Pharmacological action
Prepidil-stimulating labor activity.
It stimulates smooth muscles, modulates the response of internal organs to various hormonal influences.
Increases blood supply to the cervix, promotes its softening and opening.
Pharmacokinetics
After endocervical use of the gel, cmax is reached in 30-45 minutes, and then decreases rapidly.
Indications
Preparation of the cervix for childbirth.
Use during pregnancy and lactation
Any dose of the drug that causes a prolonged increase in uterine tone poses a certain risk to the embryo or fetus.
In experimental animal studies, prostaglandin E2 causes an increase in skeletal abnormalities in rats and rabbits, and the embryotoxicity of dinoprostone in rats and rabbits was also revealed.
Prostaglandins are excreted in breast milk in very small amounts, regardless of the time of delivery.
Contraindications
Hypersensitivity, history of cesarean section, extensive uterine surgery, difficult and / or traumatic delivery,6 or more full-term pregnancies, mismatch between the size of the mother’s pelvis and the fetal head, abnormal fetal position, previous fetal distress, opening of the fetal bladder, spotting during pregnancy from the genital tract of unspecified etiology, infections of the lower genital tract.
Side effects
Abnormal uterine contractions, vomiting and / or diarrhea, changes in fetal heart rate, fetal distress, reduced status of the newborn (Apgar score below 7).
Interaction
Increases the toxotropic activity of oxytocin.
How to take, course of use and dosage
The contents of the syringe (3 g of gel corresponds to 0.5 mg of dinoproston) are inserted into the cervical canal (below the level of the internal pharynx) using the attached catheter. After the procedure, you need to lie on your back for 10-15 minutes to minimize the flow of gel.
If the desired result from the use of Prepidil is achieved, the recommended interval before the appointment of intravenous oxytocin is 6-12 hours. If the response to the initial dose of Prepidil is not achieved, you can re-prescribe it. The recommended repeated dose is 0.5 mg, and the interval from the previous use is 6 hours.
The need for an additional appointment and the interval should be determined by the doctor, based on the clinical situation. The maximum recommended dose for a 24-hour period is 1.5 mg.
Overdose
Treatment: Â symptomatic, intravenous use of beta-adrenomimetics or rapid delivery.
Special instructions
The introduction of the gel into the extra-amniotic space should be avoided (hyperstimulation of the uterus is possible).
