Pharmacological action
Phezam is a combined drug that improves cerebral circulation and brain metabolism.
Piracetam is a nootropic agent. It has a positive effect on the metabolic processes and blood circulation of the brain. Increases glucose utilization, improves the course of metabolic processes, improves microcirculation in ischemic areas, inhibits aggregation of activated platelets. It has a protective effect on brain damage caused by hypoxia.
Cinnarizine is a calcium channel blocker with a pronounced effect on the blood vessels of the brain. Improves cerebral blood circulation, increases the resistance of tissues to hypoxia. Reduces the excitability of the vestibular apparatus.
Both components mutually potentiate each other’s action. With regard to the effect of the drug on the central nervous system, the sedative effect of cinnarizine prevails. The toxicity of the combination does not exceed that of the individual components. The therapeutic effect manifests itself in 1-6 hours.
Pharmacokinetics
Absorption is rapidly and completely absorbed from the gastrointestinal tract. Cmax of cinnarizine in plasma is noted after 1-4 hours, piracetam – after 2-6 hours.
Distribution High concentrations of cinnarizine are observed in the liver, kidneys, heart, lungs, spleen, and brain after 1-4 hours. Cinnarizine binds 91% to plasma proteins.
Piracetam is rapidly distributed to all vital organs. It penetrates the blood-brain barrier and is mainly concentrated in the gray matter of the cerebral cortex and cerebellum, as well as in the nucleus caudatus (caudate nucleus), hippocampus (hippocampus), corpus medianum, geniculus lateralis and plexus choroidalis (choroid plexus).
Elimination Piracetam is excreted unchanged in the urine in approximately 30 hours. 60% of cinnarizine in unchanged form is excreted in the faeces, the rest – in the urine in the form of metabolites in about 5 hours.
Indications
- Insufficiency of cerebral circulation (atherosclerosis of cerebral vessels, recovery period of ischemic and hemorrhagic strokes, traumatic brain injuries, encephalopathy of various origins).
- Intoxication.
- Other diseases of the central nervous system, accompanied by a decrease in intellectual and mnestic functions (memory, attention, mood disorders).
- Psychoorganic syndrome with a predominance of signs of asthenia and adynamia, asthenic syndrome.
- Labyrinthopathies, Meniere’s syndrome.
- Intellectual development lag in children.
- Prevention of migraines and kinetoses.
Use during pregnancy and lactation
Phezam is contraindicated for use during pregnancy and lactation.
Contraindications

Interaction
When used concomitantly with Phezam, it is possible to increase the effect of sedatives, tricyclic antidepressants, nootropic and antihypertensive drugs, as well as ethanol.
When used concomitantly, vasodilators enhance the effect of Phezam, and hypertensive agents reduce it.
How to take, course of use and dosage
Adults are prescribed 1-2 capsules 3 times a day for 1-3 months, depending on the severity of the disease.
Children from 5 years of age are prescribed 1-2 capsules 1-2 times a day.
Special instructions
Since Phezam contains cinnarizine, the drug can cause a positive reaction in athletes during a doping test. When using the drug, it is possible to change the results of determining radioactive iodine (dyes in the capsule shell contain iodine). Phezam is the drug of choice in cases where piracetam monotherapy causes tension and insomnia in the patient.
Influence on the ability to drive motor vehicles and manage mechanisms
Taking Phezam in therapeutic doses is possible (if the underlying disease allows) when driving vehicles and working with machinery and equipment.
