Composition
One tablet contains: active ingredients: paracetamol-500 mg, chlorphenamine maleate-2 mg, caffeine-30 mg, phenylephrine hydrochloride-10 mg. excipients: microcrystalline cellulose, corn starch, povidone K 30, talc, magnesium stearate, sodium carboxymethyl starch (type A), red dye [Ponceau 4R], methyl parahydroxybenzoate.
Pharmacological action
Means for eliminating the symptoms of acute respiratory infections and “colds” (analgesic non-narcotic agent + alpha-adrenostimulator + H1-histamine receptor blocker + psychostimulant). ATX code: N02BE71 Combined agent, has antipyretic, analgesic, alpha-adrenostimulating, vasoconstrictor and antihistamine effects, eliminates the symptoms of a “cold”. Caffeine stimulates the psychomotor centers of the brain, enhances the effect of analgesics, eliminates drowsiness and fatigue, increases physical and mental performance, shortens reaction time, temporarily reduces fatigue and drowsiness. Paracetamol is a non-narcotic analgesic that blocks cyclooxygenase, mainly in the central nervous system (CNS), affecting the centers of pain and thermoregulation; it has analgesic and antipyretic effects. Phenylephrine is an alpha-adrenomimetic with a moderate vasoconstrictor effect. Chlorphenamine is a blocker of H1-histaminoreceptors. Narrows the nasal vessels, eliminates swelling and hyperemia of the nasal mucosa, nasopharynx and paranasal sinuses; reduces exudative manifestations, suppresses the symptoms of allergic rhinitis.
Indications
Symptomatic treatment of “colds”, flu, acute respiratory viral infections, infectious and allergic rhinitis (fever syndrome, pain syndrome, rhinorrhea).
Contraindications
Hypersensitivity, severe atherosclerosis of the coronary arteries, increased blood pressure, diabetes mellitus, pheochromocytoma, pregnancy, lactation, children under 14 years of age. With caution – glucose-6-phosphate dehydrogenase deficiency; hepatic and/or renal failure, angle-closure glaucoma, prostatic hyperplasia; congenital hyperbilirubinemia (Gilbert, Dubin-Johnson and Rotor syndromes), thyrotoxicosis, COPD (emphysema of the lungs, chronic bronchitis), simultaneous use of tricyclic antidepressants and monoamine oxidase inhibitors (MAO) and beta-blockers.
Side effects
Allergic reactions (skin rash, pruritus, urticaria, angioedema), nausea, epigastric pain; anemia, thrombocytopenia, agranulocytosis. Increased excitability, dizziness, increased blood pressure, sleep disturbance. Mydriasis, paresis of accommodation, increased intraocular pressure, dry mouth, nausea, vomiting, epigastric pain; urinary retention. With prolonged use in large doses – hepatotoxic effect, hemolytic or aplastic anemia, methemoglobinemia, pancytopenia; nephrotoxicity (renal colic, glucosuria, interstitial nephritis, papillary necrosis).
Interaction
Enhances the effects of MAO inhibitors, sedatives, and ethanol. Ethanol enhances the sedative effect of antihistamines. Antidepressants, antiparkinsonian and antipsychotic drugs, and phenothiazine derivatives increase the risk of urinary retention, dry mouth, and constipation. Glucocorticosteroids increase the risk of glaucoma. Paracetamol reduces the effectiveness of uricosuric drugs. Chlorphenamine simultaneously with MAO inhibitors, furazolidone can lead to a hypertensive crisis, agitation, hyperpyrexia. Tricyclic antidepressants enhance the adrenomimetic effect of phenylephrine, while concomitant use of halothane increases the risk of ventricular arrhythmia. Reduces the hypotensive effect of guanethidine, which, in turn, increases the alpha-adrenostimulating activity of phenylephrine.
How to take, course of use and dosage
Inside, adults and children over 14 years of age – 1 tablet with an interval of 4-6 hours, but no more than 4 tablets per day. If there is no relief of symptoms within 3 days after starting the drug, you should consult a doctor.
Overdose
Symptoms: paracetamol (when taken over 10-15 g) – pallor of the skin, decreased appetite, nausea, vomiting, toxic hepatitis, up to the development of hepatonecrosis, increased activity of “liver” transaminases, increased prothrombin time. Treatment: symptomatic, gastric lavage, use of SH-group donors and glutathione – methionine synthesis precursors 8-9 hours after overdose and N-acetylcysteine-after 12 hours.
Special instructions
During the treatment period, peripheral blood parameters and liver function are monitored. During treatment, it is necessary to refrain from driving vehicles and engaging in potentially dangerous activities that require increased concentration of attention and speed of psychomotor reactions, you can not take ethanol, sleeping pills and anxiolytic drugs (tranquilizers). Do not take together with other medicines containing substances that are part of the drug. If symptoms persist within 3 days, consult a doctor.
