Plendil – antihypertensive, antianginal.
Blocks calcium channels, inhibits the flow of calcium ions into the smooth muscle cells of arterial vessels, causes their expansion, reduces OPSS, lowers blood pressure, afterload on the heart, increases coronary blood flow, improves the balance of oxygen supply/consumption by the myocardium.
Pharmacokinetics
It is completely absorbed from the gastrointestinal tract. Systemic bioavailability — 15%. It binds to plasma proteins by 99%. The plasma concentration is directly proportional to the dose. It is metabolized in the liver. T1/2-24 h. 70% is excreted in the urine (less than 0.5% of the dose is unchanged), the rest is excreted in the feces.
Arterial hypertension, angina pectoris.
It is contraindicated during pregnancy and lactation.
Hypersensitivity, decompensated heart failure, acute myocardial infarction, unstable angina, pregnancy, childhood.
Often (more than 1%): Â headache, hot flashes, peripheral edema.
Sometimes (0.1-1%): Â tachycardia, palpitations, fatigue, dizziness, paresthesia, nausea, rash, itching.
Rare (0.01-0.1%): Â fainting, vomiting, arthralgia, myalgia, sexual disorders, impotence, urticaria.
Very rare (less than 0.01%): Â gum hyperplasia, increased liver enzymes, photosensitization, pollakiuria, allergic reactions (fever, angioedema).
Biotransformation inhibitors (cimetidine) increase plasma concentrations, stimulants (phenytoin, carbamazepine, barbiturates)Â – reduce it.

The usual maintenance dose of Plendil is 5 to 10 mg once a day. In elderly patients, the possibility of starting treatment with a daily dose of 2.5 mg should be considered.
Angina pectoris. The dose should be selected individually. The initial dose is 5 mg once a day, if necessary, increase the dose to 10 mg once a day.
Symptoms: Â severe hypotension, bradycardia.
Treatment:  transfer of the patient to the supine position; symptomatic therapy — atropine (iv 0.5-1 mg), alpha-1-adrenomimetics, glucose, saline solution or dextran by infusion.
