dosage

Inside on day 1, the daily dose is 120-180 mg in 3-4 doses, on day 2-90-150 mg, then 30-60 mg/day, depending on the blood prothrombin content. Withdrawal of phenindione should be carried out gradually.

Special instructions

With caution, the drug should be used in elderly patients, with hepatic and / or renal insufficiency, pulmonary embolism (including in oncology), erosive and ulcerative lesions of the gastrointestinal tract, with pericarditis, in the postpartum period.

Treatment should be carried out under close medical supervision with mandatory systematic testing of the blood levels of prothrombin and other clotting factors. General urinalysis is regularly performed for early detection of hematuria.

In some patients, the palms of the hands turn orange and the urine turns pink, which is associated with the metabolism of phenindione.
